Use of Subcutaneous Injection of Epinephrine and Triamcinolone with Tranexamic Acid Reduces Blood Loss, Transfusion Rates, and Length of Stay in Open Sagittal Craniosynostosis Repair.

OBJECTIVE: In 2017, we adopted the use of triamcinolone/epinephrine (TAC/Epi) scalp injection and later added tranexamic acid (TXA) in open sagittal synostosis surgery. We believe that this reduced blood loss and transfusion rates. METHODS: A total of 107 consecutive patients operated for sagittal synostosis aged <4 months from 2007 to 2019 were retrospectively reviewed. We collected demographics [age, sex, weight at surgery, and length of stay (LOS)], intraoperative information [estimated blood loss (EBL)], administration of packed red blood cell, transfusion of plasmalyte/albumen, operating…

Does Combined Intravenous and Topical Tranexamic Acid Offer Additional Benefit Compared With Intravenous or Topical Alone in Open Elbow Arthrolysis-A Pilot Randomized Controlled Trial.

PURPOSES: Open elbow arthrolysis (OEA) is a common method for treating post-traumatic elbow stiffness, which may lead to severe bleeding when resecting heterotopic ossification (HO) and fibrous scar tissue. The purpose of this study is to compare the efficacy and safety among three different ways of using tranexamic acid (TXA) in reducing total blood loss and drainage volume in OEA. PATIENTS AND METHODS: This was a pilot, double-blinded, randomized controlled trial involving a total of 60 patients of post-traumatic elbow stiffness. Patients in the IV group (n = 20) received TXA intravenously,…

Targeting TRAF6/MYD88/TLR4, Nrf2, and VEGF signaling pathways by Tranexamic acid as a Novel Therapeutic Approach Protects Against Isotretinoin-Induced Liver Toxicity in Rats.

BACKGROUND: Isotretinoin is a widely prescribed retinoid for severe acne but is limited by dose-dependent hepatotoxicity. Tranexamic acid (TXA), an antifibrinolytic agent with emerging anti-inflammatory and antioxidant effects, may offer hepatoprotective potential. Aim: This study evaluated whether TXA mitigates isotretinoin-induced liver injury in rats and explored the underlying molecular mechanisms. METHODS: Thirty-two male Wistar rats were assigned to four groups (n = 8 each): control, TXA-only, isotretinoin (ISO, 7.5 mg/kg/day, 30 days), and TXA (100 mg/kg/day for 30 days) + ISO. Serum a…

A tranexamic acid-functionalized acellular dermal matrix sponge co-loaded with magnesium ions: Enhancing hemostasis, vascular regeneration, and re-epithelialization for comprehensive diabetic wound healing.

Excessive inflammation, accumulation of wound exudate, and blood seepage are common in diabetic wounds, hindering cell proliferation and disrupting tissue remodeling, leading to delayed healing. This study presents a multifunctional sponge scaffold (P5T3@Mg) created by combining an acellular dermal matrix with tranexamic acid and MgO nanoparticles, designed for hemostatic and anti-inflammatory effects. The P5T3@Mg scaffold effectively absorbs wound fluid while promoting healing. In vivo and in vitro hemostasis experiments demonstrate that the P5T3@Mg sponge exhibits excellent hydrophilicity, e…

Efficacy of high-dose versus low-dose tranexamic acid for reduction of blood loss in adolescent idiopathic scoliosis surgery: A systematic review and meta-analysis.

BACKGROUND: Recent studies have suggested that high-dose tranexamic acid (TXA) may be an effective method for reducing blood loss during adolescent idiopathic scoliosis (AIS) surgery. This study aims to perform a systematic review and meta-analysis to compare the outcomes of high-dose versus low-dose TXA for AIS surgery. METHODS: Searches were conducted in major databases such as PubMed, Scopus, Google Scholar, and Cochrane Library for relevant studies comparing high-dose and low-dose TXA outcomes in terms of blood loss, red blood cell transfusions, and hemoglobin changes. This systematic rev…

Blood-conserving and therapeutic efficacy of intravenous tranexamic acid at different time points after primary total knee arthroplasty with tourniquet application: a randomised controlled trial.

