Diagnosis and Management of Vasoplegia in Temporary Mechanical Circulatory Support: A Narrative Review.

Refractory vasodilatory shock, or vasoplegia, is a pathophysiologic state observed in the intensive care unit and operating room in patients with a variety of primary diagnoses. Definitions of vasoplegia vary by source but are qualitatively defined clinically as a normal or high cardiac index and low systemic vascular resistance causing hypotension despite high-dose vasopressors in the setting of euvolemia. This definition can be difficult to apply to patients undergoing mechanical circulatory support (MCS). A large body of mostly retrospective literature exists on vasoplegia in the non-MCS po

Interactive effects of trait hostility and anger expression on cardiovascular reactivity in young men.

Hostility and anger-expression style are personality traits often associated with elevated cardiovascular reactivity and potential heightened risk for cardiovascular disease. In the present study a sample of 50 young, healthy men were divided into groups low or high on the Cook-Medley Hostility scale and on anger-out from Spielberger's Anger Expression scale. Subjects worked on mental arithmetic and public speaking tasks in counterbalanced order. Heart rate, systolic and diastolic blood pressure and hemodynamic indices were measured at baseline and during the tasks. Hostility and anger-out int

Effects of mild hypercapnia on myocardial injury after out-of-hospital cardiac arrest. A sub-study of the TAME trial.

PURPOSE: Mild hypercapnia did not improve neurological outcomes for resuscitated out-of-hospital cardiac arrest (OHCA) patients in the Targeted Therapeutic Mild Hypercapnia After Resuscitated Cardiac Arrest (TAME) trial. However, the effects of hypercapnic acidosis on myocardial injury in patients with cardiac arrest is unexplored. We investigated whether mild hypercapnia compared to normocapnia, following emergency coronary intervention, increased myocardial injury in comatose OHCA-patients with AMI. METHODS: Single-centre, prospective, pre-planned sub-study of the TAME trial. Patients were

Cardiomyopathies of endocrine origin: A state-of-the-art review.

Endocrine disorders are increasingly recognized as potentially reversible causes of secondary cardiomyopathies, yet they often remain underdiagnosed in clinical practice. These conditions-including thyroid dysfunction, acromegaly, pheochromocytoma, diabetes mellitus, adrenal disorders, among others-can significantly alter cardiac structure and function through hormonal excess, metabolic remodeling, and neurohumoral activation. Hyperthyroidism may lead to high-output heart failure (HF) and atrial fibrillation, while hypothyroidism is associated with diastolic dysfunction, pericardial effusion,

Endovascular repair of contained abdominal aortic aneurysm rupture with aortocaval fistula presenting with high-output heart failure.

Aortocava fistula is a rare condition ranging from 0.22% to 6% of all ruptured aortic aneurysms. Recognition and diagnosis of this entity can often be difficult and requires heightened clinical suspicion to ensure that prompt surgical management leads to a favorable outcome. We herein describe the diagnosis and the technical points of successful endovascular management of aortocaval fistula in the setting of a ruptured abdominal aortic aneurysm.

The impact of arterial cannula direction in central venoarterial extracorporeal membrane oxygenation on aortic hemodynamic characteristics under various perfusion conditions.

OBJECTIVE: To systematically evaluate the influence of cannula direction in central venoarterial extracorporeal membrane oxygenation (CVA ECMO) on aortic hemodynamics and oxygen transport through a multiparametric comparative analysis under varying cardiac output conditions. METHODS: Computational fluid dynamics methods were applied to analyze five cannula directions (Down, Vertical, Inner, Center, Outer) ECMO models under four cardiac insufficiency conditions. Evaluation parameters included hyperoxic blood distribution, residence time, and scalar shear stress (SSS), oxygen saturation distrib

Human adaptation to high-altitude: A contemporary comparison of the oxygen cascade in Andean, Tibetan and Ethiopian highlanders.

Human populations native to high altitude have evolved distinct physiological adaptations to chronic hypoxia. This adaptation is evident in the O2 transport cascade. In this review, with brief inclusion of the related genetic adaptations, we compare the O2 cascade across three well-characterized high-altitude populations: Andeans (Aymara and Quechua), Tibetans/Sherpa and Ethiopians (Amhara and Oromo). We contrast the steps of the O2 cascade: (1) ventilation; (2) pulmonary O2 diffusion; (3) cardiac output and circulation; (4) haematological traits; and (5) tissue O2 utilization. Tibetans exhibi

Lung ultrasound score parallels trends in systemic haemodynamics after PDA ligation: a case series.

UNLABELLED: B-lines in lung ultrasound (LU) are non-specific but highly informative sign of interstitial pulmonary oedema (iPE). Sustained exposure to a high-volume left-to-right patent ductus arteriosus (PDA) shunt is associated with iPE. Cardiorespiratory deterioration, named post-ligation cardiac syndrome (PLCS), may follow surgical ligation between 6 and 12 h post-operatively. We conducted a pilot longitudinal evaluation of peri-procedural LU score change. Infants < 32-week gestational age or < 1500-g birthweight undergoing PDA ligation, where pre-, 1 h and 6-12 h LU were performed, were i

Flow reduction in high-flow arteriovenous fistulas improve cardiovascular parameters and decreases need for hospitalization.

