The timing of intravenous crystalloid administration and incidence of cardiovascular side effects during spinal anesthesia: the results from a randomized controlled trial.

UNLABELLED: We conducted a randomized clinical trial to evaluate the efficacy of crystalloids in preventing spinal-induced hypotension (SIH) and cardiovascular side effects (CVSE) in a group of surgical patients. Participants were assigned to receive lactated Ringer's solution at 1-2 mL/min (Placebo group, n = 142); lactated Ringer's at 20 mL/kg starting 20 min before spinal block (n = 130); or lactated Ringer's at 20 mL/kg starting at the time of spinal block (n = 132). SIH was defined as a decrease of > or = 30% in baseline systolic blood pressure, and CVSE as SIH plus nausea, vomiting, or f

Determination of Incidence and Risk Factors of Hypotension Following Spinal Anesthesia During Cesarean Delivery in Patients With Preeclampsia: A Prospective Observational Study.

Background Preeclampsia is associated with high-risk pregnancies and significant maternal morbidity. Although the incidence of hypotension is lower in preeclamptic women, its occurrence can lead to adverse maternal outcomes due to altered vascular physiology. This study aimed to determine the incidence of hypotension following spinal anesthesia in preeclamptic women and to identify associated demographic, obstetric, and clinical risk factors. Methods This prospective observational study included 180 preeclamptic women undergoing cesarean section under spinal anesthesia. Hypotension was defined

OPTIMIZING SPINAL ANESTHESIA IN URGENT CESAREAN DELIVERY: THE TAYLOR APPROACH IN A PARTURIENT WITH CORRECTED SEVERE SCOLIOSIS AND PULMONARY COMPLICATIONS: A CASE REPORT.

INTRODUCTION: Severe scoliosis with prior corrective spinal surgery poses significant anesthetic challenges in obstetric patients due to altered spinal anatomy and potential respiratory compromise. Conventional neuraxial or general anesthesia may be risky, making alternative approaches necessary for safe cesarean delivery. The Taylor paramedian technique provides an effective option by bypassing distorted midline anatomy. CASE REPORT: A 35-year-old primigravida (G1P0) at 37±1 week's gestation with surgically corrected thoracolumbar scoliosis and restrictive pulmonary disease, complicated by b

A Series of Peripheral Nerve Blocks Combined With Spinal Anesthesia Is Associated With Improved Outcomes Following Total Knee Arthroplasty: A Retrospective Study.

Background Peripheral nerve blocks (PNBs) are commonly used in conjunction with general anesthesia (GA) or spinal anesthesia (SA) during total knee arthroplasty (TKA). SA is associated with reduced complications, hospital length of stay (LOS), and mortality. PNBs improve the time to discharge readiness and patient satisfaction. In 2018, we implemented a set of novel, preoperative, single-injection PNBs used with SA and intraoperative sedation. We hypothesized this regime would reduce postoperative pain, opioid use, and hospital and post-anesthesia care unit (PACU) LOS and would improve ambulat

Estimation of the Effective Dose of Phenylephrine Infusion for Preventing Spinal Anesthesia-Induced Hypotension in Elective Cesarean Deliveries: A Randomized Controlled Trial.

INTRODUCTION: Spinal anesthesia for cesarean delivery may be associated with hypotension and fetal acidosis. Phenylephrine, an α1 adrenergic receptor agonist, is effective in treating hypotension with the benefit of less placental transfer. OBJECTIVE: The objective of this study was to determine the optimum dose of prophylactic phenylephrine infusion for preventing spinal anesthesia-induced hypotension in women undergoing elective cesarean delivery and to assess its effect on fetal outcomes, as measured by APGAR scores at 1, 5, and 10 minutes. METHODOLOGY: Two hundred and forty patients were

Neurophysiological Effects of Electrical Stimulation on a Patient with Neurogenic Bowel Dysfunction and Cauda Equina Syndrome after Spinal Anesthesia: A Case Report.

