We report on a 33-year-old male Turkish patient with primarily nonsuspect sexual behavior who presented with panuveitis unresponsive to therapy. HIV infection and secondary syphilis was diagnosed. The uveitis was the only manifestation of syphilis. Because of isosporiasis, an HIV infection of CDC class IV C1 was diagnosed. This is the second published case of acquired syphilitic uveitis in a patient with HIV infection. The diagnosis was delayed by a prozone phenomenon. Risk factors for significant visual loss were investigated in 51 patients with iridocyclitis associated with juvenile rheumatoid arthritis (JRA). Average follow-up was 12.7 years. Of 89 eyes with uveitis, 22% had visual loss to 20/200 or worse, 46% had cataracts, 30% had band keratopathy, and 27% had glaucoma. Severity of visual loss and complications correlated with the degree of inflammation found on initial ocular examination. A possible involvement of superoxide in the pathogenesis of uveal inflammation in man and experimental animals was investigated. Superoxide … f SOD against tissue damage was also observed in a bovine serum albumin-induced passive Arthus type uveitis in rabbits. These results suggest that superoxide may play a role in causing tissue damage in animal models of ocular inflammation and possibly in Behcet disease.

Acute anterior uveitis (AAU) is more often seen in ankylosing spondylitis (AS) patients with severe disease than in HLA-B27 positive relatives with or without sacroiliitis. Possibly the prevalence of AAU is associated with the severity of AS.

We developed an experimental model of anterior uveitis by injecting ovalbumin or bovine serum albumin into the anterior chamber of sensitized rabbits. All the eyes showed leakage of serum proteins into the aqueous humor within 30 min. and developed uveitis within 24 hrs. which lasted at least a week. Combined lensectomy-vitrectomy was performed on 13 patients (15 eyes) with complicated cataracts from uveitis. Preoperative visual acuities were 20/200 or less for at least one year. The postoperative follow-up period averaged 10.5 months with visual acuities of 20/25 or better in eight eyes, 20/70 in two eyes, and 20/100 to 20/400 in five eyes. The primary cause of decreased vision postoperatively was cystoid macular edema with frequently associated optic disc edema.
