Influence of anatomy and head position on intranasal drug deposition.

The objective of this study was to determine the influence of individual anatomical differences on intranasal drug deposition. The data of a comparison of seven different administration techniques in ten healthy volunteers was used in this single-blind crossover pilot study. After intranasal administration of a dyed test formulation, endoscopic video imaging was done on seven non-sequential days. The deposition pattern per individual around the head of the middle turbinate was analyzed for each technique and correlated with the individual anatomy. Decreased deposition of dyed test formulation

Temporal lobe epilepsy in children: electroclinical study of 77 cases.

PURPOSE: Temporal lobe epilepsy (TLE) is probably more difficult to recognize in children than in adults. In fact, ictal symptoms in children are less stereotyped and less obvious, and the neuropathological substrate is more heterogeneous than in adults. The aim of this study is to examine the relationships between etiology, age at onset and electroclinical findings in 77 children with TLE, 32 of whom were surgically treated. METHODS: Electroclinical study including video-EEG recording of seizures in 77 children with TLE. The investigation focused on the first five initial ictal symptoms. RE

Exercise-induced dystonia as a preceding symptom of familial Parkinson's disease.

Paroxysmal exercise-induced dystonia can occur with Parkinson's disease (PD), and in rare cases, this can also be the presenting symptom. We report on 2 second cousins (no known consanguinity) who presented with paroxysmal exercise-induced dystonia who later developed clinical features of PD. Although autosomal recessive inheritance was suggested, and the dystonic features further suggest parkin as a possible cause, sequencing for parkin mutations was negative and this family may represent a genetic variant of PD. Further genotype-phenotype studies in this and similar families may give clues t

Split-screen synchronized display. A useful video-EEG technique for studying paroxysmal phenomena.

Video-EEG is an indispensable and widely used technique for studying paroxysmal phenomena. We present a method of processing video recordings to compare the sequence of ictal manifestations of different episodes. Five presentations of two seizures recorded in each of five patients are shown, in which apparently different ictal features are strongly superimposed and stereotyped. Split-screen synchronized display is a simple and valid technique for studying and presenting particular semeiological aspects of epileptic seizures. (Published with videosequences).

Videonasopharyngoscopy in patients with 22q11.2 deletion syndrome (Shprintzen syndrome).

INTRODUCTION: Velo-cardio-facial syndrome (VCFS) (also known as DiGeorge sequence, conotruncal anomaly face syndrome, 22q11.2 deletion syndrome among other labels) is now recognized as the most common syndrome associated with cleft palate and velopharyngeal insufficiency. VCFS has been associated with medially positioned internal carotid arteries. This anomaly has been associated with obvious posterior pharyngeal pulsations seen on videonasopharyngoscopy. The purpose of this paper is to study the role of videonasopharyngoscopy for the evaluation of patients with VCFS and submucous cleft palate

The spectrum of nonepileptic events in children.

Nonepileptic events (NEE) are common in children, and can be difficult to distinguish from epileptic events. Several strategies can assist in differentiation. The first is an age-based approach to the differential of commonly presenting EEs in neonates, infants, and adolescents. The next strategy is to identify key elements of the patient's history to narrow the possibilities, and third is a rational approach to ancillary testing. There are additional challenges to the diagnosis and evaluation of NEEs in patients with cognitive impairments or mental retardation (MR). Twenty to 25% of neurologi

Video endoscopic surgery in the community hospital.

After proper training and observation, I started performing laparoscopic cholecystectomy at the community hospital in August 1990. Since then, and after further advanced training, I have performed 172 procedures, including appendectomy, hernia repair, lysis of adhesions, retroperitoneal biopsy, paraesophageal hernia repair, exploration of the common bile duct, repositioning of CAPD catheters, and colon resection with excellent results. This experience confirms that video endoscopic surgery can become the approach of choice in the community for many of what today are still considered open proce

Nurse-patient interaction after training in integrity promoting care at a long-term ward: analysis of video-recorded morning care sessions.

Morning care is a complex and problematic task for the demented patient, who has problems in understanding and coping with the situation. A training programme of integrity promoting care was given to the staff of a long-term ward and improvements of care were implemented during a 3 month intervention period. To evaluate possible effects, different parameters were used and compared with the data of a control ward. This report describes how caregivers behaved towards demented patients during morning care, before and after the training, and how patients' behaviour changed. The results are based o

Visual Preference for Biological Motion in Children and Adults with Autism Spectrum Disorder: An Eye-Tracking Study.

Participants with autism spectrum disorder (ASD) (n = 121, mean [SD] age: 14.6 [8.0] years) and typically developing (TD) controls (n = 40, 16.4 [13.3] years) were presented with a series of videos representing biological motion on one side of a computer monitor screen and non-biological motion on the other, while their eye movements were recorded. As predicted, participants with ASD spent less overall time looking at presented stimuli than TD participants (P < 10-3) and showed less preference for biological motion (P < 10-5). Participants with ASD also had greater average latencies than TD pa

Quantitative analysis of cell migration using optical flow.

