BACKGROUND: Contemporary management of ruptured vertebral artery dissecting aneurysms (VADA) has evolved beyond proximal parent artery occlusion (PPAO) to include endovascular trapping (ET) of the diseased segment and vessel preserving stent treatments. The aim of this retrospective cohort study was to assess the outcomes of patients with ruptured VADAs who underwent endovascular management with trapping of the diseased segment as the first-line treatment approach.
METHODS: We evaluated an institutional database of patients with ruptured VADAs who were treated at Auckland City Hospital from 1

Internal carotid artery (ICA) dissection often presents with headache and neck pain, along with symptoms related to the anterior circulation if stroke occurs. Less commonly, it may cause Horner's syndrome and involvement of the lower cranial nerves (IX, X, XI, and XII). However, it is rare for ICA dissection to present with pulsatile tinnitus and hearing loss, which is typically linked to vertebral artery dissection. We report a rare case of a man in his 50s who initially presented to the Otolaryngology Department with a brief episode of high-pitched pulsatile tinnitus and hearing loss, preced…

PURPOSE: Stent dislocation is a rarely encountered problem in interventional neuroradiology. This article describes the repositioning of a pipeline embolization device (PED) dislocated from the vertebral artery (VA) into the basilar artery (BA) using a stent-in-stent technique. Based on this case additional in vitro measurements were performed.
METHODS: In a patient, a larger PED (4.0 × 20 mm) was partially opened in a PED (3.0 × 20 mm) floating freely within the distal BA. The microcatheter with the partially opened stent was pulled back hereby pulling back the stent-in-stent construct into

We present a rare case of external carotid artery-vertebral artery anastomosis via the ascending pharyngeal artery, diagnosed upon cadaveric dissection. The ascending pharyngeal artery gave rise to a branch to the hypoglossal canal, which is a variation of a true persistent fetal hypoglossal artery. Knowledge of persistent carotid-vertebrobasilar anastomoses is important as these fetal vessels can contribute significantly to the posterior cerebral circulation. Only 10 cases of external carotid artery-vertebrobasilar artery anastomoses have been reported to our knowledge, and our case presents

PURPOSE: Cerebral arterial dissections are recognized as a common cause of stroke. However, few studies have reported on the distribution of cerebral arterial dissection and angiographic pattern related to the presenting clinical symptom pattern. We analyzed the distribution of cerebral artery dissection along with angiographic and clinical presenting a pattern as depicted on angiograms.
MATERIALS AND METHODS: From January 2000 to January 2007, 133 arterial dissection patients admitted to our institutes were retrospectively reviewed. The characteristic angiographic findings of all cerebral ar

Since the development of the technique of revascularizing the vertebral artery in its transversal and suboccipital portions, 22 revascularisation procedures of the distal vertebral artery (above C2) have been carried out in 21 patients. The earliest cases have been followed up for more than 4 years and the technique has proved to be reliable, both from the clinical and anatomical points of view. The indications were based on restoration of the vertebral axis compromised by major lesions (stenosis: 2 cases, occlusion: 14 cases) of both vertebral arteries (13 cases) or of the dominant vertebral

Children with brain infarction due to traumatic dissection of the carotid artery (14 cases) and vertebral artery (4 cases) were observed. In all the cases of brain infarction due to carotid dissection, history of trauma, immediate typical headache after the accident, development of infarction 24 hours after the accident and the presence of the Gorner's syndrome ipsilateral to infarction allowed definite clinical diagnosis. Clinical diagnosis of infarction due to vertebral artery dissection was more difficult and the diagnosis was established only after neuro visualization. In all the cases, ou…

OBJECTIVE: We describe our surgical technique of exposure and control of the second segment of the vertebral artery (VA V2 segment). Our basic principle is that working in the VA vicinity is more confident under visual control.
METHODS: The VA V2 segment extends classically in and between the transverse processes from C6 to C2. This segment can be exposed through an anterolateral approach, passing medially to the sternocleidomastoid muscle and laterally to the internal jugular vein. Except in case of anatomic variation, the VA V2 segment is protected by the transverse processes bone, even if

The most common manifestations of vertebral artery dissection in approximately 80% of patients are headaches or neck pain. We discuss a case of a 34-year-old patient who presented to the emergency department with altered mental status and nonspecific symptoms. A CT angiogram with intravenous contrast revealed a dissection of the left vertebral artery, and the patient was found to have thromboembolism in the right occipital lobe with ischemia on MRI. This case demonstrates the importance of maintaining a broad differential diagnosis for patients who present with altered mental status and nonspe…

Though the variations of vertebral artery are clinically asymptomatic yet abnormalities are of diagnostic importance either prior to vascular surgery in the neck region or in patients of intravascular diseases such as arteriovenous malformations or cerebral aneurysms. Therefore, the aim of the study is to bring out 2 variations in the configuration of vertebral artery and their clinical implication. During dissection of thorax of 2 female cadavers, 2 different variants of configurations of left vertebral arteries were observed. In 1 patient, the left vertebral artery arose aberrantly from arch

