The epidemiology of vertigo, dizziness, and unsteadiness and its links to co-morbidities.

Vertigo, dizziness, and unsteadiness (VDU) are common symptoms traditionally considered to result from different kinds of vestibular and non-vestibular dysfunctions. The epidemiology of each symptom and how they relate to each other and to migraine, agoraphobia, motion sickness susceptibility (MSS), vaso-vagal episodes (VVE), and anxiety-depression was the object of this population-based study in north-eastern France. A self-administered questionnaire was returned by 2987 adults (age span 18-86 years, 1471 women). The 1-year prevalence for vertigo was 48.3%, for unsteadiness 39.1%, and for diz…

Evidence-based guidelines for interpretation of the Panic Disorder Severity Scale.

BACKGROUND: The Panic Disorder Severity Scale (PDSS) is promising to be a standard global rating scale for panic disorder. In order for a cl … ESULTS: The interpretation of the PDSS total score differed according to the presence or absence of agoraphobia. When the patients were not agoraphobic, score ranges 0-1 corresponded with "Normal," 2-5 with "Borderline," 6-9 with "Slightly ill," 10-13 with "Moderately ill," and 14 and above with "Markedly ill." When the patients were agoraphobic, score ranges …

Cigarette smoking and panic: a critical review of the literature.

OBJECTIVE: Cigarette smoking increases the risk of panic disorder with or without agoraphobia's emerging. Although the cause of this comorbidity remains controversial, the main explanations are that (1) cigarette smoking promotes panic by inducing respiratory abnormalities/lung disease or by increasing potentially fear-producing bodily sensations, (2) nicotine produces physiologic effects characteristic of panic by releasing norepinephrine, (3) panic disorder promotes cigarette smoking as self-medication, and (4) a shared vulnerability promotes both conditions. The aim of this review was to su…

Impact of comorbid psychiatric disorders on the outcome of substance abusers: a six year prospective follow-up in two Norwegian counties.

BACKGROUND: Most help-seeking substance abusers have comorbid psychiatric disorders. The importance of such disorders for the long-term cour … elapsers had an earlier onset of a substance use disorder, and more frequently major depression and agoraphobia. Multivariate analysis indicated that both psychiatric disorders (major depression) and substance use factors (early onset of a substance use disorder) were independent predictors of relapse. CONCLUSION: For reducing the risk of long-term relapse, …

A general anxiety-prone cognitive style in anxiety disorders.

OBJECTIVE: This study compared scores on the Anxious Thoughts & Tendencies (AT&T) questionnaire, a putative measure of a general anxiety-prone cognitive style, among patients with panic disorder without agoraphobia (PD, n=62), panic disorder with agoraphobia (PDA, n=29), generalized anxiety disorder (GAD, n=43), limited social phobia (LSP, n=34), generalized social phobia (GSP, n=33), and community residents (n=319). METHOD: Candidates for treatment studies completed a diagnostic interview and the AT&T. AT&T scores were compared among anxious groups using analysis of variance. Then depressed …

[Social phobia with somatic symptoms including nausea and urgency of micturition].

The common symptoms of the social anxiety response include blushing, trembling, feelings of muscular tension of the face, and fear of eye contact. However, the ICD-10 mentions other less familiar symptoms such as nausea, urgency of micturition or defecation, gastrointestinal discomfort, and diarrhea as symptoms of social phobia. Since some of these somatic symptoms are classified as panic-like symptoms in the DSM-IV, it is sometimes difficult to distinguish between social phobia and agoraphobia when these somatic symptoms appear in situations usually associated with agoraphobia. We investigate…

Fluoxetine in panic disorder.

Twenty-five patients with a primary DSM-III-R diagnosis of panic disorder with or without agoraphobia were treated openly with the serotonin uptake inhibitor fluoxetine for up to 12 months. For most patients, treatment was initiated at 5 mg/day to minimize adverse effects previously reported with initiation at higher doses. Nineteen (76%) experienced moderate to marked improvement in panic attacks. Four (16%) were unable to tolerate fluoxetine due to adverse effects. Initiating treatment of panic disorder with low doses of fluoxetine may increase its acceptability and permit more patients to b…

Benzodiazepines and anxiety sensitivity in panic disorder.

1. Benzodiazepines were discontinued in 16 patients who had recovered from panic disorder with agoraphobia after exposure treatment. 2. Drug discontinuation yielded a significant decrease in anxiety sensitivity and state anxiety in these long-term users. 3. Several likely explanations for the findings are discussed. 4. In the short term, treatment of panic disorder with benzodiazepines may lower anxiety symptoms. However, in the long run, it may decrease the individual tolerance to anxiety and discomfort.

