The evaluation and initial management of arthritis.

This article reviews the appropriate workup of the patient presenting with complaints referable to a joint, including the important information to be obtained during the interview and physical examination, as well as from laboratory and imaging studies. The clinical presentation of osteoarthritis, rheumatoid arthritis, gout, and joint sepsis are discussed, and the treatment approach for each is outlined. The specific indications for surgical intervention in the course of arthritis also are detailed.

Autoimmune Haemolytic Anaemia (AIHA) Secondary to Yersinia enterocolitica Septicemia: The Second Case Reported in the Literature.

UNLABELLED: Yersinia enterocolitica infection is an uncommon but potentially fatal zoonosis, especially when it culminates with septicaemia. Post-infectious complications like reactive arthritis and erythema nodosum are well described in the literature. On the other hand, the association between yersinosis and autoimmune haemolytic anaemia (AIHA) has only been established in one clinical case. We present a case of an 87-year-old man admitted for AIHA, whose was successfully treated only after the identification and treatment of yersinosis. LEARNING POINTS: Autoimmune haemolytic anaemia is a p

[Successful management of cold agglutinin syndrome developing subsequent to rheumatoid arthritis with immunosuppressive therapy].

A 58-year-old man was admitted with shortness of breath in September 2019. He had a severe hemolytic anemia with a high cold agglutinin (CA) titer. He also had arthralgia and finger deformation. He was diagnosed with cold agglutinin syndrome (CAS) secondary to rheumatoid arthritis (RA) based on the clinical course. Occasionally, CAS has been reported to occur in parallel with collagen disease, infectious disease, or malignant tumor. CAS developing secondary to collagen disease occurs less frequently than that to infectious disease or malignant tumors. Furthermore, CAS caused by RA is very rare

Rheumatoid factor revisited.

PURPOSE OF REVIEW: Initial studies of the pathogenesis of rheumatoid arthritis focused on the role of rheumatoid factor and immune complex-associated vasculitis and synovitis. Subsequent work has delineated T cell responses, the role of cytokines, chemokines, and the aggressive nature of rheumatoid synovitis. Recent findings underscore the importance of humoral immunity in this entity and are the subject of this review. RECENT FINDINGS: By the discovery of anti-cyclic citrullinated peptide, anti-RA33, and anti-GPI antibodies in the human and mouse systems, respectively, the impact of humoral

Purulent pericarditis in a patient with rheumatoid arthritis treated with etanercept and methotrexate.

Purulent pericarditis is rarely seen in this post-antibiotic era. We report a case of spontaneous purulent pericarditis in a patient with rheumatoid arthritis being treated with etanercept, a tumour necrosis factor-alpha (TNF-alpha) antagonist, and methotrexate, an immunosuppressant. Both are disease-modifying anti-rheumatic drugs. We discuss the pathophysiology of purulent pericarditis and the recent literature on the infectious complications of TNF-alpha antagonists.

[Lyme borreliosis. The manifestations of the disease from the internal medicine and rheumatology viewpoints].

Lyme disease is an infectious syndrome transmitted by tick bites. Of the multiple manifestations, cardiac and joint involvement are of primary interest for internists and rheumatologists. Up to 10% of patients develop cardiac symptoms, most frequently a disturbance of atrioventricular conduction. The characteristic presentation of the disease, however, are of symptoms of arthritis in one or several large joints. This review summarizes Lyme disease with emphasis on presenting symptoms important to internists and rheumatologists.

Adult immunodeficiency and rheumatic disease.

Several rheumatic illnesses may develop in adults with primary or acquired immunologic defects. Individuals with primary defects in B-cell antibody production, such as common variable immunodeficiency of selective IgA deficiency, and those with defects of complement components, have an increased prevalence of systemic lupus erythematosus--like syndromes or other autoimmune diseases. Defects in immunoglobulin production may be complicated by inflammatory or infectious arthritis. Infection with HIV-1 may be associated with diverse rheumatic conditions, including diffuse infiltrative lymphocytosi

[Imaging of infectious sacroiliitis].

