Infantile subglottic hemangioma treated by intralesional steroid injection: report of one case.

Infantile subglottic hemangioma is a rare, life threatening disease of the larynx. Various treatments have been described in the literature, including tracheostomy, systemic steroids, intralesional steroid injection, carbon dioxide laser therapy, and interferon alfa-2a. The best treatment, however, remains controversial. We report a case of congenital subglottic hemangioma successfully managed with a single intralesional steroid injection, which resulted in rapid involution of the hemangioma without recurrence after 18 months of follow up. This therapy appears to minimize both short and long t

Evaluation and management of bilateral vocal cord immobility.

Bilateral vocal cord immobility can be life threatening for some patients. Others, who have an open glottic chink, may have a breathy dysphonia, intermittent dyspnea, and stridor. These signs and symptoms may also be found in a number of other conditions that cause weakness or paradoxical motion of the vocal cords that mimics paralysis. These other conditions include central nervous system diseases, neuromuscular disorders, laryngospasm, and psychogenic disorders. In addition, patients with cricoarytenoid joint immobility or interarytenoid scar can also have similar symptoms at presentation. I

Nager acrofacial dysostosis: management of a difficult airway.

Nager acrofacial dysostosis, first described by Nager and deReynier in 1948, is a rare syndrome characterized by mandibulofacial dysostosis … conservative management. This case and a review of the literature emphasize the importance of early tracheotomy for these patients.

Subglottic hemangiomas in infants.

Subglottic hemangioma, a lesion absent at birth and developing in the first few months of life, may present as an acute airway obstruction. Definitive diagnosis may be delayed or confused with other conditions. Recent experience with infants presenting with subglottic hemangioma lead to the proposal of delayed laser surgical intervention until age nine months, at which time natural involution begins. Five cases are reviewed to illustrate this contention.

A personal experience with subtotal and conservation surgery as treatment for laryngeal cancer.

A personal technique for laryngeal cancer reconstructive surgery is presented and discussed. The functional and therapeutic purpose of this surgery is to broaden its indications and to improve functional results. In particular, our surgical technique involves removal of the soft internal part of the cricoid cartilage (mucosa, submucosa and perichondrium), which is otherwise conserved. Satisfactory functional results can be achieved by: modeling of two symmetrical pseudoarytenoids; an anterior epiglottiplasty or the use of a Hiranandani base-of-the-tongue flap to close the anterior gap (if pres

Computed tomography of diffuse tracheal stenosis in Wegener granulomatosis.

A patient with Wegener granulomatosis presented with diffuse tracheal narrowing and was evaluated using CT and cine-CT. The CT findings included severe laryngeal and tracheal airway narrowing, due to abnormal soft tissue within the laryngeal cartilages and tracheal rings, and enlarged abnormally calcified tracheal cartilages. Computed tomography precisely confirmed the site, level, and submucosal extent of tracheal narrowing, not obtainable on physical or bronchoscopic examination, and assisted in choosing a site for tracheostomy. Wegener granulomatosis should be considered in patients with di

Tracheal carcinoma following short term tracheostomy in childhood (a case report).

Carcinoma of the trachea is rare in comparison to other neoplasms of the respiratory tract. Cigarette smoking has been implicated in its aetiology, but unlike carcinoma of the bronchus and lung, the incidence of tracheal carcinoma has not risen with tobacco consumption (Hajdu et al., 1970; Ranke et al., 1962). It seems, then, that there is some mechanism or mechanisms which render the trachea especially resistant to malignant change.

A comparison of endotracheal intubation and tracheostomy in the management of acute epiglottitis in children in the tropics.

In children, acute epiglottitis is rapidly fatal due to airway obstruction by swollen supraglottic structures, but the oedema is short-lived. Therefore, in many centres short-term endotracheal intubation has replaced tracheostomy as the method of choice for relieving the obstruction. We have tried both methods in 15 children over a period of three years under conditions of limited material and human resources. Tracheostomy was found to be safer. It was concluded that it is better to have a live healthy child with a small scar in the neck than a dead child without a scar.

Nasotracheal intubation--a modality in the management of acute epiglottitis in adults.

Acute epiglottitis in adults is usually treated by close observation or tracheostomy. In the last decade, this therapeutic attitude has been challenged by the impressive results obtained by nasotracheal intubation in the treatment of pediatric cases. Many authors have suggested the use of nasotracheal intubation in adults as well as in children: it would be a natural complement to close observation when airway shunting becomes advisable thus rendering tracheostomy unnecessary. Three conditions have to be met before safe nasotracheal intubation can be considered: interested anesthesiological se

Tracheal atresia, proximal esophageal atresia, and distal tracheoesophageal fistula: report of two cases and review of literature.

Two cases with unusual primitive foregut anomalies are described. Both children, born within 24 hours of each other, had complete tracheal atresia, proximal esophageal atresia, and distal tracheoesophageal fistula along with some minor anomalies; both needed immediate tracheostomy for adequate ventilation. At 15 months of age, both patients are thriving and developing normally with the exception of their speech. A review of the literature revealed no other survivors with this condition. Diagnosis of tracheal atresia should be kept in mind when a newborn infant exhibits respiratory distress wit

Mucormycosis of the trachea: an unusual cause of acute upper airway obstruction.

Mucormycosis (phycomycosis) has been reported to involve most organ systems in man. We report a young insulin-dependent diabetic who presented with acute upper airway obstruction because of isolated mucormycosis of the trachea. A combination of amphotericin B and surgical resection of the lower two thirds of the larynx and five tracheal rings with primary reanastomosis has resulted in a cure with no evidence of recurrence after nine months of follow-up. To our knowledge, this represents the first report of isolated tracheal mucormycosis and/or acute upper airway obstruction due to mucormycosis

Cricothyrotomy in the emergency department.

Thirty-eight emergency cricothyrotomies were performed over a 3-year period. This was the first airway control maneuver attempted in 5 patients, 3 of whom had facial and/or neck injury, one apneic with upper airway hemorrhage, and one with aortobronchial fistula. The remaining 33 procedures were performed only after other airway management failed. Five indications were identified among these cases: 1) excessive emesis or hemorrhage (11), 2) possible cervical spine injury with airway compromise (9), 3) technical failure (7), 4) clenched teeth (5), and 5) masseter spasm following succinylcholine

Management of laryngotracheoesophageal cleft.

Laryngotracheoesophageal cleft (LTE) is a rare disorder that is still associated with a high mortality. Often the mortality is associated wi … E cleft. Initially the airway should be maintained either by placement of an endotracheal tube or a tracheotomy. The more entensive the cleft, the more easily does the indwelling tube slip over the edge of the defect into the esophagus. In all but the mildest of clefts, gastric decompression and alimentation are required. This still allows for considerable re …

Persistent pharyngeal membrane.

A 39-year-old man was examined for a severe speech impediment that was first noted in early childhood. Otolaryngologic examination demonstrated a membranous structure confluent from anterior pillar and from the soft palate to the base of the tongue. A review of the literature reveals this to be a persistent pharyngeal membrane, which is a rare congenital anomaly.

Surgical correction of complete tracheoesophageal cleft.

Laryngotracheal esophageal cleft is a rare congenital anomaly that should be considered in the differential diagnosis of any neonatal respiratory distress aggravated by feeding. Diagnosis is difficult and repeat endoscopic examinations may be necessary. In complete clefts, immediate surgical repair is indicated with combined approach through the chest and neck and incorporation of part of the esophageal wall into the repair. Because the cleft may be associated with esophageal atresia and/or tracheal esophageal fistula, it should be suspected in patients who have persistent symptoms after succe
