Possibilities and efficacy of secondarily restoration of inveterate defects of the extensors tendons of fingers, owing various extent, were studied in conditions of cicatricial changes in the hand back skin. There was established, that in ischemic and reperfusion damage of the hand tissues the coarse cicatricial changes in the end portions of the traumatized tendons occurs, essentially influencing surgical tactics. Optimal operative techniques, directed on the extensors tendons restoration in various extent of their defects and various severity of the soft tissues changes in the hand back were…

This study reports results in 12 patients treated with "completely vascularized single-stage approaches," so defined because skin, tendon, and nerve are transferred as a compound flap, and all are vascularized. A free dorsalis pedis cutaneotendinous flap was used in 7 patients, while a radial forearm cutaneotendinous island flap was transposed in 5 patients. A dorsalis pedis flap provides four vascularized extensor tendons (extensor digitorum comunis tendons), and the radial artery flap permits the inclusion of one completely vascularized tendon (palmaris longus) and two "strips" of vasculariz…

We reviewed 9 (3-15) years postoperatively all 43 two-stage flexor tendon transplantations in fingers that had not otherwise been severely injured that we had operated on during the years 1984 to 1996. One digit had been reconstructed in each patient. The interval between the first and second stage operations was 19 (14-51) weeks. Time away from work was 44 (4-140) days after the first procedure and 101 (38-297) days after the second stage operation. After the second stage, 26 further procedures had been done in 18 of the 43 fingers. These included seven resutures of the transplanted tendon (t

Long cattle tendons were enzymatically disposed of lipoid and accompanying nitrogenous substances. Collagenous hydrolysate of average molecular mass M(a) = 0.830 +/- 1.206 kDa, suitable for application in skin and hair-care preparations, was obtained through hydrolysis by commercial protease (Bacillus subtilis) under mild conditions. Elastin, hydrolysable with greater difficulty, was subsequently hydrolysed by commercial endopeptidase (Bacillus licheniformis), under more drastic conditions, to a product of average molecular mass M(a) = 0.448 +/- 1.514 kDa. Purity of both hydrolysates was asses…

MRI is valuable in evaluation of tendons and ligaments in sports-related injuries. Tendons and ligaments show dark signal intensity on all MR sequences. Alteration in size, shape, continuity, and signal intensity of these structures can be well visualized with MRI. Impingement syndrome is the leading cause of atrophy and degeneration of the rotator cuff. Shape of the acromion, presence of subacromial spur, and involvement in certain sport activities that lead to shoulder instability may cause impingement of the rotator cuff. This article systematically reviews the injuries of tendons and ligam…

Injuries to the extensor and flexor tendons are among the most common injuries to the hand (cf. Part 1 of this overview). Severance of individual flexor tendons as also isolated injuries to individual nerves may occasionally be overlooked. In the case of doubt, operative revision is to be recommended. With the aid of dynamic fixation as described by Kleinert, good functional results can be achieved even with injuries "in no-man's-land". In the event of infections following minor injuries, conservative treatment should be applied only in the early stage; thereafter, surgical revision is indicat…

The outcome of 15 patients who sustained simultaneous laceration of the median and ulnar nerves with flexor tendons at the wrist is described. Primary nerve repair yielded satisfactory results, the median nerve achieving a better outcome than the ulnar nerve. Most patients regained a functional range of wrist movement. Flexor tendons yielded the poorest results. A functional, albeit impaired outcome can usually be anticipated following this severe injury.

Tendon injuries in humans often lead to significant functional impairments, necessitating effective regeneration methods. While mammalian tendon healing typically results in scar formation and functional loss, certain species such as axolotls exhibit remarkable regenerative abilities. This study investigates axolotl tendon regeneration following complete transection (tenotomy) and volumetric loss (tenectomy). Axolotl tendons, identified through histological and gene expression analyses, consist of collagens and exhibit specific tendon markers. Our results indicated complete regeneration of tra…

Macrophages and other phagocytic cells are central regulators of tendon immunobiology, orchestrating inflammation, tissue repair, and extracellular matrix (ECM) remodeling in the tendons. They derive from circulating monocytes and resident tendon-specific populations, including tenophages. Macrophage polarization along the M1/M2 axis exerts a decisive influence on tendon healing trajectories. Activated M1 macrophages promote the early healing phase for debris clearance initiating the reparative cascade. However, their sustained activity leads to inflammation, ECM degradation, impaired healing,…

Chronic Achilles tendon rupture is a difficult condition to treat due to tendon retraction and histologic changes. Traditional treatments include primary repair (for defects <2 cm), VY advancement (using gastrocnemius aponeurosis for larger gaps), Achilles tendon turndown (using gastrocnemius fascia, often with flexor hallucis longus transfer), allografts, and tendon transfers (using peroneus brevis or flexor hallucis longus tendons). New synthetic graft options, such as acellular collagen matrices and Artelon, may improve chronic Achilles tendon rupture healing without the morbidity associate…

Our understanding of tendon homeostasis and repair following injury has made great strides through basic science, clinical veterinary medicine, and preclinical translational research. We have now gleaned a greater understanding of the cellular and matrix dynamics that are being orchestrated within the tendon, which provide therapeutic opportunities. This article will focus on how utilizing the horse and equine tissues have advanced our understanding of tendinopathy and the cellular and matrix dynamics at play while also identifying continued gaps in our knowledge where the horse as a veterinar

Although the Achilles tendon is a highly robust tendon, it is susceptible to ruptures and overuse. To date, there are no definitive or conclusive methods of management for the various pathologic conditions that can adversely affect the tendon. Endoscopic techniques have gained popularity in recent years because of their minimally invasive nature, improved cosmetic outcomes, relatively rapid recovery, and low morbidity. Several endoscopic methods have been developed to treat the various injuries and pathologic conditions that can affect the Achilles tendon. The outcomes of such measures have be

Quadriceps tendon ruptures (QTRs) are uncommon lower limb injuries that lead to pain, loss of active knee extension, and a prolonged rehabilitation course. They typically affect middle-aged or older adults, particularly in patients with comorbidities or degenerative tendon changes, and they most often occur during eccentric contraction when the knee suddenly flexes against an active quadriceps muscle. QTR is a clinical diagnosis. Although history and examination are the cornerstones of diagnosis, imaging is commonly used to confirm the diagnosis and to stage injury severity, with magnetic reso

Musculoskeletal ultrasound (MSKUS) is a pivotal imaging modality for the evaluation and management of Achilles tendon pathologies. Its ability to provide real-time, high-resolution imaging facilitates accurate diagnosis, dynamic assessment, and precise therapeutic interventions. The Achilles tendon, the largest and strongest tendon in the body, is critical for lower limb function and prone to a variety of pathologies, particularly in athletes and active individuals. This paper explores the normal sonographic anatomy of the Achilles tendon, common pathological findings-including tendinopathy, t

Giant cell tumor of the tendon sheath (GCT-TS) is a benign, slow-growing lesion originating from the tendon sheath or adjacent bursal tissue. We present a series of four histologically confirmed cases with atypical presentations in the foot and hand. Clinical features included a slow-growing, painless mass, often associated with prior trauma. All patients underwent surgical excision, with histopathology confirming GCT-TS. This report emphasizes the importance of considering GCT-TS in the differential diagnosis of soft-tissue swellings in tendon-rich areas and highlights the role of surgery as