About 11 cases of various infectious sacroiliitis, the authors discuss the clinical, biological and radiological findings. Acute and subacute infectious sacroiliitis can be associated with another arthritis and are often accompanied by soft tissues lesions. Radiological investigations are very useful for the diagnosis, because of the unspecific clinical and biological findings. Radiography shows advanced osteoarticular remodelling but the subtle lesions are often unapparent. Bone scintigraphy shows a focal hypercaptation but is unspecific. Tomography and CT point out the erosions. With CT, sof

Coexistence of polyarteritis nodosa and psoriatic arthritis in a child: an unreported association: Polyarteritis nodosa and Psoriatic arthitritis.

Polyarteritis nodosa (PAN) is a non-frequent vasculitis of small- and medium-sized vessels. Psoriatic arthritis (PA) is described as a "unique inflammatory arthritis associated with psoriasis" with an inexact prevalence rate due to the lack of widely accepted classification criteria. We describe the case of an 11-year-old boy that presented with fever of unknown origin plus clinical signs of sacroiliitis, bilateral enthesitis of the plantar fascia, and skin psoriasis. He acutely developed erythematous and tender nodular lesions in the lower limbs that lasted around 3 weeks and spontaneously di

The role of HLA-B27 polymorphism and molecular mimicry in spondylarthropathy.

Ankylosing spondylitis (AS), reactive arthritis (ReA) and other related spondyloarthropathies (SpAs) are characterized by a strong association with the major histocompatibility complex allele HLA-B27. Experimental evidence from humans and transgenic rodents suggests that HLA-B27 is itself involved in the pathogenesis of SpA. Population and peptide-specificity analysis of HLA-B27 suggest it has a pathogenic function related to antigen presentation. Putative roles for infectious agents have been proposed in ReA and suggested in AS. However, the mechanism by which HLA-B27 and bacteria interact to

Early diagnosis and treatment of acute brucellosis knee arthritis complicated by acute osteomyelitis: two cases report.

BACKGROUND: Brucellosis is an endemic systemic infectious disease, the most common complication is bone and joint involvement. Sacroiliac joint and spinal joint are the most frequently involved sites in adults, but knee joint infection is rare, and acute infectious knee arthritis complicated by acute osteomyelitis is even extremely uncommon in adults. Here, we report two cases of acute septic knee arthritis complicated by acute osteomyelitis caused by Brucella melitensis (B. melitensis). CASE PRESENTATION: Both patients had a history of traveling in animal husbandry areas within three months.

Chlamydia trachomatis nucleic acids can be found in the synovium of some asymptomatic subjects.

OBJECTIVE: The recent identification of antigens or nucleic acids of infectious agents in the joints of patients with reactive arthritis has raised questions about whether chlamydial or other infectious agent nucleic acids are also present in normal joints. We had the opportunity to study synovium from 30 asymptomatic volunteer subjects by use of polymerase chain reaction (PCR) for attempted identification of Chlamydia and other infectious agents. METHODS: All subjects had blind needle synovial biopsies with the Parker-Pearson needle. DNA was extracted and PCR performed using primers for Chla

Primary Septic Arthritis of The Knee due to Mycobacterium Tuberculosis in a Previously Healthy Child.

Pediatric septic arthritis can be a devastating disease. Often, the diagnosis can be challenging as autoimmune and infectious causes may present in a similar fashion. Thus, we present the case of a five-year-old male patient, from the Pacific coast of Colombia, with chronic morning knee pain and stiffness thought to be caused by an autoimmune disease. He presented with a mild effusion of the left knee, a flexed posture, and limited extension to 25°. Inflammatory markers demonstrated an infectious pattern. Autoimmune markers were negative. A diagnostic arthrotomy and lavage was conducted follow