Five cases of giant, fusiform, and dissecting aneurysms of the vertebro-basilar junction, in which direct surgical treatment was not feasible, are reported. Their initial symptoms were as follows: 3 subarachnoid hemorrhages (2 fusiform aneurysms, 1 giant aneurysm), 1 mass sign (giant aneurysm), and 1 ischemic sign (dissecting aneurysm). In two patients, one with a giant and one with a dissecting aneurysm, preoperative proximal vertebral occlusion was carried out by inflated balloon for 30 to 100 minutes, under observation of clinical signs and measurement of distal arterial pressure. This cath

A dissecting aneurysm of the vertebral artery occurred in a 38-year-old male presenting with ischemic attacks. Vertebral angiography revealed irregular narrowing and dilatation of the right vertebral artery, and retention of the contrast medium. Routine T1-weighted magnetic resonance (MR) imaging demonstrated signal voids compatible with a patent but narrowed vertebral lumen surrounded by three intensity components at the abnormality location identified on the vertebral angiograms. Dynamic MR imaging demonstrated strong enhancement of these components on the early images. These findings were c

Headache is a common symptom of cervical artery dissections (CAD). Sometimes, it can be the only symptom and mimic migraine. We aimed to investigate headache characteristics and previous history of migraine in CAD patients, and to compare headache features between patients with internal carotid artery dissection (ICAD) and vertebral artery dissection (VAD). We investigated 53 CAD patients (n = 28 with ICAD, n = 25 with VAD). Demographic and clinical data were evaluated retrospectively. Headache information was obtained from the patient interviews with a structured questionnaire (based on the I…

The authors report the pathological and clinical findings of a dissecting aneursym of the basilar artery in a 32 year old man up till then in good health, who died two months later. The pathological study showed bilateral lesions of osteo-malacia in the neighborhood of the pons due to hemo-dissection affecting the right vertebral artery just before it enters the dura-mater, then its intra-cranial portion, the basilar trunk and the first part of the right posterior cerebral artery. The originality of this case resides in the finding of segmental stenosing panarteritis of the same right vertebra

BACKGROUND: Cervical artery dissection is one of the main causes of stroke in young adults, yet the characteristics of patients have been mainly described in Caucasian populations. We aimed to identify patients' characteristics associated with stroke among cervical artery dissection patients in Chile.
METHODS: We conducted a retrospective analysis of a national hospital-based database in Chile between 2019 and 2021. Patients with cervical artery dissection or aneurysm (CeDA) were identified using ICD-10 codes I72.0 (carotid artery) and I72.6 (vertebral artery); stroke events were classified u

A 61-year-old man was referred to our hospital for Stanford type A acute aortic dissection and an emergency operation was planned. In this case, the aberrant left vertebral artery originated directly from the aortic arch. Total arch replacement at the level between the left carotid artery and left aberrant vertebral artery was performed and an open stent graft was deployed in the true lumen of the descending aorta to obtain better distal aortic remodeling. The proximal edge of the nitinol skeleton of the open stent graft was placed distal to the orifice of the left subclavian artery. The left

A 54-year-old woman presented a subarachnoid hemorrhage from a ruptured dissecting aneurysm on the right vertebral artery. A right vertebral angiogram revealed a fusiform dilatation distal to the right posterior inferior cerebellar artery and "pseudo lumen" was observed in a delayed arterial phase. Conservative treatment was carried out, and the patient condition gradually improved. An angiographical examination for planned endovascular treatment 46 days after the onset, revealed the normalization of the formerly dilated caliber of the right vertebral artery with only a minor luminal irregular

Ischemic stroke is rare in children, most of which occur in the supratentorial brain, and infratentorial infarcts are very rare. Some clinical manifestations may be similar but others such as ataxia and cranial nerve palsy are more specific. Vertebral artery dissection is the most frequent cause of stroke in the vertebrobasilar territory, but the cause most often remains unknown in children. We report three cases of infratentorial stroke in children. The first observation concerns a 4-year-old boy brought to medical attention because left hemicorporal motor deficit associated with ataxia follo…

A 60-year-old man was admitted to our hospital because of abdominal pain and disturbed consciousness. Head magnetic resonance imaging showed right vertebral artery dissection and abdominal enhanced computed tomography showed dissection of the superior mesenteric artery. The patient was diagnosed as having fibromuscular dysplasia (FMD) based on conventional angiography. Although multiple vascular bed involvement is observed in approximately 40% of FMD patients, reports of sequential symptomatic dissections in various vascular beds are rare. Patients with FMD and dissection require close observa…

Described herein are findings in a 58-year-old man in whom necropsy disclosed origin of the left vertebral artery (or the arteria thryoidea ima) directly from the aortic arch. No functional consequences resulted. Study of previous publications disclosed the frequency of this anomaly in adults to be approximately 3.5%. Dissection has been reported to be more frequent in the left vertebral artery when it arises directly from the aorta than when it arises from the left subclavian artery.
