Ultrasound in follow-up of the postoperative brain. Preliminary report on six cases.

The authors report on their early results in the ultrasound (US) follow-up of postoperative brain pathology, both supra- and infratentorial. The use of US is dependent on the presence of an acoustic window in the skull after craniotomy. Five patients were investigated via a suboccipital craniotomy and one patient via a temporal skull defect. In the patients with a suboccipital craniotomy the greatest part of the posterior fossa, the 3rd and lateral ventricle could be made visible. In a patient with a temporal craniotomy the involved parts of the brain were demonstrated satisfactorily. If a sku

Changes in transcranial motor evoked potentials during intramedullary spinal cord tumor resection correlate with postoperative motor function.

OBJECTIVE: Intraoperative monitoring of transcranial motor evoked potentials (TcMEPs) has been investigated recently as a means of preventing motor deficits associated with resection of intramedullary spinal cord tumors (IMSCTs). In this study, we hypothesized that changes in the intraoperative MEPs during tumor resection correlate with postoperative motor function deficits. METHODS: A retrospective record review was conducted for 28 patients who underwent resection of an IMSCT using myogenic or muscle-recorded TcMEPs during a 44-month period. Intraoperative MEP recordings and results from pr

Bevacizumab in treating the cystic components of pediatric low-grade gliomas: A report of four patients.

Low-grade gliomas (LGG) are among the most common types of brain tumors in children and young adults. These tumors often consist of solid and cystic components. Bevacizumab is a documented treatment for progressive LGG, yet the impact of therapy on the cystic component of these tumors is unknown. We present four patients with prominently cystic LGG treated with bevacizumab at the time of progression. In each case, the cystic component responded to treatment. This is the first known study to investigate bevacizumab's impact on the cystic component of low-grade gliomas.

[Computer-tomographic cisternography with amipaque].

As compared to ordinary computer tomography, computer-aided tomographic cisternography (CTC) demonstrates better the anatomy of the brain cisterns and at the same time gives an idea of the processes of c.s.f. circulation in dynamic examination. The article discusses the results of examination of 112 patients with various neurosurgical diseases by means of CTC. A detailed classification of the brain cisterns is given. The authors show the advantages of the method in examination of patients with small three-dimentional processes in the chiasma-sellar region, cerebellopontine angle, and posterior

Computed tomography in the diagnosis of posterior fossa lesions.

A series of 3,500 cases examined by computed tomography (CT) of the head were reviewed to identify patients who had symptoms and signs suggestive of neurologic disease in the posterior fossa. Three hundred forty eight such cases (about 10%) were found, and the scans were reviewed to determine the character of lesions that could be detected and the accuracy of the neuroradiologic diagnosis. Overall, 16 errors were made (4.6%), of which 13 were false negatives and 3 were false positives. False negatives included nine tumors, two infarcts, and two arteriovenous malformations. Four of the tumors a

Is 3D-CRT still a reasonable option for high-grade gliomas in contact with the brainstem in the absence of IMRT/VMAT?

INTRODUCTION: High-grade gliomas (HGGs) abutting the brainstem present a planning challenge because target coverage must be balanced against brainstem dose constraints. This study assessed whether three-dimensional conformal radiotherapy (3D-CRT) remains a reasonable option when intensity-modulated radiotherapy (IMRT) and volumetric modulated arc therapy (VMAT) are unavailable, and whether partial-arc VMAT (P-VMAT) offers advantages over full-arc VMAT (F-VMAT) in lateral HGGs. METHODS: Thirty-seven patients with lateral HGGs in direct brainstem contact, treated between 2021 and 2025, were pla

A comparative analysis of the modified Rankin Scale, Karnofsky Performance Status and Kurtzke expanded disability status scale in the perioperative management of patients with brainstem cavernous malformations.

BACKGROUND: The neurological conditions of brainstem cerebral cavernous malformation (BCM) patients are usually ascertained using Karnofsky Performance Status (KPS) or the modified Rankin scale (mRS). However, these scales do not reflect slight changes in brainstem function, because neither KPS nor mRS includes brainstem symptoms such as worsening of swallowing or diplopia. The main problem when managing the BCM patients is that we can neither systematically record neurological changes nor conduct clinical outcome investigations of BCM due to the lack of an adequately detailed assessment syste

Effect of the gamma knife treatment on the trigeminal nerve root in Chinese patients with primary trigeminal neuralgia.