Neurogenic bowel dysfunction (NBD) is common in patients with cauda equina syndrome (CES). Previous studies have reported that electrical stimulation (ES) improves NBD but more neurophysiologic evidence is required. This case report describes a patient who experienced difficulty with defecation as a result of cauda equina syndrome (CES) that developed after a cesarean section performed 12 years ago under spinal anesthesia. The neurophysiological effects were assessed using the bulbocavernosus reflex (BCR) and electromyography (EMG). Two ES treatments, interferential current therapy and transcu

The influence of depth of anesthesia and blood pressure on muscle recorded motor evoked potentials in spinal surgery. A prospective observational study protocol.

For high-risk spinal surgeries, intraoperative neurophysiological monitoring (IONM) is used to detect and prevent intraoperative neurological injury. The motor tracts are monitored by recording and analyzing muscle transcranial electrical stimulation motor evoked potentials (mTc-MEPs). A mTc-MEP amplitude decrease of 50-80% is the most common warning criterion for possible neurological injury. However, these warning criteria often result in false positive warnings. False positives may be caused by inadequate depth of anesthesia and blood pressure on mTc-MEP amplitudes. The aim of this paper is

The Effect of Prophylactic Infusion of Combined Ephedrin and Phenylephrine on Maternal Hemodynamic after Spinal Anesthesia for Cesarean Section: A Randomized Clinical Trial.

UNLABELLED: Several techniques have been proposed to prevent hypotension in obstetric patients. Ephedrine and phenylephrine are individually used to prevent maternal hypotension; however, each has its own drawbacks. Some researchers have reported that the infusion of combined ephedrine and phenylephrine immediately after spinal anesthesia for cesarean delivery reduces the incidence of maternal hypotension. Other studies have indicated that the combination is not superior to the infusion of an individual agent. The present study aimed to evaluate the effect of prophylactic infusion of ephedrine

Maintenance of the parturient in the left lateral position after spinal anesthesia with plain levobupivacaine for cesarean section reduces hypotension: a randomized study.

PURPOSE OF INVESTIGATION: Hypotension during spinal anesthesia is a main concern in cesarean delivery. The authors hypothesized that keeping parturients in a prolonged left lateral position before turning them to a supine position with left lateral tilt would reduce the incidence of hypotension without jeopardizing the quality of anesthesia. MATERIALS AND METHODS: Randomized comparative unblinded prospective study. This randomized comparative prospective study was conducted at Gaziantep University Hospital between June and December 2011. Sixty parturients undergoing cesarean section were incl

Effects of opium addiction on level of sensory block in spinal anesthesia with bupivacaine for lower abdomen and limb surgery: a case-control study.

BACKGROUND: In clinical practice, the level of sensory block in spinal anesthesia in opium abusers is lower than that in non-abusers because of adaptive changes caused by opium use. OBJECTIVES: The aim of this study was to investigate the level of sensory block resulting from the intrathecal administration of 0.5% bupivacaine in opium abuser patients undergoing lower extremity and lower abdominal surgeries. PATIENTS AND METHODS: A total of 100 patients who were candidates of elective lower extremity orthopedic and lower abdominal surgeries were recruited and assigned to two groups based on t

The effects of crystalloid and colloid preload on cardiac output in the parturient undergoing planned cesarean delivery under spinal anesthesia: a randomized trial.

BACKGROUND: Hypotension after spinal anesthesia for cesarean delivery remains a major clinical problem. Fluid preloading regimens together with vasopressors have been used to reduce its incidence. Previous studies have used noninvasive arterial blood pressure measurement and vasopressor requirements to evaluate the effect of preload. We used a suprasternal Doppler flow technique to measure maternal cardiac output (CO) and corrected flow time (FTc, a measure of intravascular volume) before and after spinal anesthesia after 3 fluid preload regimens. We hypothesized that colloid solutions, compar

Influence of the injection site (L2/3 or L3/4) and the posture of the vertebral column on selective spinal anesthesia for ambulatory knee arthroscopy.

BACKGROUND: We tested the hypothesis that selective spinal anesthesia for ambulatory knee arthroscopy can be accomplished with a small dose of bupivacaine at the L3/4 interspace with or without a head-down tilt of 5 degrees when the patients were in the lateral decubitus position. METHODS: In this double-blind study, 123 patients were randomly allocated to receive spinal anesthesia with 4 mg of hyperbaric bupivacaine inserted at either the L2/3 interspace, while the vertebral column was kept horizontal (L2/3 group), or the L3/4 level, with the vertebral column horizontal (L3/4H) or tilted 5 d
