The initial experience of left bundle branch area pacing in patients with hypertrophic cardiomyopathy.

PURPOSE: Whether left bundle branch area pacing (LBBAP) could be achieved in patients with hypertrophic cardiomyopathy (HCM) requiring ventricular pacing remains unknown. The present study aimed to investigate the feasibility and effect of LBBAP in HCM. METHODS: Patients with HCM who underwent LBBAP were recruited from November 2018 to September 2021. Clinical characteristics, echocardiographic, and pacing parameters were prospectively collected at baseline and during follow-up. RESULTS: Eleven consecutive HCM patients who attempted LBBAP were included (mean age 64.0 ± 8.7 years, female 45.5

Preliminary study on left bundle branch area pacing in children: Clinical observation of 12 cases.

INTRODUCTION: Left bundle branch area pacing (LBBAP) maintains electrical and mechanical synchronization of the ventricles. It is a relatively physiological pacing mode, with low and stable pacing threshold and wider indications. LBBAP in children has been rarely reported. METHODS: This study observed 12 children attempted LBBAP from January 2019 to January 2021 in the department of pediatric cardiology of Anzhen Hospital prospectively. Clinical data, pacing parameters, electrocardiograms, intracardiac electrograms, echocardiographic measurements and complications were recorded at implant and

Generating Evidence to Support the Physiologic Promise of Conduction System Pacing: Status and Update on Conduction System Pacing Trials.

Conduction system pacing avoids the potential deleterious effects of right ventricular pacing in patients with bradycardia and provides an alternative approach to cardiac resynchronization therapy. We focus on the available observational and randomized evidence and review studies supporting the safety, feasibility, and physiologic promise of conduction system approaches. We evaluate the randomized data generated from the available clinical trials of conduction system pacing, which have led to the recent inclusion of CSP in international guidelines. The scope for future randomized trials will b

Pacing of Specialized Conduction System.

Right ventricular pacing for bradycardia remains the mainstay of pacing therapy. Chronic right ventricular pacing may lead to pacing-induced cardiomyopathy. We focus on the anatomy of the conduction system and the clinical feasibility of pacing the His bundle and/or left bundle conduction system. We review the hemodynamics of conduction system pacing, the techniques to capture the conduction system and the electrocardiogram and pacing definitions of conduction system capture. Clinical studies of conduction system pacing in the setting of atrioventricular block and after AV junction ablation ar

Physiologic lead placement with electroanatomic mapping: A case series.

INTRODUCTION: His bundle pacing (HBP) and left bundle branch area pacing (LBBAP) have emerged as attractive alternatives to traditional biventricular pacing to achieve cardiac resynchronization therapy. Early reported results have been inconsistent, particularly amongst patients in whom initial placement with traditional approaches has been unsuccessful or those with complex anatomy or congenital abnormalities. In this report, we describe the use of three-dimensional electroanatomic mapping (EAM) in five selected cases. METHODS: Five patients from multiple clinical sites underwent EAM-guided

Successful slow pathway ablation in a patient with a rare unroofed type coronary sinus.

We report a case of atrioventricular nodal reentrant tachycardia coexistent with a coronary sinus (CS) anomaly. During a standard electrophysiological study, the CS could not be cannulated despite several attempts. A persistent left superior vena cava angiogram through the left brachial vein confirmed an unroofed type CS. Successful slow pathway ablation from the right posterior paraseptum lesion was achieved using an anatomical approach.

Simultaneous conduction over the fast and slow pathways during induction of atrioventricular nodal reentrant arrhythmia with a rate of less than 100 bpm and infra-His block after radiofrequency ablation of the slow pathway.

Atrioventricular nodal reentrant tachycardia (AVNRT) is the most common form of paroxysmal regular supraventricular tachycardia in adults. It is typically induced with an anterograde block over the fast pathway (FP) and conduction over the slow pathway (SP), with subsequent retrograde conduction over the FP. Rarely, a simultaneous conduction of a premature atrial complex occurs over the FP and SP to induce AVNRT and is called "one for two phenomenon". We present a 46-year-old woman with atrioventricular nodal rhythm with a rate of 95 beats per minute with distinct electrophysiological characte

Exercise induced atrioventricular block with gap phenomenon in atrioventricular conduction.

A 54-year-old man with normal atrioventricular (AV) conduction at rest gave a 4-year history of presyncope during exercise. Treadmill testing showed exercise induced AV block. Electrophysiological study demonstrated rate dependent infranodal AV block and abnormal refractory period of the His-Purkinje system. The gap phenomenon in AV conduction occurred during the programmed stimulation. Supernormal conduction could be considered as the mechanism of the gap phenomenon in this patient.

[Treatment of arrhythmia by intracardiac electrocoagulation].