AIM: To understand the mechanism of the gamma knife treating the trigeminal neuralgia. MATERIAL AND METHODS: Using the MASEP-SRRS type gamma knife treatment system, 140 Chinese patients with trigeminal neuralgia (NT) were treated in our hospital from 2002 to 2010, in which the pain relief rate reached 95% and recurrence rate was 3% only. We investigated the effect of the gamma knife treatment on the trigeminal nerve root in 20 Chinese patients with primary trigeminal neuralgia by the magnetic resonance imager (MRI) observation. RESULTS: 1) The cross-sectional area of trigeminal nerve root be

Stereotactic linear radiosurgery for cavernous angiomas.

Optimal management of symptomatic cavernous angiomas (CA) located in the thalamus and the brainstem is problematic. Clinical and radiological (MRI) follow-up series suggest that having hemorrhaged once, recurrent hemorrhage with progressive neurologic dysfunction may commonly occur. We have therefore chosen to treat these lesions when first symptomatic with stereotactic linear radiosurgery (SLR). We now report, after a median follow-up of 27 months, 12 patients with CAs (9 women, 3 men, mean age 40 years) treated in this fashion. Ten patients presented with hemorrhage (3 had more than one hemo

Photophobia, epiphora, and torticollis: a masquerade syndrome.

Three young children presented with photophobia, epiphora, and torticollis as the initial manifestation of a posterior fossa tumor. In each case there was a delay in treatment due to the presumptive diagnosis of a local ocular inflammatory condition. We recommend that children with unexplained photophobia, epiphora, and torticollis undergo an imaging technique to evaluate the posterior fossa.

[Controlled multimodality treatment of brain stem gliomas].

Thirty-eight children were diagnosed as having a brain stem glioma at Nagoya University. Thirty-three patients in our previous series from 1957 to 1983, were treated traditionally with radiation and at late stage with shunting operation for hydrocephalus and/or suboccipital decompression, but not with direct operation for tumors. In general, tumors constantly grew regardless histology and their mean survival time was only 7.0 months even with transient neurological remission. On the other hand, recent five patients since 1984 were treated with prospective multimodality treatment. According to

Malignant astrocytoma arising 10 years after combined treatment of craniopharyngioma.

Craniopharyngioma is the third most common intracranial tumor in childhood. Following surgery, virtually all patients present with hypopituitarism and are at considerable risk of tumor recurrence. Secondary tumors, however, are rare, occurring usually 10 years after diagnosis and associated with poor prognosis. We report on a 5 year-old boy in whom craniopharyngioma was diagnosed due to unilateral visual loss. After surgery he underwent conventional radiation therapy with a total tumor dose of 55 Gy, and had hormonal support with DDAVP, thyroxine, and a variable dose of hydrocortisone. Growth

Sudden infant death syndrome "gray zone" disclosed only by a study of the brain stem on serial sections.

Sudden infant death syndrome (SIDS) "gray zone" or borderline cases are defined as those cases in which it is difficult to establish whether the pathological findings are sufficiently severe to have caused the death. Examination of the brainstem in 103 cases of SIDS disclosed five SIDS "gray zone" cases in which only further investigations of serial sections successfully identified anatomico-pathological findings that likely represent the morphological substrates for a sudden reflexogenic death. A complete autopsy was performed, including close examination of the brainstem and cardiac conducti

[Embryonic stem cell therapy in experimental stroke: host-dependent malignant transformation].

INTRODUCTION: Therapeutic application of embryonic stem cells in neurodegenerative disorders like stroke is widely investigated in preclinical animal models. AIM: The authors studied the therapeutic potential of murine embryonic stem cells in two rodent models of stroke. METHODS: Undifferentiated and predifferentiated stem cells were implanted into the non-ischemic hemisphere of mice and rats following focal brain ischemia. The brains were analysed by immunohistochemistry and histology. The in vitro differentiation of the cells was checked by immunocytochemistry and Western-blot. RESULTS: A

Transoral transclival removal of anteriorly placed cavernous malformations of the brainstem.

BACKGROUND: The natural history of brain stem cavernous malformations is unfavorable because of their high hemorrhage rate and resulting neurological deterioration among patients. However, direct surgery of intrinsic and anteriorly situated cavernomas is hazardous and leads to a bad postoperative outcome because of trauma to lateral and dorsally situated eloquent areas of the brain stem. METHODS: We review the cases of two patients with symptomatic cavernous malformations of the anterior brain stem and describe the usefulness of a transoral-transclival approach. A 23-year-old man developed pr
