It is established that laparoscopic cholecystostomy presents the most acceptable method of gall bladder decompression in acute cholecystitis patients of advanced age comprising a high-risk group for surgery. In order to establish the animal model of papillary insufficiency, 10% formalin solution (0.5-1.0 ml) was injected into duodenal papillae just after cholecystectomy in ten dogs. We have evaluated the incidence and evolution of sludge, microlithiasis and lithiasis formation of the biliary tract in 12 patients who underwent total gastrectomy and postoperative total parenteral nutrition (TPN) beginning immediately after operation. Benign diseases of the major duodenal papilla were detected in 77 of 120 patients (64.1%) reoperated upon after cholecystectomy in 1968-1984. Active surgical tactics for diseases of the papilla resulted in less amount of reoperations in this disease (from 7.8% to 2.7%).
