Hemodynamic problems are common in neonatal intensive care. They occur in the context of incomplete myocardial and vascular development and in cardiovascular responses to interventions which are, as a result, limited and often uncertain and unpredictable. In this review, I outline the hemodynamic features of 4 neonatal conditions which often require intervention: (1) persistent pulmonary hypertension of the newborn, (2) cardiogenic shock (most commonly in the context of hypoxic ischemic injury), (3) sepsis and (4) low blood pressure in the transitional period of the extremely preterm infant. I

INTRODUCTION: Advances in disease-modifying therapies have improved outcomes and survival in spinal muscular atrophy (SMA), resulting in a growing population of adolescents and young adults who must transition from pediatric to adult healthcare systems. Despite longstanding professional guidance supporting structured transition planning, limited data describe how these processes are implemented and experienced in SMA in the United States (U.S.). This study evaluated patient-reported experiences of transition planning and engagement with adult healthcare providers among young adults living with

BACKGROUND: Health care transitions in the last months of life can be burdensome and potentially of limited clinical benefit for patients with advanced cognitive and functional impairment.
METHODS: To examine health care transitions among Medicare decedents with advanced cognitive and functional impairment who were nursing home residents 120 days before death, we linked nationwide data from the Medicare Minimum Data Set and claims files from 2000 through 2007. We defined patterns of transition as burdensome if they occurred in the last 3 days of life, if there was a lack of continuity in nurs

INTRODUCTION: Tuberous sclerosis complex (TSC) is a multisystemic genetic disease with high clinical variability and age-related manifestations. These characteristics add to the complexity of transition to adulthood. This study aimed to explore the perception of medical follow-up and transition experience in a large group of patients with TSC who presented epilepsy in childhood.
METHOD: This multicenter French study included patients with TSC aged 18 years or older who developed epilepsy before the age of 16 years. A questionnaire specifically designed for the study explored patients' opinion

OBJECTIVES: To describe neonatologist continuity of care and estimate the association between these transitions and selected patient outcomes.
STUDY DESIGN: We linked Children's Hospitals Neonatal Database records with masked neonatologist daily schedules at 4 centers, which use 2- and 3-week and 1-month "on service" blocks to provide care. After describing the neonatologist transitions, we estimated associations between these transitions and selected short-term patient outcomes using multivariable Poisson, logistic, and linear regression analyses, independent of length of stay (LOS) and case

BACKGROUND: Today, most children with end-stage renal disease and after kidney transplantation reach adulthood, even with complex underlying diseases. As a result, nephrologists in internal medicine are increasingly encountering patients with clinical pictures that used to play only a minor role in the past.
RESEARCH QUESTION: The aim of this article is to provide an overview of the transition currently taking place in German nephrology and to offer perspectives for improving the process.
MATERIALS AND METHODS: Based on current literature and transition initiatives in German nephrology, the

Incarcerated populations in Canada face significant health and social challenges during transitions into and out of correctional facilities. These transitions around facilities pose disproportionate barriers to care for people living with HIV. Further research is crucial to comprehend these challenges and reimagine care concepts for people who experience structural marginalization. In this article, experiences of transitions into and out of Alberta correctional facilities for people living with HIV are explored using narrative inquiry. Conducted in a Western Canadian city from 2021 to 2022, th

The sixth update of the Canadian Stroke Best Practice Recommendations for Transitions and Community Participation following Stroke is a comprehensive set of evidence-based guidelines addressing issues faced by people following an acute stroke event. Establishing a coordinated and seamless system of care that supports progress achieved during the initial recovery stages throughout the transition to the community is more essential than ever as the medical complexity of people with stroke is also on the rise. All members of the health-care team engaged with people with stroke, their families, and

INTRODUCTION: In Australia, nearly 400,000 people live in residential aged care, of those, 40% experience a hospital admission annually. High-quality care transitions are crucial for ensuring safe and effective continuity of care. Ineffective information sharing increases risks of harm for already vulnerable consumers, contributing to adverse health outcomes, creating care delays and causing distress to individuals moving between care settings.
AIM: This study aimed to co-design, pilot and evaluate the implementation of a comprehensive discharge documentation and medication package aligned wi

BACKGROUND: Adolescents in foster care endure frequent disruptive transitions, often culminating in discharge to independent living rather than reunification or adoption. Former foster youth fare poorly once on their own, with high rates of homelessness and social disconnection. This study explored the use of the Bridges Transitions Framework near the end of placement to help youth cope with the transition to adulthood.
METHODS: In this exploratory study, the framework was integrated into a foster agency's programming; then, we assessed outcomes using administrative data and youth interviews.

BACKGROUND: Patients with Anorexia Nervosa (AN) often experience the transition between Child and Adolescent Mental Health Services (CAMHS) and Adult Mental Health Services (AMHS) as challenging. This period tends to have a negative influence on the continuity of care for the adolescents and represents a demanding and difficult period for the parents. To our knowledge, no previous study has explored the parents' experience with the transition from CAMHS to AMHS. Therefore, this qualitative study examines how parents experience the transition process from CAMHS to AMHS.

Patients transitioning from pediatric to adult HIV care have lower rates of retention in care and viral suppression. Health system interventions can improve outcomes in adolescents and young adults living with HIV (AYA-HIV). We designed and implemented an Adolescent Young Adult Healthcare Transition (AYAHCT) clinic in a U.S.-based healthcare system. We present a mixed-method (quantitative and qualitative) evaluation. The quantitative analysis included retrospective clinical outcomes between 2017-2023. Qualitative analysis included stakeholder interviews in pediatric, AYAHCT, and adult HIV clin

This article describes the application of transition theory to assist a family with an infant with congenital complex gastroschisis. The nursing period, from March 3, 2023 to May 9, 2023, encompassed care from hospitalization to discharge. The author employed transition theory as a guide and used physical assessments, observations, and interviews for data collection as well as behavioral processes records. The primary nursing problem was identified as "preparation for family operation process enhancement/child's congenital disease and complex care needs, and the family's response to the challe
