Urinary incontinence following total prostatectomy was evaluated in 10 patients by urodynamics and lateral urethrography. The pathological stage of the tumors in these patients was pT2 in 5 patients, pT3 in 3 patients and pT4 in 2 patients. Urinary incontinence was present in 8 patients, of whom 7 were stress incontinence and one was urge and stress incontinence. From June 1982 to November 1990, 37 patients, aged 13 to 19 years (35 males and 2 females), were treated in our division for neuropathic urinary incontinence with an artificial sphincter. There are many causes of urinary incontinence in the female, with stress incontinence reported as the most common. However, a high incidence of detrusor instability has been described in incontinent patients with Parkinson disease. To assess this further, urodynamic studies were performed on 17 female patients with Parkinson disease and complaints of urinary dysfunction. Results of the abdominal levator-muscle repair in 175 stress incontinent women are presented. Two years after the operation 83% were cured or improved regarding urinary incontinence. The urodynamic evaluation revealed a normalization of the maximum flow rate, residual urine and cystometry in nearly all cases. The abdominal levator-muscle repair carried a low morbidity, and is especially recommended in women with combined urinary and anal incontinence.

A pad-weighing test of urinary stress incontinence during a set program of physical exercises is presented. The test was begun at a fixed bladder volume (75% of maximum capacity) and was repeated once. The duration of the exercises was approx. 15 min. Leakage was greater in the second test, but statistical analysis showed highly significant intraindividual correlation between the two amounts. Degree of leakage was not correlated to subjective assessment of incontinence. The test can be done as part of a routine urodynamic investigation.

Free autogenous muscle transplantation has now been carried out in 13 patients with anal incontinence and 16 patients with urinary incontinence. In anal incontinence the results have been excellent; all patients were improved and 12 out of 13 reached an acceptable level of continence. Of 75 patients with complete urinary incontinence (25 female and 50 male patients) 25 were managed by the posterior bladder tube (Leadbetter) technique and 50 were managed by the anterior tube reconstruction of Tanagho.
