A new concept of motion preservation surgery of the cervical spine: PEEK rods for the posterior cervical region.

BACKGROUND: Laminectomy may cause kyphotic postoperative deformity in the cervical region leading to segmental instability over time. Laminoplasty may be an alternative procedure to laminectomy, as it protects the spine against post-laminectomy kyphosis; however, similar to laminectomy, laminoplasty may cause sagittal plane deformities by destructing or weakening the dorsal tension band. OBJECTIVE: Using finite element analysis (FE), we attempted to determine whether a posterior motion preservation system (PEEK posterior rod system concept) could overcome the postoperative complications of la

Splitting of the semispinalis capitis muscle as a less invasive approach for atlantoaxial fusion - A technical note.

PURPOSE: Atlantoxial fusion with screw rod constructs traditionally requires subperiostal dissection of the C1/C2 muscle attachments, which may lead to postoperative axial neck pain and intraoperative blood loss. We evaluate a potentially less invasive, muscle splitting approach for atlantoaxial fusion in a cadaver model. METHODS: A technical feasibility study was performed in a human cadaveric specimen with standard surgical instruments and an operating microscope. Surgically relevant anatomical structures as well as final exposure of the C1, C1/C2 and C2 screw entry points are demonstrated.

Cervical spine giant cell bone tumor: a case report.

BACKGROUND: Giant cell tumors (GCTs) of the bone are locally aggressive primary bone tumors with a benign character. Spinal involvement is rare and they are quite rare in the cervical spine. CASE PRESENTATION: A 31-year-old male patient presented with neck pain. Cervical CT revealed a lytic lesion extending posteriorly and causing the collapse of the C4 vertebra corpus. The patient underwent excision of the tumor extending from the anterior to the posterior with a single-stage anterior intervention followed by the placement of an anterior cage and plate-screw system for fusion. The pathology

Cervical spinal deformity in craniometaphyseal dysplasia.

A child with craniometaphyseal dysplasia had the presenting symptoms of progressive quadriparesis. She exhibited proportionate tall stature, peculiar face with craniomegaly, genu valgum, and 46,XX,t(12;18)(q13;q12) chromosome aberration. Delayed physical development and mild mental retardation were also present. Subluxation of C-2 on C-3 and kyphosis of the cervical spine, and myelographic blockage at this level were noted. Treatment consisted of fusion of the C-2 to C5-6 vertebra following laminectomies of C-3 and C-4 with satisfactory results. Early detection and surgery for cervical spine d

[Guideline-conform treatment of injuries to the subaxial cervical spine].

Injuries to the subaxial cervical spine are increasing and have an increased neurological risk compared to the thoracic and lumbar spines. The current treatment recommendations according to the therapeutic recommendations of the Spine Section of the German Society for Orthopedics and Trauma Surgery (DGOU) as well as the S1 guidelines of the German Trauma Society (DGU) are presented. This second part of the article describes the correct indications and treatment planning for injuries to the cervical spine. Based on the AOSpine classification for subaxial cervical spine injuries, decisions can b

A new method for the radiographic evaluation of spondylolisthesis.

The methods of evaluating the degree of spondylolisthesis in present use are based solely on morphological criteria related to the alignment of the profile of the vertebral bodies. The authors maintain that these methods often result in errors due to false positives or false negatives. To remedy this they propose a method of qualitative and quantitative evaluation based on the alignment of the barycentres of the vertebral bodies, with a correction to take account of their different angles of inclination. If this is expressed as a percentage of body width, the degree of slippage can be exactly

A New Automated AI-Assisted System to Assess Cervical Disc Herniation.

STUDY DESIGN: An algorithm was developed with MATLAB platform to automatically quantify the volume of cervical disc herniation (CDH) based on the sagittal magnetic resonance images. This automated program was used for CDH data set, and then compared with manual measurement results confirming its reliability. OBJECTIVE: The aim was to develop a new software for automated CDH volume measurement. SUMMARY OF BACKGROUND DATA: CDH compresses the spinal cord, regarding as the leading cause of cervical myelopathy. However, the CDH volume, of great value to clinical symptoms, can be only manually mea

[A Case of Cervical Metastasis from Breast Cancer].

A 59-year-old female was performed a left mastectomy with axillary lymph node dissection. Final diagnosis of the surgical specimen was left breast cancer pT2N1M0, Stage ⅡB, Luminal type. She was treated with adjuvant endocrine therapy, however, chest wall recurrence was identified at 1 year and 3 months after surgery, and curative resection of this tumor and radiotherapy were performed. Nine months later, she was admitted to the hospital for cervical pain and dyspnea, and magnetic resonance imaging showed bone metastasis in cervical vertebra which compressed spinal cord. Although cervical fusi

Intradural Dirofilariasis in a Dog with Chronic Cervical Pain.

A 7 yr old female spayed Yorkshire terrier was referred to the author's institute for a 5 mo history of recurrent cervical spinal pain. Neurologic examination did not reveal any deficits. Hematologic and serum analyses were within normal limits. Thoracic radiographs that incorporated the cervical spine did not show structural abnormalities. Magnetic resonance imaging of the cervical spine demonstrated a contrast enhancing, intradural extramedullary lesion at the level of the C2 vertebra. Hemilaminectomy was performed, during which a long, narrow nematode was visualized upon opening of the dura

Unilateral hyperplasia of the left posterior arch and associated vertebral schisis at C6 level.

We report on a 5-year-old girl with unilateral hyperplasia of the left posterior arch of C6 associated with spina bifida occulta at the same level. Anteroposterior and lateral radiographs of the cervical spine showed hypertrophy of the left lamina as well as overgrowth and elongation of the left spinous process of the sixth cervical vertebra. Computed tomography (CT) examination better depicted this congenital variant and clearly showed the associated schisis of the posterior arch at the same level. Magnetic resonance (MR) imaging examination ruled out other spinal anomalies. The neck pain, th

[Spinal manipulation--treatment associated with a high risk of complications].

The article consists in a review of injuries for which claims were submitted to one or another of three insurance companies during a two-year period, and which had arisen in conjunction with manipulation of the cervical spine (n = 21), thoracic spine (n = 6), lumbar spine (n = 13), or the sacro-iliac joint (n = 14). Cervical spine manipulation had caused injury of the vertebral artery with resulting paralysis in three cases, and cervical disc herniation in three cases. Lumbar spine manipulation had caused disc herniation in six patients, of whom three had severe persistent problems. Of the 14
