Distraction osteogenesis in secondary microsurgical mandible reconstruction: report of seven cases.

INTRODUCTION: Fibula, ilium and scapula are the preferred donor areas for vascularised bone grafts. For secondary mandibular reconstruction, … olidation phase of 4-5 months. RESULTS: Ten of the 13 distractions were uneventful with an average bone lengthening of 20mm, retained after a follow-up time of 6-47 months (average 27 months). The intermaxillary relationship and the facial profile were improved in all cases. Distraction plate fractures, screw loosening or absence of new bone formation were the co …

Treatment planning and biomechanics of distraction osteogenesis from an orthodontic perspective.

As in traditional combined surgical and orthodontic procedures, the orthodontist has a role in the planning and orthodontic support of patients undergoing distraction osteogenesis. This role includes predistraction assessment of the craniofacial skeleton and occlusal function in addition to planning both the predistraction and postdistraction orthodontic care. Based on careful clinical evaluation, dental study models, photographic analysis, cephalometric evaluation, and evaluation of three-dimensional computed tomographic scans, the orthodontist, in collaboration with the surgeon, plans distra

Management of segmental defects by the Ilizarov intercalary bone transport method.

Seventeen patients with segmental skeletal defects were managed with the Ilizarov intercalary bone transport method, whereby an osseous defect is eliminated by elongating one fragment. On average, the regenerate new bone length measured 5.14 cm, corresponding to the creation of new osseous tissue equaling 13.7% of the bone's original length (range, 4.2%-35%). The average time in fixation was 9.6 months, including 4.8 months to transport the bone fragment throughout the limb. Numerous complications were encountered, most commonly wire-site sepsis and fixator instability. No serous nerve or vess

Complications common in motorized intramedullary bone transport for non-infected segmental defects: a retrospective review of 15 patients.

Background and purpose - Since the introduction of intramedullary bone transport nails only very few cases have been reported in the literature. Thus we evaluated the results and complications in a single institution retrospective cohort.Patients and methods - 15 (median age 40 years (18-70), 8 males) consecutive patients, were included and the electronic patient records and radiographs were reviewed. Complications were severity graded and categorized as device or non-device related. Results - The segmental bone loss was due to non-union site in 8 femurs and 4 tibias, or traumatic bone loss in

The results of 39 fractures complicated by major segmental bone loss and/or leg length discrepancy.

Thirty-nine fractures with segmental bone loss and/or limb length discrepancy with defects of 1.8 to 12.0 cm were reviewed retrospectively. Most patients presented with an acute open fracture with segmental bone loss or a late reconstructive problem with a nonunion, frequently complicated by infection, segmental bone loss, and adjacent soft-tissue disruption. One hundred forty-two weeks was the average length of time of treatment before referral of a patient to us. Eighty-eight per cent of the patients received a bone graft that included either autologous cancellous bone, bank bone, a vascular

[Osteomyelitic pseudarthrosis of the tibia with loss of substance treated with a graft of the opposite fibula].

Dragan Dj./Re. No. 2755/76, 5,5-year-old boy, referred to this hospital with signs of bipolar osteomyelitis, destruction and fistulisation of the left tibia and talocrural arthritis. Staphylococcus albus and streptococcus haemoliticus were isolated from the fistula. Osteomyelitis was treated with an adequate therapy including long usage of Lincocin. In Feb. and Oct. of 1977 a sequestrotomy of the mid portion of the tibia and transmedular fixation using Ratschmann's nail were performed. The process resulted in pseudarthrosis of the mid portion of the tibia with loss of bone substance in the len

[Are there any indications for surgery of distal radius malunion in elderly patients?].

Malunions of the distal radius are associated most of the time with disturbances of the distal radioulnar (DRU) joint. While a corrective osteotomy of the distal radius is a well-known procedure in younger patients, manual workers and active patients, is it justified in independent elderly patients above 70 years of age? These techniques improve function, reorient the articular surfaces, protect against complications such as degenerative osteoarthritis and stiffness of the wrist, and they improve DRU balance. Osteotomies performed with bone or artificial grafts have comparable outcomes. Either

Spinal osteotomies to treat post-traumatic thoracolumbar deformity.

Fractures of the thoracolumbar junction can lead to regional kyphosis, this being a significant cause of pain and disability for the patients. After a traumatic fracture of the thoracolumbar spine, early or late regional kyphosis can be observed. This post-traumatic deformity can, however, be corrected with appropriate surgical methods. Posterior tricolumnar osteotomies are some of the most powerful methods of correction and are particularly indicated when sagittal and coronal deformities have to be simultaneously corrected or when anterior surgery is not possible. Anterior corpectomy and leng

Lower limb deformities in children: two-stage correction using the Taylor spatial frame.