BACKGROUND: The use of a tourniquet in combination with tranexamic acid (TXA) not only ensures clear vision, reduces intraoperative blood loss and shortens operative time but also improves cement-bone inter-digitation in total knee arthroplasty (TKA). However, there is no proof whether the blood flow blocking effect of tourniquet affects the antifibrinolytic effect of TXA, and the optimal timing of TXA administration is still unclear. Therefore, this study aims to investigate the effect of the first dose of TXA administered intravenously before tourniquet compression and release in TKA on peri…

Topical use of tranexamic acid can reduce opioid consumption compared with intravenous use for patients undergoing primary total hip arthroplasty: a prospective randomized controlled trial.

BACKGROUND: The problem of opioid addiction after total hip arthroplasty (THA) has been widely concerned. Tranexamic acid (TXA) has been shown to be effective in reducing blood loss for patients undergoing THA, but few studies focus on its alleviation of postoperative local pain symptoms. The purpose of this study was to investigate whether topical TXA could reduce early postoperative hip pain for primary THA patients, thereby reducing the use of opioids, and whether local pain is related to inflammatory response. METHODS: In this prospective randomized controlled study, we randomly divided 1…

Tranexamic Acid Demonstrates Efficacy without Increased Risk for Venous Thromboembolic Events in Cranial Neurosurgery: Systematic Review of the Evidence and Current Applications in Nontraumatic Pathologies.

BACKGROUND: The cautionary stance normally taken towards tranexamic acid (TXA) is rooted in concerns regarding its complication profile, namely its purported risk for venous thromboembolic events (VTEs). In the present review, we intend to bring increased attention to TXA as a remarkably valuable tool that does not appear to increase the risk for VTE when used as indicated in select patients. METHODS: We queried three databases to identify reporting use of TXA during nontraumatic cranial neurosurgery procedures (excluded traumatic brain injury). Data gathered included VTE complications, deep …

Tranexamic Acid Reduces Total Blood Loss and the Amount of Stored Preoperative Autologous Blood Donation Needed for Adolescent Idiopathic Scoliosis Patients Undergoing Posterior Spinal Fusion.

Introduction There are few published studies on posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS) that have reported that the stored amounts of autologous blood donation (ABD) needed for the procedure were estimated by taking into account total blood loss (TBL). The aim of this study was to clarify the following clinical questions: (1) Does the use of tranexamic acid (TXA) reduce the TBL during PSF for AIS? (2) What volume of ABD should be stored to avoid allogeneic blood transfusions? Methods This study investigated 44 female patients who underwent PSF for AIS. A total o…

Cost-Benefit Analysis of Using A Single Dose of Tranexamic Acid in Degenerative Lumbar Scoliosis Patients Undergoing Long-Segment Spinal Fusion Surgery: A Retrospective Study.

BACKGROUND Degenerative lumbar scoliosis (DLS) patients undergoing posterior long-segment spinal fusion surgery often require perioperative blood transfusions, and previous studies have reported that increased complications and additional costs accompany these transfusions. One method for decreasing transfusions is the administration of tranexamic acid (TXA). We sought to evaluate the costs and benefits of preoperative administration of 1 g of intravenous TXA, without maintenance, in DLS patients undergoing long-segment spinal fusion surgery. MATERIAL AND METHODS Patients who received TXA (TXA…

Successful Hemostasis With Platelet Transfusion and Tranexamic Acid, and Inhibitor Elimination With Cyclosporine, in Steroid-Resistant Acquired Coagulation Factor V Deficiency Caused by Antibiotic Readministration.

Acquired factor V deficiency (AFVD) is a rare coagulation abnormality associated with infectious diseases, antibiotics, surgery, autoimmune … readministration. The melena subsequently improved with platelet transfusion and administration of tranexamic acid, while prednisolone-resistant coagulation abnormalities improved with cyclosporine A (CsA) treatment. This case shows the importance of avoiding suspected drugs and the effectiveness of CsA as a second-line treatment of AFVD.

Mortality Benefit of Tranexamic Acid for Hemorrhage With Concurrent Traumatic Brain Injury: Outcomes From a Prospective Cohort Study in a High-Trauma, Prolonged Care Setting.

BACKGROUND: Traumatic brain injury (TBI) and hemorrhage are leading causes of trauma death and disability worldwide. The concurrence of hemorrhage and brain injury carries a two-fold increase in mortality and clinical management of patients with concurrent TBI and hemorrhage is challenging. Tranexamic acid (TXA) has been shown to reduce mortality from hemorrhage and TBI independently, however there is sparse evidence on the potential benefit of TXA in patients with both non-head hemorrhage and TBI. METHODS: We conducted a secondary database analysis of EpiC, a multicenter, prospective cohort …