Since July 1982, 12 patients have undergone catheter ablation for refractory arrhythmias. The ablated pathway was the His bundle in 8 patients, an accessory pathway in 3 patients and a ventricular reentrant circuit in 1 patient. The 8 patients in whom the His bundle was ablated comprised 3 cases of atrial flutter, 3 cases of atrial fibrillation, 1 case of atrio-ventricular tachycardia and 1 case of junctional reentrant tachycardia. Results were complete heart block in 6 of these cases, right bundle branch block and first degree a-v block in 1 case and first degree a-v block in the last case. A

Effects of phenytoin on refractoriness and conduction in the human heart.

Using His bundle electrograms and the atrial (A2) and ventricular extrastimulus (V2) techniques, anterograde and retrograde refractory period studies were performed (in 9 and 12 patients, respectively) before and 10 min after intravenous infusion of phenytoin (DPH; mean plasma level, 17.3 micrograms/ml). DPH had no effect on the duration of the QRS complex or the H-V interval of the sinus beats; it had variable but insignificant effects on the sinus rates and the atrial, A-V nodal, and ventricular muscle refractoriness. With the use of the A2 technique, the effective refractory period (ERP) of

[Methods and results of catheter electrodestruction in treating tachyarrhythmias].

Catheter electrodestruction (fulguration) was used in 56 patients with supraventricular and ventricular tachycardias resistant to conservative treatment. Its procedure and results are described. Transvenous electrodestruction of the His bundle and proximal portions of the atrioventricular junction was performed in 40 patients. Complete atrioventricular block and pacemaker implantation were used in 39 patients. In 3 cases with first- or second-degree atrioventricular block, tachycardiac paroxysms were absent in the presence of an antiarrhythmic therapy. In 1 case, tachyarrhythmia recurred after

Electrophysiological effects of intravenous dantrolene on canine heart.

The electrophysiological effects of an intravenous dantrolene infusion (10 mg kg-1) were evaluated in healthy, anaesthetized dogs by intracardiac electrophysiological study. Dantrolene administration resulted in a significant prolongation of the refractory periods of the right atrium and ventricle, while the functional refractory period of the AV node was not altered. A slight increase of AV nodal conduction, measured as atrial-His bundle interval, without any change in infranodal conduction, measured as His bundle-ventricular interval, was observed during sinus rhythm. Dantrolene had no signi

Avoid It or Reach It? The Cardiac Conduction Axis as a Common yet Divergent Determinant of Patient Outcomes in Transcatheter Aortic Valve Replacement and Conduction-System Pacing.

Transcatheter aortic valve replacement (TAVR)-related conduction injury and the benefit of conduction-system pacing (CSP) are usually considered in isolation, despite depending on one structure: the His-Purkinje axis crossing the basal interventricular septum. We argue that implant position relative to that axis is the dominant modifiable determinant of outcome in both. In TAVR, pacemaker rates remain near 16% in contemporary multicenter practice, while single-center strategies interpreting depth against membranous septum length have reached 3-5%; the odds of pacing rise 25% per millimeter of

Initial Experience With Implantation Left Bundle Branch Pacing Leads Without a Dedicated Three-Dimensional Sheath.

AIMS: Left bundle branch area pacing (LBBAP) has emerged as an alternative to cardiac stimulation via right ventricular pacing and cardiac resynchronization therapy using coronary sinus leads. The approach utilizes dedicated three-dimensional guiding catheters for lead placement. Our objective was to evaluate the feasibility and safety of a simplified approach of implantation of an LBBAP electrode without a dedicated guiding catheter. METHODS: This was a prospective single-center proof-of-concept evaluation. Patients with an indication for dual-chamber pacemaker implantation were consecutivel

Small patients, significant findings: Electrophysiological properties of Bachmann's bundle in pediatric patients.

BACKGROUND: Bachmann's bundle (BB) may potentially play a role in the earlier onset and faster progression of atrial fibrillation in adult patients with congenital heart disease (CHD). It is unknown whether electrophysiological alterations already exist at BB in pediatric patients with CHD and whether they are related to aging. OBJECTIVE: This study aimed to investigate BB electrophysiology in pediatric patients with CHD and assess the impact of age on BB electrophysiology. METHODS: BB mapping was conducted in 55 patients (0.2-17.5 years). Activation patterns, potential voltages, low-voltage

Procainamide administration during electrophysiology study--utility as a provocative test for intermittent atrioventricular block.

The utility of procainamide, up to 10 mg/kg IV, as a provocative test for intermittent high degree atrioventricular (AV) block was evaluated in a total of 89 patients. Forty two patients had resting 1:1 AV conduction but had bifascicular block and a history of syncope. High degree AV block had not been documented in anyone. Before procainamide, the HV interval was greater than 60 ms in 17 of the 42 patients but no patient developed infra-Hisian block with fixed rate atrial pacing or following programmed atrial extrastimuli. Procainamide administration lengthened the mean HV interval by 11.9 ms