High output heart failure (HF) and pulmonary hypertension have been demonstrated in patients with prevalent arteriovenous (AV) fistulas. Fistulas with flow >2000 mL/minutes are more likely to induce changes in cardiac geometry and pulmonary artery pressure. The effects of reducing flow in AV access and its implications on HF decompensation and hospitalizations have not been studied. Retrospective analysis of 12 patients who needed hospitalization for acute Congestive Heart Failure (CHF) decompensation with AV access flow of 2 L/minutes (as defined by Kidney Disease Outcomes Quality Initiative

Eplerenone improves kidney function and cardiac performance in obese male mice.

AIMS: Renin-angiotensin-aldosterone system (RAAS) activation mediates obesity-associated cardiorenal dysfunction. Due to an incomplete understanding of the molecular mechanisms, the clinical use of RAAS antagonism in obesity-associated cardiometabolic complications remains uncertain. The present study investigated the effects and associated molecular mechanisms of eplerenone, a selective mineralocorticoid receptor antagonist, in the heart and kidney of obesity. METHODS: Male C57BL/6 mice were fed a high-fat diet (HFD) for 12 weeks before receiving either vehicle or eplerenone treatments for 3

Hemodynamic effects of synchronized high-frequency jet ventilation compared with low-frequency intermittent positive-pressure ventilation after myocardial revascularization.

BACKGROUND: The purpose of this prospective study was to examine the effect on cardiac performance of selective increases in airway pressure at specific points of the cardiac cycle using synchronized high-frequency jet ventilation (sync-HFJV) delivered concomitantly with each single heart beat compared with controlled mechanical ventilation in 20 hemodynamically stable, deeply sedated patients immediately after coronary artery bypass graft. METHODS: Five 30-min sequential ventilation periods were used interspersing controlled mechanical ventilation with sync-HFJV twice to control for time and

Coronary Microvascular Dysfunction in Acute Cholestasis-Induced Liver Injury.

BACKGROUND: Previous studies have shown cardiac abnormalities in acute liver injury, suggesting a potential role in the associated high mortality. METHODS: We designed an experimental study exploring the short-term effects of acute cholestasis-induced liver injury on cardiac function and structure in a rodent bile duct ligation (BDL) model to elucidate the potential interplay. Thirty-seven male Sprague-Dawley rats were subjected to BDL surgery (n = 28) or served as sham-operated (n = 9) controls. Evaluation and Management of Pulmonary Hypertension in Kidney Transplant Candidates and Recipients: Concepts and Controversies.

Although cardiac evaluation before kidney transplantation commonly focuses on coronary artery disease, a comprehensive pretransplant cardiac evaluation must consider other prognostically important cardiac conditions including functional and structural heart disease. Pulmonary hypertension (PH) is increasingly recognized among patients with kidney failure and may be driven by left heart failure, high cardiac output from arteriovenous fistula, hypoxic lung diseases, and metabolic derangements associated with renal disease. In this article, we examine several key concepts and controversies releva

Maternal cardiovascular impairment in pregnancies complicated by severe fetal growth restriction.

Fetal growth restriction and preeclampsia are both conditions of placental etiology and associated to increased risk for the long-term development of cardiovascular disease in the mother. At presentation, preeclampsia is associated with maternal global diastolic dysfunction, which is determined, at least in part, by increased afterload and myocardial stiffness. The aim of this study is to test the hypothesis that women with normotensive fetal growth-restricted pregnancies also exhibit global diastolic dysfunction. This was a prospective case-control study conducted over a 3-year period involvi

Aortic valve replacement after percutaneous valvuloplasty--an approach in otherwise inoperable patients.

OBJECTIVES: Cardiac surgery can relieve symptoms and improve the prognosis of patients with aortic stenosis. However, surgery-related morbidity and mortality are high when severe cardiac or extracardiac factors co-exist. We report the outcome of percutaneous balloon aortic valvuloplasty (BAV) as a bridge to aortic valve replacement (AVR) in temporarily inoperable patients in our institution. METHODS: All patients who, between January 2002 and December 2006, underwent a bridging percutaneous BAV and had an AVR performed 8-14 weeks later, were included in the study. Clinical and investigational

Hemodynamic profiles of arterial hypertension with ambulatory blood pressure monitoring.

Blood pressure (BP) measurements obtained during a twenty-four-hour ambulatory blood pressure monitoring (24 h ABPM) have not been reliably applied to extract arterial hemodynamics. We aimed to describe the hemodynamic profiles of different hypertension (HT) subtypes derived from a new method for total arterial compliance (Ct) estimation in a large group of individuals undergoing 24 h ABPM. A cross-sectional study was conducted, which included patients with suspected HT. Cardiac output, Ct, and total peripheral resistance (TPR) were derived through a two-element Windkessel model without having
