Reconceptualizing Informed Consent as the Objective Activity of Real Person.

This paper argues that viewing informed consent as an autonomous action is problematic because it is based on an idealized conception of the person and of autonomy. Instead, informed consent should be viewed as the objective activity of real, concrete persons. In this activity, an individual, acting as a subject, authorizes a specific object for a particular purpose. A person's choice is fundamentally constrained by the reality of both the object and the subject (the individual themselves). Accordingly, individuals must understand the nature of the object and its relevance to their own situati

Seizures, Cultural Beliefs, and Ethical Tensions.

This case narrative explores the ethical challenges that arise when culturally rooted surrogate decision-making conflicts with standard biomedical care. A 20-year-old Somali man presented with suspected autoimmune encephalitis requiring urgent immunosuppressive treatment to prevent permanent brain injury. Lacking decisional capacity due to encephalopathy, his mother served as his surrogate.Guided by religious and cultural beliefs, his family attributed his condition to demonic possession and declined medical treatment in favor of a spiritual healing ritual. This decision generated profound mor

The Ethical and Psychosocial Dimensions of Palliative Care in Prison : A Scoping Review.

BACKGROUND: With prison populations expanding and aging, palliative care (PC) is an urgent concern. People in prison (PIP) face chronic illness, mental health conditions, and social vulnerabilities, creating ethical and psychosocial challenges to dignified end-of-life (EOL) care. OBJECTIVE: This scoping review mapped key ethical and psychosocial domains in the provision of PC in prisons. METHODS: PubMed, Web of Science, and ScienceDirect were searched using the strategy: Population-individuals in prison; Concept-ethical and/or psychosocial issues in PC; Context-PC provision in prisons across

Finding Community and Losing the Plot: Social Media, Healthcare, and Patient Experience.

This commentary examines 12 first-person narratives of sharing health information on social media. Online storytelling simultaneously offers vital support and imposes concerning pressures. The authors describe how digital platforms mitigate isolation by fostering community, solidarity, and practical "community-driven knowledge," particularly around stigmatized conditions, disability, and reproductive health. Across many of the authors' narratives, we see that peer networks can supplement clinical care by providing expertise that shapes treatment decisions and offers support. At the same time,

Camouflaging in autism as a dual-normative construct: a philosophical critique.

Social camouflaging-broadly defined as the suppression or modulation of autistic traits to conform to dominant ableist social norms-has become a dominant construct in autism research. Yet despite its empirical and cultural prominence, the concept has not undergone sustained philosophical scrutiny. This paper develops a fourfold critique. Ontologically, camouflaging lacks stability, as it fails to identify a coherent class of phenomena distinguishable from adjacent behaviours. Epistemologically, it relies on theory-laden and circular methodologies that presuppose what they purport to measure. L

Ibsen's An Enemy of the People and the Ethics of Public Health Disclosure.

Henrik Ibsen's An Enemy of the People (1882) dramatizes a physician's discovery that his town's spa waters are contaminated. His efforts to disclose this danger lead to a collapse of civic support, as the truth threatens the town's economic and political interests. Beyond being a literary classic, the play serves as a case study in the ethics of public health disclosure. This article places the drama in the context of nineteenth century medical history and explores why Dr. Stockmann fails to communicate the truth effectively. Drawing on virtue ethics, public health frameworks, and organization

Facilitating Decision Making at End of Life: How Physicians' Experiences Can Inform the Work of Clinical Ethicists.

BACKGROUND: End-of-life treatment decisions can be morally complex and may lead to contention. Clinical ethicists are often consulted to address uncertainty or conflict during end-of-life decision-making. This study explores physicians' perceptions of challenges to end-of-life decision-making to help inform clinical ethics practice. METHODS: Semi-structured interviews were conducted with surgeons, nephrologists, intensivists, emergency physicians, and oncologists regarding challenges and barriers to effective end-of-life decision-making. Interview transcripts were thematically analyzed. RESU

The second patient: family suffering and the ethics of dependency in later life.

Clinicians have long used the phrase the hidden patient to describe the relative who cares for a declining elder and whose own health quietly fails in the process. The moral philosophy of ageing has not taken the phrase seriously. It has concentrated on the older person, on her autonomy and her dignity, and has left the family that sustains her at the margin of attention. This paper argues that the phrase is more than a clinical courtesy. When serious illness and frailty arrive in later life they characteristically produce a second patient, the carer, whose suffering is real, measurable, gende

When Dialogue Fails: Arguing With the Rabbi.

It has conventionally been assumed that there are no limits to ethical discourse: that is, that no matter how difficult or fraught the questions being asked some common ground can always be found. This has been thought to arise from fundamental shared values and the conditions of possibility of dialogue itself. Contemporary events have put this assumption in doubt and raised questions about when and where dialogue might cease to be possible. This article examines some aspects of the conditions of, and obstacles confronting, ethical discourse today, focusing on the Kantian model, as represented

Strengthening an Ethical Framework and Research Practice to Better Link Global Health Research to Health Equity : A Case Study of the SPOT-Impact Consortium.

