Treatment Persistence and Factors Associated With Discontinuation in Real-World Biologic Therapy for Chronic Rhinosinusitis With Nasal Polyps.

BACKGROUND: Biologic therapies are effective for severe, uncontrolled CRSwNP, yet real-world data on treatment persistence remain limited. METHODS: This retrospective single-center study included 95 adults with CRSwNP who initiated dupilumab (88.4%), mepolizumab, or omalizumab between 2021 and 2025. The primary outcome was persistence with the index biologic therapy. Discontinuation was a permanent cessation, switching, or a ≥ 90-day dosing gap. Switching ended index-biologic persistence but not biologic therapy overall. Response was graded with European Position Paper on Rhinosinusitis and N

Breakthrough Psoriasis in Patients Receiving Biologicals.

BACKGROUND: Biological therapies, including TNF-alpha, IL12/23, IL17 and IL23 antagonists, adequately control a very high number of patients with moderate-to-severe psoriasis with an excellent long-term safety profile. However, on occasion, patients on biological therapy with stabilized disease or complete remission report episodes of sudden breakthrough psoriasis. AIM: To study prospectively in a monocentric tertiary setting, the clinical characteristics of patients presenting a sudden breakthrough psoriasis although completely stabilized (PASI 90-100) under biological therapy. MATERIALS AN

Bioterrorism and catastrophe response: a quick-reference guide to resources.

BACKGROUND: Dentists' responses to catastrophe have been redefined by bioterrorism. Informed response requires accurate information about agents and diseases that have the potential to be used as weapons. METHODS: The authors reviewed information about the most probable bioterrorist weapons (those from the Center for Disease Control and Prevention's Category A) from the World Wide Web and print journals and distilled it into a resource list that is current, relevant to dentistry and noncommercial. The Web sites cited include those sponsored by federal agencies, academic institutions and profe

Biologics in Older Adults with Chronic Obstructive Pulmonary Disease: A Narrative Review.

Background: Chronic obstructive pulmonary disease (COPD) disproportionately affects older adults. While targeted biologic therapies have shown efficacy in type 2 eosinophilic endotypes, elderly patients remain underrepresented in pivotal clinical trials. This narrative review examines the pathophysiological rationale, efficacy data, and unique geriatric challenges of biologic therapies in older adults. Methods: A literature search was conducted in PubMed and Scopus for phase II and III randomized controlled trials (RCTs) and observational studies evaluating biologic agents (anti-IL-5, anti-IL-

Ultrasound-mediated modification of biological properties of Sepia esculenta ink polysaccharide SIP4.

Ultrasonication is increasingly used as an eco-friendly method to depolymerize natural polysaccharides for improving physicochemical properties and bioactivities of the native macromolecules. However, a systematic evaluation is required to determine how degradation-induced alterations in polysaccharide molecular properties modulate their biological functions. To this end, this study focuses on SIP4, a bioactive sulfated glycosaminoglycan from Sepia esculenta ink, to clarify the physicochemical changes caused by ultrasonic depolymerization and evaluate their subsequent impact on biological acti

Smart drug delivery systems for immunotherapeutic agents: focus on biological/biogenic platform-based intelligent delivery in cancer immunotherapy.

Cancer immunotherapy marks a major advance in oncology, yet its effectiveness is constrained by challenges such as inefficient tumor targeting, immune-related toxicities, low response rates, and an immunosuppressive tumor microenvironment (TME). To overcome these obstacles, smart drug delivery systems (SDDSs) have been developed to deliver immunotherapeutic agents in a targeted, controlled manner in response to specific stimuli. This review summarizes recent advances in SDDSs for cancer immunotherapy, focusing on biological and biogenic platforms. These systems respond to endogenous and exogen

[Trichinellosis outbreaks in Aragón (1998-2017)].