Neural crest cells exhibit dramatic migration behaviors as they populate their distant targets. Using a line of zebrafish expressing green fluorescent protein (sox10:EGFP) in neural crest cells we developed an assay to analyze and quantify cell migration as a population, and use it here to characterize in detail the subtle defects in cell migration caused by ethanol exposure during early development. The challenge was to quantify changes in the in vivo migration of all Sox10:EGFP expressing cells in the visual field of time-lapse movies. To perform this analysis we used an Optical Flow algorit

Mouth opening during sleep may be a critical predictor of surgical outcome after uvulopalatopharyngoplasty for obstructive sleep apnea.

STUDY OBJECTIVES: To evaluate the value of mouth opening during sleep for predicting surgical outcomes after uvulopalatopharyngoplasty (UPPP). METHODS: Retrospectively, 69 out of 120 consecutive patients with obstructive sleep apnea who underwent uvulopalatopharyngoplasty at an academic tertiary referral center were included in this study. Sixty-nine subjects underwent cephalometry, nocturnal polysomnography and sleep videofluoroscopy before and after UPPP. Multiple parameters from the above studies were evaluated as potential predictors of UPPP outcomes. RESULTS: Multivariate analysis showe

Iron deficiency anemia in infancy and reach and grasp development.

This study assessed 9 kinematic characteristics of infants' reach and grasp to test the hypothesis that iron deficiency anemia (IDA) delays upper extremity motor development. Reach and grasp movements, recorded with a 3D-motion capture system, were compared in 9- to 10-month-old infants (4 IDA vs. 5 iron-sufficient [IS]). Based on normative motor development data available for 6 characteristics, the results indicated poorer upper extremity control in IDA infants: 2 characteristics showed statistically significant group differences despite small n, and the other 4 had strong indications for suc

Resistance to phenobarbital extends to phenytoin in a rat model of temporal lobe epilepsy.

PURPOSE: Most patients who are resistant to the first antiepileptic drug (AED) treatment are also resistant to a treatment with a second or third AED, indicating that patients who have an inadequate response to initial treatment with AEDs are likely to have refractory epilepsy. Animal models of refractory epilepsy are important tools to study mechanisms of AED resistance and develop new treatment strategies for counteracting resistance. We have recently described a rat model of temporal lobe epilepsy (TLE), in which spontaneous recurrent seizures (SRS) develop after a status epilepticus induce

Predictors of laryngeal complications in patients implanted with the Cyberonics vagal nerve stimulator.

OBJECTIVES: Since its approval by the US Food and Drug Administration in 1997 for management of medically refractory seizures, more than 35,000 patients have been implanted with the Cyberonics vagal nerve stimulator. Preliminary reports described transient vocal changes in the majority of subjects, which were thought to be short-term. However, these reports were for the most part based upon perceptual evaluations by the subjects themselves. Later reports described possibly more permanent recurrent laryngeal nerve injury and recommended measuring the nerve diameter to use the safest spiral cuff

Strategies to prevent HIV transmission among heterosexual African-American men.

BACKGROUND: As part of qualitative research for developing a culturally sensitive and developmentally appropriate videotape-based HIV prevention intervention for heterosexual African- American men, six focus groups were conducted with thirty African-American men to determine their perceptions of AIDS as a threat to the African-American community, characteristics of past situations that have placed African Americans at risk for HIV infection, their personal high risk behaviors, and suggestions on how HIV intervention videotapes could be produced to achieve maximum levels of interest among Afric

Family functioning and children's adjustment: associations among parents' depressed mood, marital hostility, parent-child hostility, and children's adjustment.

Relations between parents' depressed mood, marital conflict, parent-child hostility, and children's adjustment were examined in a community sample of 136 ten-year-olds and their parents. Videotaped observational and self-report data were used to examine these relations in path analyses. A proposed model was tested in which mothers' and fathers' depressed mood and marital hostility were associated with children's adjustment problems through disruptions in parent-child relationships. Results showed that both mothers' and fathers' marital hostility were linked to parent-child hostility, which in

CSF B-cell expansion in opsoclonus-myoclonus syndrome: a biomarker of disease activity.

Lack of a biomarker of disease activity has hindered the therapy of childhood opsoclonus-myoclonus syndrome (OMS), which is purported to be mediated humorally. To determine if the cerebrospinal fluid (CSF) B lymphocyte, which may traffic into the central nervous system (CNS) to produce antibody locally, is one such biomarker, B lymphocytes were immunophenotyped in the CSF and blood of 56 children with OMS and 26 pediatric controls by dual-laser flow cytometry. Neurological severity was rated blindly from videotapes using a validated 12-item motor evaluation scale. Children with OMS manifested