Phobias in India and the United Kingdom. A trans-cultural study.

Thirty-nine phobic patients from India were compared with 39 similarly diagnosed age and sex matched patients from the United Kingdom. The British sample contained significantly more patients with agoraphobia and social phobia compared to the Indian group. Sudden death and illness phobias were more frequent in the Indian group. The Indian phobic patients also had more somatic and depressive symptoms. The influence of culture on the presentation of different phobias is discussed.

[Analysis of possibility of genotypic correlation between fear and anxiety].

Special features of anxious behavior in the elevated plus maze test and acoustic startle response were analyzed in 11 inbred mouse strains. A significant influence of the genotype both on the startle amplitude and behavior in the elevated plus maze was found. However, analysis of covariance did not reveal a genotype-related association between anxiety and startle amplitude. The data indicates that the fear-induced acoustic startle response and anxious behavior in the elevated plus maze (agoraphobia) are not genetically related.

["Zurich Vertigo Meeting"--phobic postural vertigo].

Phobic postural vertigo has been described as a syndrome that is distinguishable from agoraphobia, acrophobia, and "space phobia". Closely related to locomotion, it is characterized by a combination of nonrotational vertigo with subjective postural and gait instability mainly in patients with an obsessive-compulsive personality. The monosymptomatic disturbance of balance manifests with superimposed attacks that occur with and without recognizable provoking factors in the same patient and are experienced with and without accompanying excess anxiety, misleading both patient and physician to a fa…

The clinical spectrum of panic attacks.

Current psychiatric nosologies associate recurrent panic attacks mainly with panic disorder, and agoraphobia with panic attacks, circumscribing their clinical relevance to the anxiety group of diagnosis. Through selected clinical presentations and a literature review the authors propose the existence of a cross-syndromal panic-anxiety represented by the recurrent panic attack that crosses the borders of any single group of diagnosis. This concept is of relevance for clinical psychiatry and may also constitute an important source of variance in psychiatric research.

[Evidence to support psychodynamic psychotherapy].

In general, psychodynamic psychotherapy is not considered evidence-based treatment. This review includes recent meta-analyses and review papers. We conclude that evidence in favour of psychodynamic psychotherapy exists for unipolar depression, panic anxiety with and without agoraphobia, social phobia, generalised anxiety and post-traumatic stress disorder. For complex mental conditions, long-term psychodynamic psychotherapy shows greater effect than no treatment, standard treatment and short-term psychotherapy. Psychodynamic psychotherapy can be recommended for treatment of specific psychiatri…

Social phobias: behavioural and pharmacotherapy.

Five to 10% of the general population suffers from symptoms of an anxiety disorder. Social phobias, while less common than panic disorder, agoraphobia or simple phobias, are just as debilitating. Patients present with somatic, behavioural, mood and cognitive disturbances, of which unrecognized social isolation, depression, loss of employment, and drug and alcohol abuse can be the result. A symptomatic approach can be implemented through the use of education, insight, support, behaviour therapy, cognitive therapy, and pharmacological intervention.

Cognitive-behavior therapy for discontinuation of SSRI treatment of panic disorder: a case series.

In this report we describe the outcome of eight outpatients with panic disorder and agoraphobia who discontinued their treatment with a selective serotonin reuptake inhibitor (SSRI) in the context of a structured, group program of cognitive-behavior therapy. All patients successfully discontinued their SSRI medication while demonstrating clinical improvement. These results were maintained at 3-month follow-up. This case series suggests that manualized CBT for discontinuation of benzodiazepine treatment for panic disorder may be successfully applied to SSRI discontinuation as well.

Issues in the assessment of treatment response in panic disorder with special reference to fluvoxamine.

Assessment of treatment response in panic disorder is complicated by the multidimensional aspects of panic disorder and agoraphobia, the short-term benefits from nonspecific aspects of treatment, and placebo response. Response to treatment with psychological and pharmacologic treatments of panic disorder is reviewed in this context. The experience of several Phase III studies of fluvoxamine in the treatment of panic disorder is examined as an illustrative example. When the response to placebo or comparison treatment in a study is high, no conclusion can be drawn about the true efficacy of the …

Hormonal response during a fenfluramine-associated panic attack.

Secretion curves for prolactin, cortisol, TSH, and GH from a 37-year old woman with dysthymia and panic disorder with agoraphobia were determined one day prior to (day I), and during a panic attack (day II) associated with an oral dose of 60 mg dl-fenfluramine, a drug known to increase anticipatory anxiety. The increased cortisol secretion observed is discussed in relation to the hormonal correlates of anxiety and the possible role of depression, dl-fenfluramine, and serotonergic receptor sensitivity.