OBJECTIVE: In Aragon, official veterinary controls to detect the presence of Trichinella in meat products have allowed trichinosis to be a low-incidence disease in the form of outbreaks associated with family consumption of meat products that are not sanitarily controlled. The aim of the work was to describe the epidemiological characteristics of the trichinosis outbreaks that occurred in Aragon during the study period and the action measures carried out, which served as a basis for redesigning and strengthening prevention and control measures. METHODS: Descriptive study of the characteristic

Beyond cold to hot: oncolytic virotherapy as the next cornerstone of immuno-oncology.

Oncolytic virotherapy has evolved from a platform predicated on lysis of cancer cells into a sophisticated system for intratumoural immune reprogramming; the prevailing 'cold-to-hot' paradigm captures only part of this potential. A fundamental limitation shared by immune checkpoint inhibitors (ICIs) and early-generation oncolytic viruses (OVs) is a reliance on pre-existing tumour-specific T cells (TSTs), which presents an immunological ceiling that constrains antitumour activity - given that these agents have a limited capacity to generate TSTs de novo. Next-generation OVs overcome this constr

Bighorn sheep pneumonia: sorting out the cause of a polymicrobial disease.

Pneumonia of bighorn sheep (Ovis canadensis) is a dramatic disease of high morbidity and mortality first described more than 80 years ago. The etiology of the disease has been debated since its initial discovery, and at various times lungworms, Mannheimia haemolytica and other Pasteurellaceae, and Mycoplasma ovipneumoniae have been proposed as primary causal agents. A multi-factorial "respiratory disease complex" has also been proposed as confirmation of causation has eluded investigators. In this paper we review the evidence for each of the candidate primary agents with regard to causal crite

Recent advances in targeted drug delivery systems for multiple myeloma.

Despite significant therapeutic advances, multiple myeloma (MM) remains a challenging, incurable, hematological malignancy. The efficacy of traditional chemotherapy and currently available anti-MM agents is in part limited by their adverse effects, which restrict their therapeutic potential. Nanotherapeutics is an emerging field of cancer therapy that can overcome the biological and chemical barriers of existing anticancer drugs. This review presents an overview of recent advancements in nanoparticle- and immunotherapy-based drug delivery systems for MM treatment. It further delves into the ta

Hepatitis B-related autoimmune manifestations.

This article focuses on autoimmune manifestations related to the hepatitis B virus (HBV). Although the HBV vaccination has resulted in the decline of the virus, approximately 400 million individuals are infected worldwide. Up to twenty percent of the afflicted may develop extrahepatic manifestations ranging from the severe polyarteritis nodosa to the many, varied, and less severe clinical and biologic forms. Currently, control of the viral infection is mainly based on the use of antiviral drugs (with the current availability of potent agents). Discussion of two hypotheses of the pathophysiolog

β-caryophyllene and docosahexaenoic acid, isolated or associated, have potential antinociceptive and anti-inflammatory effects in vitro and in vivo.

Inflammation is a complex biological response involving the immune, autonomic, vascular, and somatosensory systems that occurs through the synthesis of inflammatory mediators and pain induction by the activation of nociceptors. Staphylococcus aureus, the main cause of bacteremia, is one of the most common and potent causes of inflammation in public health, with worse clinical outcomes in hospitals. Antioxidant substances have been evaluated as alternative therapeutic analgesics, antioxidants, anti-inflammatory agents, antitumor agents, and bactericides. Among these, we highlight the essential

[The abuse and dependence of chemical agents in an evolutionary approach/From adaptation to the phenomenon of "run-away" and beyond].

A recent paradigm shift in medicine resulted in the emergence of so-called evolutionary medicine, which studies the interactions between the environment and biological phenomena in a timeline to understand diseases and develop new treatments. In the course of evolutionary discussion of chemical substance abuse, the issue was approached through five steps. The biological level helps to understand the interference and coevolution of chemical agents and biochemical mechanisms, thus demonstrating the vulnerability toward chemical agents. The psychological level approaches behavior and its change,
