BACKGROUND: Obstructive sleep apnea (OSA) is a prevalent but often underdiagnosed sleep disorder associated with intermittent hypoxia and fragmented sleep. It has been increasingly implicated in the pathogenesis of cardiac rhythm disorders (CRDs), including atrial fibrillation (AF), bradyarrhythmia's, and ventricular arrhythmias. Despite substantial global evidence, limited Indian data exist regarding the burden and characteristics of CRDs among OSA patients.
OBJECTIVE: The objective is to determine the prevalence and types of CRD in patients diagnosed with OSA and explore their correlation w…

BACKGROUND: Obstructive sleep apnea (OSA) is a prevalent sleep disorder characterized by intermittent hypoxia and oxidative stress, which can impair mitochondrial function and accelerate cellular aging. Mitochondrial DNA copy number (mtDNA-CN) is a useful biomarker of mitochondrial health. We conducted a cross-sectional study investigated differences in mtDNA-CN between individuals with severe OSA and those without severe OSA.
METHODS: We analyzed 97 individuals who underwent overnight polysomnography and blood tests at Taichung Hospital, Taiwan, between March 1 and December 31, 2022. Leukocy…

This overview examines the craniofacial and myopathic endotypes of obstructive sleep apnea in relation to the spatial matrix hypothesis. Recently, attention has been drawn to the phenotype of obstructive sleep apnea since this condition includes neurologic, metabolic, craniofacial and myopathic endotypes. Most often, practitioners of dental sleep medicine focus their attention on the manipulation of the mandible for therapeutic purposes, but the etiology of the craniofacial endotype may reside elsewhere. Specifically, in line with the cranio-caudal gradient of development, consideration needs …

Obstructive sleep apnoea-hypopnea syndrome (OSAHS) is characterized by the recurrent occurrence of apnoea and hypopneas during sleep. OSAHS is an independent risk factor for cardiovascular diseases, including hypertension, coronary artery disease, arrhythmias, and stroke. The high prevalence of OSAHS among patients with psychiatric disorders, along with the reciprocal links between these two categories of conditions, deserve emphasis. This association indeed carries diagnostic, prognostic, and therapeutic implications. Diagnosis poses specific challenges, including difficulties to perform slee

Obstructive sleep apnea (OSA) is a heterogeneous disorder with varying clinical presentations and pathophysiologic mechanisms. Multiple studies demonstrated increased cardiovascular morbidity and mortality, impaired daytime function and quality of life in moderate to severe OSA. New sleep study metrics are being developed to better predict OSA-related cardiovascular risk that may help to target therapy to patients most likely to benefit. Novel treatments, including hypoglossal nerve stimulation, as well pharmacologic approaches, provide options to personalize therapy.

PURPOSE: Empirical evidence suggests that the duration of the Cutaneous Silent Period (CuSP) is significantly extended in pathological states such as Idiopathic Parkinson's Disease (IPD), Restless Legs Syndrome (RLS), and dystonia, all of which are correlated with the degeneration of dopaminergic neurons. This neurodegenerative process is a critical factor in the etiology of Obstructive Sleep Apnea (OSA). In light of this active participation, the present study sought to assess the CuSP in patients with OSA.
METHODS: A cohort comprising forty individuals diagnosed with OSA and forty healthy c…

Obstructive sleep apnea (OSA) is a sleep-related breathing disorder characterized by recurrent collapse of a structurally vulnerable upper airway during sleep. Airway caliber and stability are influenced by multiple anatomical structures, including the nasal cavity, soft palate, lateral pharyngeal walls, tongue base, hyoid complex, and craniofacial skeleton. Detailed knowledge of these structures is therefore essential for rational surgical planning. This narrative review summarizes key anatomical features relevant to upper pharyngeal surgery, including nasal obstruction and the internal nasal…

Obstructive sleep apnea (OSA) syndrome is a severe, debilitating, and pervasive sleep disorder. OSA mainly affects people with obesity, type 2 diabetes mellitus (T2DM), hypertension, and dyslipidemia and is strongly associated with cardiovascular complications. Based on the bidirectional relationship between T2DM and OSA, the latter represents a risk factor for the former, and, vice versa, people with T2DM have a high risk of OSA. Mechanical and hormonal factors, inflammatory mediators, and a dysregulated autonomic nervous system contribute to the mechanisms underlying the disease. Treatment o…

A dysregulated circadian system is independently associated with both Obstructive Sleep Apnea (OSA) and cardiovascular disease (CVD). OSA and CVD coexistence is often seen in patients with prolonged untreated OSA. However, the role of circadian dysregulation in their relationship is unclear. Half of the human genes, associated biological pathways, and physiological functions exhibit circadian rhythms, including blood pressure and heart rate regulation. Mechanisms related to circadian dysregulation and heart function are potentially involved in the coexistence of OSA and CVD. In this article, w…

UNLABELLED: In the management of obstructive sleep apnea, the integrative concept of care is gathering momentum. This is an encouraging development to prevent deficiencies or errors due to a lack of professional collaboration since risks, benefits and alternatives of various techniques can be discussed candidly. However, while the deployment of a multidisciplinary team is encouraged, a scientific, evidence-based approach needs to be rigorously engendered to encompass the phenotypic heterogeneity of obstructive sleep apnea, including neurologic, metabolic, craniofacial, and myopathic endotypes.…

The interrelationship between asthma, obesity, and obstructive sleep apnea (OSA) presents a critical area of investigation within sleep medicine, given the rising prevalence of these conditions globally. This article explores the multifactorial interactions among these three disorders that contribute to significant morbidity. Asthma, a chronic inflammatory condition of the airways, is one of the most common chronic respiratory conditions globally. Asthma in people with obesity is associated with poor asthma control, increased asthma severity, and an increased frequency of exacerbations. Obesit…

Obstructive sleep apnoea-hypopnoea syndrome (OSAHS) is a heterogeneous condition whose clinical presentation varies according to sex. The available data, primarily from male cohorts, underestimates the specific characteristics of women and female, which are influenced by hormonal changes throughout their life. In women, the clinical presentation often differs from the more classical pattern observed in men, contributing to underdiagnosis and delayed treatment. Better characterisation of female phenotypes is essential in order to adapt diagnostic and therapeutic strategies. This article summari
