Femoral interference screw placement through the tibial tunnel: a novel method without graft damage.

A frequently encountered problem in endoscopic 1-incision anterior cruciate ligament (ACL) reconstruction is the difficulty involved in accurately inserting the femoral interference screw without significant and undesirable divergence between the screw and the graft when a femoral interference screw is fixed through the anteromedial portal. To minimize divergence, the authors demonstrated a modified, easy, and reproducible procedure that can be performed without causing graft injury or requiring special instrumentation. A ligament reconstruction route is created with the use of an ACL guide sy

Scaphoid fractures: current treatments and techniques.

Scaphoid fractures are among the most common fractures of the bones of the wrist and usually result from a forceful extension of the wrist. If the diagnosis cannot be established by clinical and radiographic examination, bone scans are recommended and are preferred over tomography or MRI, which are more expensive diagnostic procedures. Scaploid fractures should be classified as either undisplaced, stable or displaced, unstable. Nonsurgical treatment with cast immobilization (short arm-thumb spica cast) is recommended for stable fractures; however, there is increased interest in percutaneous sc

Neurocentral synchondrosis fracture in immature spines associated with pedicle screw type fixation devices.

The purpose of this study is to clarify the weak point in immature lumbar vertebrae associated with pedicle screw instrumentation. Ten immature thoracic and lumbar vertebrae were collected from calf spines. After installation of 6- and 7-mm-diameter pedicle screws into the pedicles of each specimen, pullout force was applied to the screw using the uniaxial MTS system until failure. Tightening torque during installation was measured. From the load-displacement curve, failure load was calculated and failure site was confirmed by radiographs. Inner pedicle diameters were measured after the pullou

[Internal fixation of radial shaft fractures: Anatomical and biomechanical principles].

Radius is a critical bone for functioning of the forearm and therefore its reconstruction following fracture of its shaft must be anatomical in all planes and along all axes. The method of choice is plate fixation. However, it is still associated with a number of unnecessary complications that were not resolved even by introduction of locking plates, but rather the opposite. All the more it is surprising that discussions about anatomical and biomechanical principles of plate fixation have been reduced to minimum or even neglected in the current literature. This applies primarily to the choice

Stability of medially and laterally malpositioned screws: a biomechanical study on cadavers.

BACKGROUND CONTEXT: Pedicle screw instrumentation is widely used in spine surgery. Axial screw misplacement is a common complication. In addition to the recognized neurovascular risks associated with screw misplacement, the biomechanical stability of misplaced screws remains a subject of debate. PURPOSE: The present study investigates whether screw misplacement in the lumbar spine reduces mechanical screw hold. STUDY DESIGN/SETTING: Cadaveric biomechanical study. METHODS: Pedicle screw (mis)placement was planned for 12 fresh frozen cadaveric spines between the T12 and the L5 levels. The scr

Improving Outcomes with Sequential Fixation Using Long-Threaded Screws for Valgus-Impacted Femoral Neck Fractures.

Background and Objectives: Valgus-impacted femoral neck fractures (OTA 31B1.1 and 31B1.2) are considered stable fractures with favorable outcomes compared to displaced fractures. However, complications such as femoral neck shortening, screw sliding, and suboptimal recovery can occur, particularly in severe deformities. This study evaluated the outcomes of a sequential fixation technique using short-threaded screws followed by long-threaded screws. Materials and Methods: This prospective study included 135 patients aged 60 years or older with valgus-impacted femoral neck fractures (OTA 31B1.1 a

[Lag screw osteosynthesis in median mandibular fractures--technique and outcome].

The lag screw technique for treatment of simple median and paramedian fractures of the mandible is reemphasized. Major advantages over bone-plate fixation are the rapid application since adapting and bending of a bone plate is obviated as well as the lower costs of the osteosynthesis material. A review of 12 patients with fractures of the anterior mandible who had lag screws placed for fracture treatment showed that it is a successful method of providing rigid internal fixation associated with a low postsurgical complication rate.

Method of bone fixation in reattachment of amputations in the upper extremities.

The object of secure bone fixation in the reattachment of amputated fingers is early motion. The use of specially designed screws and Kirschner wires is essential for this objective. This becomes even more important in the reattachment of fingers than the upper arm or forearm. If there should be non-union of the bone, an autologous bone graft should be applied as a secondary procedure.

A biomechanical comparison of scaphoid screws.

The compressive forces generated by various scaphoid screws were measured in the laboratory using a custom designed load washer (transducer) as a means to compare their fixation capabilities. Testing was done using simulated bones fabricated from polystyrene pipette tubing and Pedilen polyurethane foam and paired cadaver scaphoids. The Herbert scaphoid screw was found to have inferior fixation capabilities compared with screws of a more conventional design. However, its size compatibility was the best of all the screws tested. The Heune scaphoid screw demonstrated the best fixation capability,

Effect of screw tunnels on proximal femur strength after screw removal: A finite element analysis.

BACKGROUND: The presence of screw tunnels in the femoral neck is a problem for patients with proximal femoral fractures after removal of internal fixation. The question of how much does the existence of the screw tunnels affect the strength of the femur and whether the patient needs to be protected with an adjunctive device has been controversial. The objective of this finite element analysis was to determine (1) whether the screw tunnels affects normal weight bearing after removal of internal fixation of a proximal femur fracture, (2) which screw tunnels parameters affect the weight bearing c

The role of Poller screws in intramedullary nailing for lower limb extra-isthmic fractures.

INTRODUCTION: Intramedullary nailing is the preferred treatment for lower limb long bone fractures, but stability is reduced in extra-isthmic fractures due to nail-metaphysis mismatch. Poller screws may help, but their use is controversial. This study evaluates whether Poller screws are associated with improved prognosis in intramedullary nailing for lower limb extra-isthmic fractures and explores their mechanical mechanisms. METHODS: PubMed, EMBASE, Cochrane Library, and Web of Science were searched up to December 2025 using keywords like 'Fracture,' 'Intramedullary nail,' 'Poller screw,' an

Comparison of Partially Threaded and Fully Threaded 4mm Cancellous Screws in Fixation of Medial Malleolar Fractures.

BACKGROUND: Several devices have been described for fixation of displaced medial malleolar fractures. Fully threaded cancellous screws engaging the bone may provide advantages compared to partially threaded screws. This study was designed to compare the clinical results of fully and partially threaded 4 millimeter cancellous screws in fixation of medial malleolar fractures. METHODS: In a randomized clinical trial study 44 patients with displaced closed medial malleolar fractures were randomly divided into two groups. Two fully threaded four millimeter cancellous screws were used for fracture
