[Research on the role of life events in bipolar disorder].

This review focuses on latest research regarding the role of life events in bipolar disorder. Description of important patients experiences and mechanisms, according to which they operate, is the key issue in diagnostics, therapy, prevention and planning of further studies. Definitions of life events differ depending on theoretical conceptions. At the same time they can be divided on the basis of time relation between the occurrence of event and disorder. Events that can play important role in bipolar disorders are the following: early childhood traumas, early loss of important family member a…

Prospective longitudinal course of aggression among adults with bipolar disorder.

OBJECTIVES: Bipolar disorder (BP) has been associated with increased aggressive behaviors. However, all existing studies are cross-sectional and include forensic or inpatient populations and many do not take into account the effects of comorbid conditions. The goal of this study was to evaluate the longitudinal course of aggression among adult outpatients with BP compared with non-BP patients and healthy controls. METHODS: Subjects with bipolar I disorder (BP-I)/bipolar II disorder (BP-II) (n = 255), those with non-BP psychopathology (n = 85), and healthy controls (n = 84) (average 38.9 years…

Perinatal factors and the risk of bipolar disorder in Finland.

BACKGROUND: Complications during the perinatal period have been associated with neurodevelopmental disorders like schizophrenia and autism. However, similar studies on bipolar disorder (BPD) have been limited and the findings are inconsistent. The aim of this study was to examine the association between perinatal risk factors and BPD. METHODS: This nested case-control study, based on the Finnish Prenatal Study of Bipolar Disorders (FIPS-B), identified 724 cases and 1419 matched controls from population based registers. Conditional logistic regression was used to examine the associations betwe…

The comorbidity of ADHD and bipolar disorder: any less confusion?

The clinical confusion surrounding childhood ADHD and bipolar disorder centers on overlaps between severe ADHD with mood lability and mania/hypomania. Perplexity has been exacerbated by the removal of mood symptoms from the diagnostic criteria for ADHD and a lack of stringent criteria for a manic/hypomanic episode. This review summarizes current knowledge of the relationship between ADHD and bipolar disorder, the rates with which ADHD and bipolar disorder coexist in youth of differing ages, their presence in community, clinical, and high risk samples, and their longitudinal course. Treatment s…

Mood state dependency of dysfunctional attitudes in bipolar affective disorder.

INTRODUCTION: Studies of cognitive styles among euthymic people with bipolar affective disorder (BAD) without use of mood induction techniques to access those cognitive styles give misleading impressions of normality of those cognitions. The aim of this study was to assess dysfunctional attitudes of participants with BAD, and control participants with no previous psychiatric histories, after mood inductions. METHODS: Sad and happy moods were induced within 49 BAD and 37 controls. Dysfunctional attitudes were measured following mood inductions using the Dysfunctional Attitude Scale-short form

Prodromes, precipitants, and risk factors for relapse in bipolar disorder.

OBJECTIVE: The purpose of this qualitative review is to examine prodromes, precipitants, and risk factors for repeated episodes of mania and depression in bipolar disorder. METHODS: PubMed, EMBASE, and PsychInfo were searched for "bipolar disorder" in conjunction with: "prodromes", "triggers", and "life change events". RESULTS: Phenomenology and prevalence of prodromes, precipitants, and risk factors are described, and their therapeutic implications are outlined. CONCLUSIONS: Most patients with bipolar disorder are able to recognize their prodromes. This ability depends largely on insight. …

Family-focused treatment for children and adolescents with bipolar disorder.

The course of bipolar disorder in children and adolescents is highly recurrent and impairing. This article describes the adaptation of family-focused treatment (FFT) for children and adolescents with bipolar disorder. FFT is given in 21 sessions over 9 months, and is usually initiated during the recovery period following an acute episode of depression or (hypo)mania. The treatment consists of an engagement phase followed by psychoeducation, communication enhancement training, and problem-solving skills training. Results of randomized trials in adults and adolescents find that patients with bip…

[Mood stabilizers in the treatment of bipolar disorder mixed states].

INTRODUCTION: Mixed states represent a frequent issue in the context of bipolar disorders (up to 40% of acute phases). They are complex and serious clinical pictures, which pose diagnostic challenges but also require specific treatment interventions. The purpose of this article is to provide an overview of the current literature on the pharmacotherapy of bipolar mixed-states, focusing on mood stabilizers. METHODS: Literature was identified by searches in MEDLINE (Medical Literature Analysis and Retrieval System Online), using PubMed search engine. Studies are chosen if they contained the keyw…

Pharmacological treatments for bipolar disorder: present recommendations and future prospects.

In selecting and adapting medications to treat the specific clinical features of a patient with bipolar disorder (BPD) over time, a foundation strategy is to have good working knowledge of up-to-date practice guidelines. The World Federation of Societies of Biological Psychiatry Guidelines has the reasoned advantage of weighing safety/tolerability as high as efficacy. Most successful treatments for BPD start to separate from placebo within 1 week; most differences between regimens occur within the first 4 weeks. This observation extrapolates to a strategy of discontinuing or adding a second dr…

Stigma: a core factor on predicting functionality in bipolar disorder.

