Iron Deficiency Anemia Associated With Acid-Modifying Medications: Two Cases and Literature Review.

Iron deficiency anemia is often listed among potential adverse effects of gastric acid-suppressive medications, given that gastric acidity promotes intestinal absorption of nonheme iron. Additionally, the antacid calcium carbonate can inhibit iron absorption. However, there is little direct clinical evidence that proton-pump inhibitors, histamine-2 receptor antagonists, or calcium carbonate cause iron deficiency anemia. Most case reports have had substantial limitations (e.g., minimal follow-up and presence of other causes of iron deficiency), and retrospective cohort studies have lacked suffi

The diagnosis and management of globus pharyngeus: our perspective from the United Kingdom.

PURPOSE OF REVIEW: To review recent literature on diagnostic and treatment options for globus pharyngeus. RECENT FINDINGS: There are no controlled studies looking at the use of proton pump inhibitors specifically for globus. The small volume of level I evidence has failed to demonstrate superiority of proton pump inhibitors over placebo for treatment of laryngopharyngeal reflux symptoms (including globus). A recent pilot nonplacebo-controlled study has shown promising results for treating laryngopharyngeal reflux symptoms with liquid alginate suspension. The role of cognitive-behavioural ther

Examination of the potential antiulcer activity of the calcium antagonist propyl-methylenedioxyindene. III. Lack of effect on cysteamine-induced duodenal ulcers in rats.

Since propyl-methylenedioxyindene (pr-MDI) exhibits significant protective effects against stress-induced ulcers in rats at subcardiovascular doses (10-30 mg/kg, i.p.), the aim of the present study was to explore the effect of this intracellular calcium antagonist on cysteamine-induced duodenal ulcers at the same low doses. Duodenal ulcers were induced in rats with a single dose of cysteamine (425 mg/kg, s.c.), which produced an 80% ulcer incidence within 24 h without affecting gastric acid concentration. Administration of pr-MDI (10 and 30 mg/kg, i.p.) at 0, 6 and 12 h post-cysteamine did not

Effects of long-term medical treatment with cimetidine and bethanechol in patients with esophagitis and Barrett's esophagus.

To evaluate long-term medical therapy in patients with Barrett's esophagus, six patients were studied before and after long-term therapy with cimetidine for a mean period of 11.7 months. Some patients also received bethanechol. All had severe symptoms of gastroesophageal reflux resistant to intensive antacid therapy, specialized columnar epithelium by biopsy, and endoscopic evidence of severe inflammation. Esophageal manometry documented a hypotensive lower esophageal sphincter in three patients and low peristaltic amplitude in the distal esophagus in four. Treatment was begun with cimetidine,

Dulcerozine-induced duodenal ulcers in rats: a simple, highly-reliable model for evaluating anti-ulcer agents.

Dulcerozine-induced duodenal ulcers in rats were studied in order to establish the optimum conditions for a routine production of ulcers and to elucidate their possible pathogenesis. Single doses of dulcerozine administered either subcutaneously or orally produced a dose-related duodenal ulceration in rats. The ulcerogenic effect of dulcerozine was most potent when given subcutaneously to 24-hr fasted animals at 300 mg/kg. At this dose, deep or perforating ulcers were consistently produced within 18 hr, but mortality due to general toxicity of the agent was excluded at least up to 48 hr. Feedi

Maintenance treatment of recurrent peptic ulcer by cimetidine.

68 patients with chronic peptic ulcer took part in a double-blind trial in which 32 received the histamine H2-receptor antagonist cimetidine, (400 mg twice daily) and 36 received a placebo, for a year. 6 of those on cimetidine had an endoscopically proven recurrent ulcer within a mean of 7 months, while 30 of those on placebo had one after a mean of 4 months (P less than 0.0005). 1 patient on cimetidine had two recurrences compared with 12 patients on placebo (P less than 0.0005). No patient in the cimetidine group had complications compared to 4 patients in the placebo group, 2 of whom had me

Evidence of aluminum absorption from the gastrointestinal tract and bone deposition by aluminum carbonate ingestion with normal renal function.

The question of whether aluminum is toxic in patients ingesting large oral loads, either for phosphate binding in renal dialysis or as an antacid for peptic ulcer, has been raised. The permeability of the gut barrier for aluminum has not been established because of the lack of precise analytical techniques. Urine aluminum was measured in this study before and during oral aluminum carbonate loading in six subjects with normal renal function, one of which had peptic ulcer disease and a 25-year history of high oral aluminum ingestion. The analytical procedure involved destructive neutron activati

Acid Suppression in Neonates: Friend or Foe?

Medications that reduce gastric acid secretion are commonly prescribed for treating gastroesophageal reflux disease. However, several studies have shown that these medications are not very effective, and are associated with adverse effects. This article discusses the physiology of gastric acid secretion, clinical indications and pharmacology of acid suppressing medications, and possible adverse effects of these medications.

Pepto bismuth associated neurotoxicity: A rare side effect of a commonly used medication.

A 56 years old female with medical history significant for collagenous colitis and GERD for which she was taking Pepto Bismuth for months. She presented with progressive confusion for two weeks, followed by myoclonus, tremors, gait instability and visual hallucinations. Patient was admitted and comprehensive work up was done over a ten day course. This included a CBC, CCP CT head, MRI brain, EEG, Lumbar puncture, and various antibody and serology testing which were all essentially unremarkable. It was noted that patient had been taking OTC Pepto Bismuth chronically for GI symptoms. Based upon

Emerging drugs for gastroesophageal reflux disease.

Proton pump inhibitors (PPIs) are very effective and safe drugs for the treatment of erosive and non-erosive gastroesophageal reflux disease (GERD). Nevertheless, a significant proportion of GERD patients (30 - 40%) continue to suffer from symptoms during PPI treatment, which has stimulated the search for better drugs. Improvement of PPI pharmacokinetics and pharmacodynamics has been the main focus of drug development in the past decade with the ultimate goal of optimizing acid inhibition. New inhibitors of the proton pump with a longer half-life, acting faster and longer, have been developed,

Gastroesophageal reflux disease: natural history and long-term medical and surgical outcomes.

Gastroesophageal reflux disease (GERD) is a chronic condition that affects a large proportion of the adult population. Persistent untreated GERD can lead to esophageal strictures, premalignant Barrett's esophagus, and an increased risk of adenocarcinoma of the esophagus. Currently, the most effective medical treatment targets gastric acid suppression, allowing healing of erosive/peptic lesions and controlling symptoms. Most patients take over-the-counter medication or are successfully treated by their primary care physician. Gastroenterologists and gastrointestinal surgeons see patients with c

[Acute renal failure caused by hypercalcemia].

BACKGROUND: Hypercalcaemia may cause acute renal failure. MATERIAL AND METHODS: We present and discuss two patients with acute renal failure caused by hypercalcaemia. RESULTS: Patient no. 1 was treated with too high doses of vitamin D for hypoparathyroidism. Patient no. 2 had been taking extremely high doses of calcium carbonate for dyspeptic pain. Volume depletion and renal vasoconstriction are the mechanisms that lead to acute renal failure. Long-lasting hypercalcaemia will lead to calcium deposits in the kidneys (nephrocalcinosis), which is known to cause chronic renal failure. INTERPRET
