Genetic response to an environmental pathogenic agent: HLA-DQ and onchocerciasis in northwestern Ecuador.

The aim of this study is to explore human leukocyte antigen (HLA)-DQ variability in two populations (Cayapas Amerindians and Afro-Ecuadorians) who live near one another along the Cayapa River and who are exposed to the same environmental stresses, such as infection by Onchocerca volvulus. HLA-DQA1 and HLA-DQB1 of 149 unrelated individuals (74 Cayapas and 75 Afro-Ecuadorians) have been analyzed. HLA high-resolution molecular typing was performed by sequence-based typing, sequence-specific oligonucleotides hybridization and sequence-specific primer (SSP) amplification. The comparison between aff

Veterans Hospitals vs Non-Veterans Centers in the Prevention of Limb Loss.

IMPORTANCE: US veterans remain an at-risk population for limb loss, given the prevalent rates of coexisting diabetes and peripheral arterial disease (PAD). Moreover, despite the perception that a healed minor amputation might obviate the need for a future major amputation, previous work documented increased longitudinal rates of limb loss among non-veteran Medicare beneficiaries. OBJECTIVE: To document the corresponding incidence of major amputation among veterans after a prior minor amputation and accordingly benchmark Veterans Affairs (VA) health care delivery compared to non-VA civilian ho

Native Americans have comparable transcatheter aortic valve replacement outcomes but higher stroke and venous thromboembolism after surgical aortic valve replacement.

BACKGROUND: Racial disparities in aortic valve replacement outcomes have been established. However, the current literature lacks comprehensive studies that examine the outcomes for Native Americans, probably due to their limited population size. This study aimed to investigate whether disparities in transcatheter aortic valve replacement (TAVR) and surgical aortic valve replacement (SAVR) also exist for outcomes among Native Americans. METHODS: Patients who underwent SAVR and TAVR were identified in National Inpatient Sample from the last quarter of 2015 to 2020. A 1:5 propensity score matchi

Association of diabetes with prostate cancer risk in the multiethnic cohort.

Among men of European ancestry, diabetics have a lower risk of prostate cancer than do nondiabetics. The biologic basis of this association is unknown. The authors have examined whether the association is robust across populations in a population-based prospective study. The analysis included 5,941 prostate cancer cases identified over a 12-year period (1993-2005) among 86,303 European-American, African-American, Latino, Japanese-American, and Native Hawaiian men from the Multiethnic Cohort. The association between diabetes and prostate-specific antigen (PSA) levels (n = 2,874) and PSA screeni

Higher prevalence of Concomitant Hyperparathyroidism in African American Patients Undergoing Thyroid Surgery.

BACKGROUND: Hyperparathyroidism (HPT) occurs in about 1% of the general population. Previous studies have suggested that the incidence is higher in those patients with thyroid disease who are undergoing thyroid surgery. The study purpose was to examine the incidence of concomitant HPT in patients already undergoing a thyroid procedure and to identify risk factors. MATERIALS AND METHODS: A prospective database of all patients who had thyroidectomy by the endocrine surgery team was reviewed between August 2012 and April 2020. Per institutional protocol, all patients having thyroid surgery were

Unusual presentation of hepatitis B serological markers in an Amerindian community of Venezuela with a majority of occult cases.

BACKGROUND: Occult hepatitis B infection (OBI) is characterized by the presence of hepatitis B virus (HBV) DNA in the absence of HBsAg in the serum of patients. The aim of this study was to characterize HBV infection among a Piaroa community, an Amerindian group which exhibits significant evidence of exposure to HBV but relatively low presence of HBsAg, and to explore the presence of OBI in this population. RESULTS: Of 150 sera, with 17% anti-HBc and 1.3% HBsAg prevalence, 70 were tested for the presence of HBV DNA. From these, 25 (36%) were found positive for HBV DNA by PCR in the core regio

Wait Time Modifications for Black Transplant Candidates Affected by Race-Based Kidney Function Estimation.

IMPORTANCE: Until 2021, national guidelines upheld race-based equations that assigned higher kidney function estimates to Black patients, delaying subspecialist referral and transplant waitlisting. In 2023, the Organ Procurement and Transplantation Network (OPTN) mandated that US kidney transplants programs submit wait time modifications for Black candidates who were disadvantaged by these equations. OBJECTIVE: To evaluate whether implementation of the OPTN wait time modification policy was associated with changes in kidney transplant rates by race and ethnicity in the US. DESIGN, SETTING, A

Predictors and Correlates of Depression and Anxiety Symptom Trajectories in a Large Digital Mental Health Provider: Retrospective Analysis of Data From Rula Health.

BACKGROUND: Depression and anxiety are highly prevalent and burdensome, yet many individuals, especially those with subclinical symptoms, remain underserved by traditional care models. While digital mental health interventions (DMHIs) have improved access, few integrate high-frequency measurement-based care (MBC) or evaluate outcomes across the full spectrum of symptom severity in real-world settings. OBJECTIVE: The purpose of this study was to examine the effects of participation in a commercial MBC DMHI, Rula Health, on changes in depression and anxiety over time in both subclinical and cli

Association of Neighborhood Racial and Ethnic Composition and Historical Redlining With Built Environment Indicators Derived From Street View Images in the US.

IMPORTANCE: Racist policies (such as redlining) create inequities in the built environment, producing racially and ethnically segregated communities, poor housing conditions, unwalkable neighborhoods, and general disadvantage. Studies on built environment disparities are usually limited to measures and data that are available from existing sources or can be manually collected. OBJECTIVE: To use built environment indicators generated from online street-level images to investigate the association among neighborhood racial and ethnic composition, the built environment, and health outcomes across

Racial/Ethnic Disparity in Association Between Fetal Alcohol Syndrome and Alcohol Intake During Pregnancy: Multisite Retrospective Cohort Study.

BACKGROUND: Alcohol consumption during pregnancy is associated with a range of adverse birth-related outcomes, including stillbirth, low birth weight, preterm birth, and fetal alcohol syndrome (FAS). With more than 10% of women consuming alcohol during pregnancy worldwide, it is increasingly important to understand how racial/ethnic variations affect FAS onset. However, whether race and ethnicity inform FAS risk assessment when daily ethanol intake is controlled for remains unknown. OBJECTIVE: This study aimed to assess racial/ethnic disparities in FAS risk associated with alcohol consumption

Early trajectory of clinical global impression as a transdiagnostic predictor of psychiatric hospitalisation: a retrospective cohort study.

BACKGROUND: Identifying patients most at risk of psychiatric hospitalisation is crucial to improving service provision and patient outcomes. Existing predictors focus on specific clinical scenarios and are not validated with real-world data, limiting their translational potential. This study aimed to determine whether early trajectories of Clinical Global Impression Severity are predictors of 6 month risk of hospitalisation. METHODS: This retrospective cohort study used data from the NeuroBlu database, an electronic health records network from 25 US mental health-care providers. Patients with