An ethical framework titled "Research for Health Justice" has been developed that provides global health researchers with guidance on how to design their research to promote health equity and global justice. This study sought to test the framework's guidance by comparing it to the governance and experiences of the "Severe Pre-eclampsia adverse Outcome Triage" (SPOT)-Impact consortium-a transdisciplinary maternal health research consortium. The aim was to identify ways in which the "Research for Health Justice" framework could learn from the consortium's insights and practices and to identify h

The Algorithm's Blindspot: An Ethical Critique of Data-Fetishism and the Deskilling of the Clinical Gaze in Ophthalmology.

The rapid integration of artificial intelligence (AI) in ophthalmology has produced remarkable diagnostic accuracy while simultaneously raising significant ethical and epistemic concerns. This article argues that the prevailing AI paradigm fosters a form of data-fetishism: the tendency to treat datasets and algorithmic outputs as objective truth, privileging measurable metrics over the patient's lived experience and clinical context. Inspired by Marx's concept of commodity fetishism, the analysis demonstrates how this phenomenon produces "algorithmic substitution": a process through which the

How Should Ongoing Surgical Care Needs be Met During Global Health Engagements?

The Department of Defense has a long history of Global Health Engagement, beginning with identifying and developing vaccines for yellow fever, influenzae, and malaria. Contemporary examples of global health engagement include the worldwide deployment of military assets to aid in natural disasters, and deployment of ship and land based medical teams for short-term medical and surgical engagements. In 2017, the Department of Defense Instruction on Global Health Engagement was published to help guide these efforts. Surgery is an indispensable part of global health and presents unique ethical chal

Should people be able to have access to medical assistance in dying to avoid living with Alzheimer's? Opinions from Mexico and Colombia.

Growing population aging is accompanied by a growing fear of suffering dementia. Four hundred and thirty-six Mexican and Colombian adults completed a survey about their opinion on Medical Assistance in Dying (MAID) for patients with dementia, both in the early stage of the disease and in the advanced stage through an advance directive. In Colombia, MAID is allowed while in Mexico it is banned. The main reasons given by those who agreed with MAID were "right to decide" and "avoiding suffering." Religious beliefs were the main reason for disagreeing. More Mexicans than Colombians agreed with MAI

Navigating Ethical Crossroads in Pediatric Ophthalmology.

Baby J is a 10-month-old infant who presented at birth with bilateral opacification of the cornea and elevated eye pressure. She was diagnosed with a severe form of glaucoma due to a genetic mutation. Her condition can be managed with surgery and daily medications. However, studies on her specific condition report poor outcomes and frequent post-operative complications. Despite receiving the first-line surgeries, Baby J's pressure remained difficult to control, and her visual prognosis remained uncertain. The parents, who are both blind due to genetic causes, no longer wished to pursue surgica

'An Unimaginable Challenge': A Cross-Cultural Qualitative Study of Ethics and Decision-Making Around Tracheostomy Ventilation in Patients with Amyotrophic Lateral Sclerosis.

BACKGROUND: The rate of tracheostomy with invasive ventilation (TIV) for patients with Amyotrophic Lateral Sclerosis (ALS) varies widely. Previous studies have shown that doctors' values may affect decision-making. There have been no previous international qualitative comparisons of medical decision-making process for TIV or why practice varies. METHODS: We conducted semi-structured in-depth interviews with 16 doctors actively involved in the management of ALS patients from Japan (n = 7), the UK (n = 5), and the US (n = 4). We used three hypothetical cases to explore decision-making. Conversa

Why We Should Recognize AI as an Inventor.

It is now possible for AI systems to generate novel inventions without meaningful human direction and control. Should such inventions be patented? The prevailing consensus, confirmed in recent test cases and official guidance, is that patent law only covers inventions by natural persons (i.e., humans). This, however, sometimes creates an odd situation in which AI-generated inventions cannot be patented, nor can the humans responsible for those systems gain patent rights indirectly through the operation of the law. In this article, we argue against this prevailing consensus. We present five rea

Institutional Responses to Voluntary Assisted Dying: An Empirical Study in Victoria and Western Australia.

Patients seeking to access voluntary assisted dying (VAD) are necessarily at the end of their lives. Hence, they are likely to be concurrently receiving care from institutions (community nursing services, health services, palliative care services, and aged care facilities) with different levels of participation in VAD. This article reports on the various institutional approaches to VAD based on eighteen semi-structured interviews with regulators from Victoria and Western Australia, representing fifteen institutions with varying levels of support for VAD. We generated five main themes from refl

End of life in Italy: ethical issues and medical-legal aspects of medically assisted suicide.

The debate on euthanasia and medically assisted suicide remains a complex issue, full of ethical, moral and legal implications, which has aroused strong emotions and conflicting opinions both in Italy and around the world. In Italy, the legal context surrounding euthanasia and medically assisted suicide is still characterized by an absence of specific rules regulating these practices. This has led to a situation in which the discussion on the end of life remains open and the practice of euthanasia remains illegal as the only interruption of life-sustaining treatments is allowed at present in a
