Clinical significance of vibration-induced nystagmus and head-shaking nystagmus through follow-up examinations in patients with vestibular neuritis.

OBJECTIVES: To verify if vibration-induced nystagmus (VIN) in patients with vestibular neuritis changes over time and to compare the results of VIN test to those of caloric test and head-shaking nystagmus (HSN) test. STUDY DESIGN: Retrospective study of a series of cases. METHODS: We compared VIN and HSN tests using caloric test results in 22 patients (male-to-female ratio, 11:11; age range, 15-67 yr) with acute vestibular neuritis seen at onset and in follow-up after 2 months. The eye movement recordings were made, and the maximum slow-phase eye velocities (SPVs) were calculated during vibr

Neurootologic findings in patients with so called Menière-like disease.

A group of 150 patients was clinically observed using a complete battery of neurootological methods. The patients were believed to suffer from Menière's disease (MD). After taking anamnestic data [NODEC III] and performing calorimetry, cranio-corpo-graphy and audiology tests we could confirm a former established diagnosis of MD in a few cases only.

Three-dimensional analysis of eye movements during off vertical axis rotation in patients with unilateral labyrinthine loss.

3D analysis of eye movements during off vertical axis rotation (OVAR) was carried out in seven subjects with unilateral labyrinthine loss (ULL). The modulation component (MOC) of these patients was not different from that of normal subjects. However, the horizontal MOC was significantly smaller when the rotation was directed towards the diseased side as opposed to the healthy side. With rotation to the diseased side, most subjects exhibited a horizontal bias component (BIC) to the opposite side as compared with the diseased side in normal subjects. The vertical and torsional BICs were also inf

Vestibular compensation in a patient with a cerebellar infarction.

We report the case of a 67-year-old man who suffered a traumatic unilateral peripheral vestibular injury subsequent to an unrelated prior cerebellar infarction that occurred at least 2 years earlier. The patient's clinical course was marked by poor compensation for his peripheral vestibular loss. Four months after his vestibular injury, his symptoms of dizziness had not resolved. He had a spontaneous vestibular nystagmus, and laboratory testing indicated an asymmetric semicircular canal-ocular reflex. The otolith-ocular reflex, tested using off-vertical axis rotation, also was asymmetric but h

Changes in clinical and instrumental vestibular parameters following acute exposition to auditory stress.

Besides Tullio's phenomenon, resulting from anatomic changes in the labyrinth, a hypersensitivity to acoustic stimuli of the saccular structures appears to be the underlying cause of the vestibular responses detected in some patients. In order to evaluate the incidence of vestibular symptoms triggered by acute exposure to auditory stress (disco music), 40 subjects aged between 18 and 26 years, with no audiological and vestibular disorders, were submitted to otoneurologic tests. Subjects were exposed to disco music [intensity 128 dB (C)], for 3 hours. Tests have been carried out before and imme

Electrical vestibular stimulation after vestibular deafferentation and in vestibular schwannoma.

BACKGROUND: Vestibular reflexes, evoked by human electrical (galvanic) vestibular stimulation (EVS), are utilized to assess vestibular function and investigate its pathways. Our study aimed to investigate the electrically-evoked vestibulo-ocular reflex (eVOR) output after bilateral and unilateral vestibular deafferentations to determine the characteristics for interpreting unilateral lesions such as vestibular schwannomas. METHODS: EVOR was recorded with dual-search coils as binocular three-dimensional eye movements evoked by bipolar 100 ms-step at EVS intensities of [0.9, 2.5, 5.0, 7.5, 10.0

[Characteristics of the subtype of vestibular migraine with ear fullness: association with early onset age and low-frequency hearing loss].

Objective:The clinical significance of aural fullness in patients with vestibular migraine(VM) remains unclear, and it is yet to be determined whether VM with aural fullness represents a distinct subtype of VM; this study aimed to compare differences in demographic characteristics, clinical manifestations, audiological findings, and vestibular function tests between VM patients with and without aural fullness, and explore whether the former is a subtype of VM and whether it requires differentiated treatment. Methods:A total of 174 VM patients were enrolled, including 75 with aural fullness(aur…

An exploration of vestibular function pre and post unilateral cochlear implantation.

Objectives: To examine the effects of unilateral cochlear implantation on the balance system for adult patients with bilateral severe to profound sensory neural hearing loss. Methods: 7 CI candidates. The function of the sacculus, utricle, and three semi-circular canals (SCCs) was assessed separately using air conduction cervical vestibular evoked myogenic potentials (VEMP), vibration ocular VEMP and the video head impulse test (vHIT) respectively on each side, pre- and post-operatively. Results: The Otoliths appear more affected by implantation than SCCs. In 3 of 7 cases VEMP was affected by

The "broken escalator" phenomenon: Vestibular dizziness interferes with locomotor adaptation.

BACKGROUND: Although vestibular lesions degrade postural control we do not know the relative contributions of the magnitude of the vestibular loss and subjective vestibular symptoms to locomotor adaptation. OBJECTIVE: To study how dizzy symptoms interfere with adaptive locomotor learning. METHODS: We examined patients with contrasting peripheral vestibular deficits, vestibular neuritis in the chronic stable phase (n = 20) and strongly symptomatic unilateral Meniere's disease (n = 15), compared to age-matched healthy controls (n = 15). We measured locomotor adaptive learning using the "broken

[The visual fixation test in the study of visual-vestibular interactions. General aspects and results in normal subjects and patients].

Whenever we need to study visual-vestibular interactions, our method of choice among the ones based on thermal stimulation is the visual suppression test. In the vestibular laboratory of the II E.N.T. Clinic of the University of Parma the V.S.T. has been employed since 6 years systematically in all patients undergoing Electronystagmography. Our study was based on 10 normal patients and on 30 patients suffering from different pathologies. These patients were divided into groups according to the site of the lesion. The results we obtained are interesting: this test gives us important clinic data

[Frequent errors in neuro-otologic diagnosis].

A number of characteristic sources of error during the testing of vestibular function, originating from anamnesis, equipment and examination, are described. It is stressed that false-positive findings are often due to the influence of alcohol and that false-negative findings may be the result of tiredness or sedative drugs. With regard to the caloric test, it is recommended that water provides a better stimulus than air, and that the warm caloric stimulus is more valid than the cold caloric stimulus. Attention should be paid to the ENG-recorders, particularly to an adequate writing breadth, an

The effects of vestibular dysfunction on balance and self-concept in children with cochlear implants.

OBJECTIVES: Children with cochlear implants (CCI) have an increased rate of vestibular dysfunction. Vestibular dysfunction is associated with decreased balance and dynamic visual acuity ability. Hearing loss alone is associated with reduced speech perception and vocabulary in children. In adults, vestibular dysfunction is associated with reduced quality of life; however, similar relationships have not been studied in children with vestibular dysfunction. Therefore, the objective of the present study was to evaluate the effect of hearing loss and vestibular dysfunction on self-concept in CCI (n
