Medial displacement osteotomy for unstable intertrochanteric fractures. Twenty years later.

Fifty-five unstable intertrochanteric fractures were treated with a sliding compression screw apparatus. Thirty-seven cases were treated by anatomic nailing and 18 cases were treated by primary medial displacement osteotomy (MDO) by the method of Dimon and Hughston. The mean age of these patients was 82.5 years. Women out-numbered men seven to one. The times to healing for fractures treated with MDO and anatomic nailing were the same. There were no nonunions. The rate of major complications was similar for both groups; however, trochanteric bursitis was more apparent in the anatomically nailed…

Total hip replacement: a comparison between the transtrochanteric and posterior surgical approaches.

It can be argued with good justification, that the trochanteric complications which stimulated this study could have been avoided by strict … roblems that increase the potential for complications. These problems include: painful trochanteric bursitis, displacement of trochanter, non-union of trochanter, broken wires and, an increase in dislocation rate (12% as compared to 4%). The posterior approach reduces operative time, blood loss, hospitalization, and shortens the period of time to unprotect …

Symptomatic joint hypermobility.

Joint hypermobility is relatively common and has many influences such as age, gender, training, and ethnicity among many. Joint hypermobility may be asymptomatic or symptomatic. It may also be non-syndromic or syndromic. However, "asymptomatic" joint hypermobility may result in repetitive use injury, alter biomechanics of joints at other body sites, or become symptomatic later in life. Symptomatic joint hypermobility can result from soft-tissue rheumatism (e.g. bursitis, tendonitis, etc.) or muscular tension pain due to muscular imbalance. Generalized joint hypermobility (GJH) can be easily as…

Editorial Commentary: Stiff Patients After Rotator Cuff Repair: How Many Had Underrecognized Preoperative Adhesive Capsulitis?

Stiffness after rotator cuff repair is one of the most common sources of disability and one of the most common complications. Smoking, diabetes, Workers' compensation status, and traumatic tears are among the strongest risk factors. This is important information in setting appropriate expectations for both the patient and the surgeon preoperatively. Some of these factors are also associated with preoperative stiffness, so surgeons should maintain a high index of suspicion for concomitant adhesive capsulitis in patients presenting with limited motion, particularly in diabetic and non-English sp

Hypertension and frozen shoulder: A bidirectional 2-sample Mendelian randomization study.

Observational studies have demonstrated an association between hypertension and frozen shoulder (FS). However, there is still a lack of clarity about their possible causal relationship. This study aimed to explore whether a genetic relationship exists between hypertension and FS bidirectional through 2-sample Mendelian randomization (MR) analysis. In this study, we investigated the following using data from genome-wide association studies. Exposure-related genetic variants that are independent of each other, were rigorously screened for use as instrumental variables (IVs). To investigate the c

An injectable thermosensitive hyaluronic acid/pluronic F-127 hydrogel for deep penetration and combination therapy of frozen shoulder.

Frozen shoulder (FS) is a common and progressive shoulder disorder that causes glenohumeral joint stiffness, characterized by inflammation and fibrosis. The treatment options are quite limited, and the therapeutic response is hindered by the fibrous membrane formed by excessive collagen and the rapid removal by synovial fluid. To address these challenges, we designed a hyaluronic acid/Pluronic F-127 (HP)-based injectable thermosensitive hydrogel as a drug carrier loaded with dexamethasone and collagenase (HPDC). We screened for an optimal HP hydrogel that can sustain drug release for approxima

Is there an association between breast cancer and incident adhesive capsulitis of the shoulder? A retrospective cohort study from Germany.

PURPOSE: Preliminary data suggest that women with breast cancer are at particularly high risk of adhesive capsulitis of the shoulder. Therefore, this study aimed to investigate the potential association between breast cancer and adhesive capsulitis in adults from Germany. METHODS: This retrospective cohort study included all women aged ≥ 18 years who were diagnosed for the first time with breast cancer in one of 1,274 general practices in Germany between January 2000 and December 2018 (index date). Women without breast cancer were matched (1:1) to those with breast cancer using a propensity s

Positive effects of neuromuscular exercises on pain and active range of motion in idiopathic frozen shoulder: a randomized controlled trial.

BACKGROUND AND OBJECTIVES: Frozen shoulder (FS) is characterized by pain and significant loss of active and passive shoulder motion. Strengthening exercises are among the standard exercises used for FS. Neuromuscular exercise (NME) effectively improved pain and the range of motion in shoulder. However, no prior research has looked into the effects of NME compared to strengthening exercises in FS rehabilitation. The aim of the present study was to evaluate the effects of NME compared to strengthening exercises on pain and active range of motion (AROM) in individuals with idiopathic frozen shoul

Peripheral Joint Injections.

Peripheral joint injections are a common interventional treatment of peripheral joint-mediated pain, including arthritis, tendinopathy, and bursitis that are not responsive to conservative management. Degenerative changes of articular joints are often related to these symptoms through chronic inflammatory changes, which typically arise due to repetitive trauma, autoimmune disease, or metabolic abnormalities. The primary diagnosis for degenerative disease in the peripheral joints is osteoarthritis but can also include rheumatoid arthritis, gout, and other less common etiologies. Chronic inflamm…

The epidemiology and etiology of adhesive capsulitis in the U.S. Medicare population.

BACKGROUND: Adhesive capsulitis (AC) of the shoulder, also known as frozen shoulder, causes substantial pain and disability. In cases of secondary AC, the inflammation and fibrosis of the synovial joint can be triggered by trauma or surgery to the joint followed by extended immobility. However, for primary AC the inciting trigger is unknown. The burden of the disorder among the elderly is also unknown leading to this age group being left out of therapeutic research studies, potentially receiving delayed diagnoses, and unknown financial costs to the Medicare system. The purpose of this analysis

Does the timing of surgical intervention impact the clinical outcomes and overall duration of symptoms in frozen shoulder?

BACKGROUND: The optimal timing of arthroscopic capsular release in patients with frozen shoulder is controversial. Some surgeons delay surgery in the belief that early surgical intervention results in a poorer prognosis. However, whether early surgical intervention causes inferior clinical outcomes and a longer duration of symptoms in frozen shoulder remains unclear. The objective of this study was to compare the clinical outcomes and overall duration of symptoms in frozen shoulder between patients who underwent early surgical intervention and those subjected to late surgical intervention. Our

Design and Preliminary Evaluation of a Novel Robotic System for Mobilization of Glenohumeral Joint.

Joints mobilization is an essential but subjective treatment in the physical therapy of the patients with joint hypomobility such as frozen shoulder. Recently many instrumented force and displacement indentations have been proposed for assessing and diagnosis of joints stiffness. The devices are not, however, feasible and applicable for use in clinical and therapeutic conditions considering the requirements of the joints mobilization principles in physiotherapy. This paper describes a novel design of a robotic system for mobilization of glenohumeral joint and the preliminary evaluation of mobi