OBJECTIVE: Extant literature indicates that bipolar disorder (BD) is associated with significant poor psychosocial functioning. However, the relationship between functioning and demographic and clinical variables is unclear. The aim of this study is to investigate the predictors of functioning such as demographic and clinical variables, social support, self-perceived stigma, and insight in remitted patients with BD. METHODS: Eighty patients with a diagnosis of BD, complete remission according to the criteria of Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition were include…

[Mania in late life: bipolar disorder as diagnosis by exclusion].

BACKGROUND: The underlying cause of mania in later life can be an early- or late-onset bipolar disorder or it can be a mood disorder arising from a physical illness, also known as 'a secondary mania'. Thorough diagnostic tests are needed to differentiate between the two types of mania. AIM: To discuss the epidemiology, presentation, diagnostic considerations and treatment options when a mania develops in later life. METHOD: The literature was studied systematically with the help of PubMed, the Cochrane Library, specialist manuals, Dutch guidelines and references. RESULTS: So far, research i…

Comparison of women with confirmed versus presumably misdiagnosed bipolar disorder.

OBJECTIVE: Because bipolar disorder can be difficult to diagnose, we compared characteristics of women with confirmed versus presumably misdiagnosed bipolar disorder. METHOD: This cohort study was conducted from July 2005 to January 2010 in the outpatient clinic of the Emory Women's Mental Health Program, Atlanta, Georgia. Young adult women (mean age = 32 years) who were either pregnant or planning to conceive and who reported having previous clinical diagnoses of bipolar disorder completed 2 independent diagnostic assessments: the Structured Clinical Interview for DSM-IV Axis I Disorders (SC…

[Affective temperaments and psychopathological dimensions of personality in Bipolar Disorder].

OBJECTIVES: The first objective of our study is the evaluation of correlation between affective temperaments and psychopathological dimensions in a clinical sample of patients with Bipolar Disorder and the individualization of possible differences within the three diagnostic subtypes (bipolar I, II and cyclothymia). The second one is to observe whether any specific temperament may influence the number of hospitalizations or the age of the bipolar depression onset. MATERIALS AND METHODS: At the Bipolar Disorder Unit of Policlinico Gemelli (Rome, Italy) a group of 60 patients with Bipolar Disor…

SHORT COMMUNICATION: Apolipoprotein E genotype and cognition in bipolar disorder.

Apolipoprotein E (APOE) has been extensively studied as a risk factor for sporadic and late onset Alzheimer's Disease (AD). APOE allele (∗)3, the most frequent variant, is not associated to cognitive dysfunction (CD) or to increased AD risk. Differently, the (∗)4 allele is a well-established risk factor for CD, while the (∗)2 allele is associated with survival and longevity. CD is an important feature of Bipolar Disorder (BD) and recent data suggest that CD may be one of its endophenotypes, although controversial results exist. The aim of this research is to study the association of APOE genot…

[History of bipolar disorder: a consideration on ancient western literature].

We can find the description suggesting pathological shift from melancholia to mania in Diseases I (4th century BC) of Corpus Hippocraticum. However, it was 2nd century that detailed account concerning bipolar disorder in modern sense was supposed to appear. Especially the text of Aretaeus of Cappadocia contained several paragraphs indicating this disease. Thereafter in 6th century, Alexander of Tralles clearly mentioned the transition from melancholia to mania. In ancient world, both melancholia and mania embodied vast area of general mental derangement on the one hand, they had restricted mea…

Pediatric bipolar disorder: evidence for prodromal states and early markers.

BACKGROUND: Childhood bipolar disorder remains a controversial but increasingly diagnosed disorder that is associated with significant impairment, chronic course and treatment resistance. Therefore, the search for prodromes or early markers of risk for later childhood bipolar disorder may be of great importance for prevention and/or early identification. METHODS: Literature searches were conducted to identify reviews, case reports and empirical papers addressing the issue of prodromes of childhood bipolar disorder. RESULTS: A total of 54 articles were found that related to bipolar prodromes,…

Impaired nerve growth factor homeostasis in patients with bipolar disorder.

OBJECTIVE: Neuro-trophins are critically involved in neuro-plasticity, the impairment of which is a major role-player in bipolar disorder (BD), and their altered levels have been recently advocated in the patho-physiology of this affective malady. The aim of this study, therefore, was to evaluate the plasma levels of nerve growth factor (NGF) in BD patients in comparison with control subjects. METHODS: Forty-nine BD type-I individuals (30 in mania and 19 in euthymia) and 36 healthy controls were assessed by Mini-plus, Young mania and Hamilton depression rating scales. NGF levels were detected…
