The results of treatment of 538 patients, operated on for perforative ulcer (PU), are studied up. Perforative aperture was sutured in 75 patients, in 153 the ulcer was excised, in 291 the excision was supplemented by vagotomy, resectional methods were applied in 19. In 94 (17.5%) of patients 114 complications have occurred after the operation. In recent years vagotomy has become more and more prefered in ulcer surgery. Based on the results of prospective trials, gastric resection should be abandoned as treatment of uncomplicated duodenal ulcer. Resections are still indicated for most ulcer complications, such as bleeding, penetration of chronic ulcer, and in recurrent ulcers after vagotomy. The same is true for gastric ulcer type I. Questionnaires indicate, however, that German surgeons still favor the two-thirds resection.

Following truncal vagotomy some patients develop dysphagia, which rarely can progress to complete obstruction. It has been postulated that haematoma formation in the tissues surrounding the lower oesophagus may lead to peri-oesophageal fibrosis and compression of the lumen of the oesophagus. This has been confirmed by open operation in only a few patients. A case is described in which oesophageal obstruction occurred 3 weeks after a vagotomy and pyloroplasty had been performed.

Experience with six morbidly obese patients with symptomatic reflux esophagitis associated with hiatal hernia, the diagnoses confirmed by upper GI x-ray contrast studies and endoscopy, has been reported. Truncal vagotomy with Heineke-Mikilicz pyloroplasty and gastric bypass with Roux-en-Y gastrojejunostomy has controlled all symptoms of esophagitis and produced effective weight loss. It is suggested that if the proximal gastric pouch is no larger than 25 ml there is no need to perform truncal vagotomy and pyloroplasty.

The experience of treatment of 303 patients with the operated stomach diseases (OSD) is summarized. It was established that to reduce the frequency of their occurrence it is necessary: to perform vagotomy especially selective proximal (SPV) thoroughly for complete acid producing zone denervation achievement; to examine gastric cardia and pyloroduodenal zone before and intraoperatively thoroughly to select an optimal method of its drainage; to conduct conservative therapy of ulcer disease after vagotomy conduction up to the ulcer defect healing according to endoscopic control data.

Clinical observations of 247 patients with ulcer disease of the stomach younger than 30 years of age were analyzed. Ulcers of the body and the proximal part of the stomach were diagnosed in 39 patients, associated gastroduodenal ulcers--in 37 patients, pyloric and propyloric ulcers in 171 patients. Conservative treatment was effective almost in 2/3 of the patients. Surgical treatment included resection of the stomach and vagotomy with draining operations. Vagotomy used in gastric ulcer can be considered valid in patients of young age.
