INTRODUCTION: Besides being a risk factor for urolithiasis, obesity is a challenge in the treatment of urolithiasis from the perspective of both the surgeon and the anesthetist. In this study, we tried to assess the feasibility and safety of supine percutaneous nephrolithotomy (PCNL) under regional anesthesia in obese patients with a body mass index (BMI) ≥30.
METHODS: This was a prospective observational study and included 51 obese patients (BMI ≥30 kg/m2) with renal stones planned for PCNL. All patients underwent supine PCNL under regional anesthesia with the standard technique. A decision …

CONTEXT: Urolithiasis and obesity are now public health problems with an increased incidence worldwide. Management of urolithiasis in patients with morbid obesity is usually associated with higher morbidity and mortality compared to non-obese patients. ESWL is a nonsurgical procedure for the treatment of lithiasis. In obese patients the ESWL efficacy is influenced by the skin-to-stone distance or poorer detection of the calculus because of the fatty tissue.
OBJECTIVE: To highlight the overweight or obese lithiasic patient profile that could be best treated by ESWL.
SUBJECTS AND METHODS: We e…

OBJECTIVE: To assess the impact of new technology on the management of pediatric urolithiasis by analyzing our local practice over the past 24 years, and determining the role of open surgery at the present time.
METHODS: We retrospectively reviewed the charts of 768 children (783 procedures) who underwent surgical treatment for urolithiasis between June 1987 and October 2010. Data were analyzed with respect to patient characteristics and changing patterns of treatment with time. We compared the type of procedures performed between four time periods: the first was before ESWL, the second was a…

OBJECTIVE: To review a multicentre experience of using a transportable lithotripter (STS-T, Medstone, Inc, Aliso Viejo, CA. USA) for treating patients with urolithiasis in all parts of the urinary tract.
PATIENTS AND METHODS: In all, 326 patients with a total of 370 stones were treated as outpatients with the STS-T lithotripter. All patients received a single shock wave lithotripsy treatment and were followed after 4-6 weeks in the outpatient clinic, the primary endpoint being to determine the efficacy (as defined by the stone-free rate). Secondary objectives included establishing a database …

To address whether a renal tubular dysfunction is encountered in a particular patient subgroup with urolithiasis, the following parameters of tubular function were measured in urine taken in the morning from 214 stone formers after fasting: pH, excretion of lysozyme and gamma-glutamyl transferase (gamma-GT); fractional excretion (FE) of glucose, insulin, Mg, K, and HCO3 after an alkali loading; and the renal threshold for phosphate (TmP/GFR). The following diagnoses were made in the patient group: primary hyperparathyroidism (N = 8), medullary sponge kidneys (N = 21), hyperuricemia (N = 10), c…

We present here two special cases of urolithiasis. The first one shows a giant bladder lithiasis resulting in severe renal insufficiency in a 63-year-old patient, who had previously had nicturia (2-3 times), occasional episodes of urinary frequency and burning micturition, in the absence of renal colic, hematuria or interrupted urination. The second case referes to an 85-year-old man suffering from prostatic enlargement and bladder stones, hospitalized to undergo intervention of trans-vesical prostatic adenomectomy, during which two star-shaped stones were found without obvious symptoms.

Extracorporeal shock wave lithotripsy (ESWL) is one of the most frequently applied procedures for the treatment of urolithiasis. ESWL breaks and splits stones by the means of repeated acoustic shock waves. Despite its non invasive nature, ESWL has been intuitively associated with potential complications, mostly related to residual stone fragments. While non stone-related complications are rare (< 1 %), awareness and identification of these complications could help clinicians to prevent and manage them safely and effectively. The current study reviews the pathophysiology, predicting factors and…

Statistical studies on 2,040 new outpatients, 575 inpatients and 570 operative procedures at our department in 1982 revealed the following. The most frequent diseases among the outpatients were urogenital infections followed by anomalies and tumors. Over half of the inpatients were pediatric patients and the major diseases among inpatients were hypospadias, vesicoureteral reflux, urolithiasis, congenital urethral stenosis and undescended testis. A total of 570 operations were performed on 531 patients, and the major five operations were hypospadias repair (79), optic internal urethrotomy (71),…

UNLABELLED: Urolithiasis, as defined by the presence of one or more stones in the urinary tract, is a common disease, affecting up to 20% of the global population. Extracorporeal shock wave lithotripsy (ESWL) is a minimal invasive intervention for the medical management of urolithiasis. Despite the short duration of this noninvasive procedure, patients may experience anxiety.Purpose of the study was to explore state and trait anxiety before and after ESWL as well as the associated factors.
METHODS AND MATERIAL: In the present cross-sectional study, 100 patients who underwent ESWL (60 men and …

Adenine phosphoribosyltransferase (APRT) deficiency is a rare autosomal recessive disorder of purine metabolism causing 2,8-dihydroxyadenine urinary tract stones and crystal nephropathy. Disease manifestations include acute kidney injury (AKI), progressive chronic kidney disease (CKD), and not infrequently, kidney failure. A significant proportion of affected individuals remain asymptomatic into adulthood. The diagnosis of APRT deficiency should be considered in all children presenting with renal colic, radiolucent urinary stones and/or AKI, as well as in infants with a history of reddish-brow

BACKGROUND The renal condition known as 2,8-dihydroxyadeninuria is an autosomal recessive disorder caused by adenine phosphoribosyl transferase (APRT) deficiency, which most commonly manifests as nephrolithiasis. APRT deficiency has also led to the development of chronic kidney disease and end-stage kidney disease in the absence of nephrolithiasis and has also been shown to recur following kidney transplantation. CASE REPORT This report aims to highlight the diagnostic and therapeutic challenges associated with APRT deficiency by presenting 2 patients with end-stage kidney disease who develope

OBJECTIVE: To compare the frequency of stone-related events among subgroups of high-risk patients with and without 24-hour urine testing before preventive pharmacological therapy (PPT) prescription. While recent studies show, on average, no benefit to a selective approach to PPT for urinary stone disease (USD), there could be heterogeneity in treatment effect across patient subgroups.
MATERIALS AND METHODS: Using medical claims data from working-age adults and their dependents with USD (2008-2019), we identified those with a prescription fill for a PPT agent (thiazide diuretic, alkali therapy

INTRODUCTION: Chronic heart failure (CHF) is associated with high morbidity and mortality and is diagnosed more and more frequently. Fifteen to 30% of patients with systolic CHF develop atrial fibrillation (AF).
AIM: To establish whether persistent AF was an independent predictor of mortality, and had a predictive value with respect to late clinical outcomes in patients with systolic CHF.
METHODS: Analysis comprised 120 men with systolic CHF. In 35 (58%) patients CHF was the result of ischaemic heart disease and in 25 (42%)--idiopathic dilated cardiomyopathy (DCM). Presence or absence of AF

BACKGROUND: Endourology is a widely used means by which to manage urolithiasis. Patient anatomy can oftentimes limit what can be accomplished with current technology.
CASE PRESENTATION: This is a case of a patient with renal and ureteral stones within a transplant kidney. Her anatomy would not allow for a standard retrograde ureteroscopic approach. We describe a method by which to overcome this difficult scenario by using a rigid cystoscope as a platform by which a ureteroscope was passed to allow for stone removal.
CONCLUSION: For this difficult case, we effectively used our instruments to …