The Taylor spatial frame is a new external ring fixation system developed for correction of axial, sagittal, planar and rotational deformities of the extremities. Between August 1999 and October 2000 five children were treated by this system. Four had shortened and deformed femurs after fracture, osteomyelitis or a congenital short femur. One patient had a pseudoarthrosis of the tibia. Mean corrections were leg lengthening 5.9 cm (1.7-7.2), valgus correction 10.6 degrees (0-19), lateral translation 12.6 mm (0-33) and rotation 10 degrees. The mean healing index was 48.4 days/cm. In all patients

A Systematic Review of the Systematic Utilitarian Procedure for Extremity Reconstruction of the Hip (SUPERhip) for Congenital Femoral Deficiency with Reference to Recent Extra-network Cohort Findings.

AIMS AND BACKGROUND: Congenital femoral deficiency (CFD) is a rare limb anomaly requiring Hip reconstruction before femoral lengthening. The Systematic Utilitarian Procedure for Extremity Reconstruction of the Hip (SUPERhip), introduced by Paley in 1997, is the principal protocol for type Ib and type II CFD. No systematic review has consolidated outcomes in this population. This review synthesised radiographic outcomes, complications and protocol fidelity. Retrograde Femoral Lengthening Below a Total Hip Arthroplasty.

BACKGROUND: Limb length discrepancy (LLD) after total hip arthroplasty (THA) is a common occurrence and can lead to back pain, disordered gait, and decreased functional outcomes. Femoral lengthening ipsilateral to a THA using a retrograde motorized intramedullary lengthening nail (MILN) is a hip-sparing option for limb equalization. There has been little published on the technique and results of this method. METHODS: We retrospectively reviewed all patients at our institution who underwent unilateral femoral lengthening using a retrograde MILN ipsilateral and distal to a THA between April 201

Replantation and Lengthening of a Lower Leg in a 7-Year-Old Child: A Case Report.

Replantation of the lower leg has controversial indications, but it may be considered in carefully selected patients. Although the function of prosthetic lower legs has been improved in recent decades, leg salvage remains a laudable goal. We present the case of a 7-year-old child who sustained a traumatic amputation at the level of the middle tibia with loss of the middle portion of the lower leg. We performed successful replantation, and tibia lengthening was performed starting 10 days after replantation and lasted 6 months.

How precise is the PRECICE compared to the ISKD in intramedullary limb lengthening? Reliability and safety in 26 procedures.

BACKGROUND AND PURPOSE: The PRECICE intramedullary limb lengthening system uses a new technique with a magnetic rod and a motorized external remote controller (ERC) with rotational magnetic field. We evaluated the reliability and safety of the PRECICE system. METHODS: We compared our preliminary results with PRECICE in 24 patients (26 nails) with the known difficulties in the use of mechanical lengthening devices such as the ISKD. We used the Paley classification for evaluation of problems, obstacles, and complications. RESULTS: 2 nails were primarily without function, and 24/26 nails length

Lower limb inequality treatment with subtrochanteric femoral shortening osteotomy fixed with intramedullary nail.

BACKGROUND: The aim of the study is to analyze the results of treating the lower limb inequality with subtrochanteric femoral shortening osteotomy fixed with interlocking intramedullary nail. The analyzed material includes 14 patients aged 16 to 36 (mean 25), in whom femoral shortening osteotomy fixed with interlocking nail was performed. In all patients the indication for this therapeutic method was lower limb inequality caused by femur shortening and accompanied by contraindication for femoral lengthening procedure. Limb length inequality ranged from 1.5 to 6.0 cm (mean 3.8). The follow-up p

[Fat embolism during limb lengthening with a centromedullary nail: three cases].

Several methods are available for progressive limb lengthening, including centromedullary nailing, external fixation, or a combination. Each technique has its own advantages and drawbacks. In trauma victims, use of centromedullary nailing is associated with potentially fatal fat embolism. This fatal outcome might also occur during limb lengthening, particularly in bilateral procedures. To our knowledge, fat embolism has not been reported with the use of centromedullary nail for limb lengthening. This was a multicentric study of three cases of fat embolism, including one fatal outcome. In all,

[Correction of post-traumatic diaphyseal malalignment of the lower extremity].

Post-traumatic deformities do occur as a result of disturbed fracture healing with loss of bone stock, necrosis of fragments or the development of pseudarthrosis resulting in malunion or progressive malalignment. In the majority of the cases these disturbances can be related to technical problems of primary fracture treatment like insufficient reduction or implant failure. On the other hand, complex deformities with involvement of the adjacent joints may also be a result of injuries of the growth plate in childhood. In some cases primary correction is impossible because of critical conditions
