Developing community-driven quality improvement initiatives to enhance chronic disease care in Indigenous communities in Canada: the FORGE AHEAD program protocol.

BACKGROUND: Given the dramatic rise and impact of chronic diseases and gaps in care in Indigenous peoples in Canada, a shift from the dominant episodic and responsive healthcare model most common in First Nations communities to one that places emphasis on proactive prevention and chronic disease management is urgently needed. METHODS: The Transformation of Indigenous Primary Healthcare Delivery (FORGE AHEAD) Program partners with 11 First Nations communities across six provinces in Canada to develop and evaluate community-driven quality improvement (QI) initiatives to enhance chronic disease

Effects of Chronic Central Arginine Vasopressin (AVP) on Maternal Behavior in Chronically Stressed Rat Dams.

Exposure of mothers to chronic stressors during pregnancy or the postpartum period often leads to the development of depression, anxiety, or other related mood disorders. The adverse effects of mood disorders are often mediated through maternal behavior and recent work has identified arginine vasopressin (AVP) as a key neuropeptide hormone in the expression of maternal behavior in both rats and humans. Using an established rodent model that elicits behavioral and physiological responses similar to human mood disorders, this study tested the effectiveness of chronic AVP infusion as a novel trea

Is age and not antiretroviral therapy the strongest risk factor for chronic pain in HIV-infected population?

BACKGROUND: Chronic pain in HIV-infected patients on effective antiretroviral therapy (ART) limits patients' normal functioning both somatically and psychologically. The current state of knowledge on the topic is insufficient, with the underlying causes of this pain unexplained. Therefore we analyzed the frequency and factors associated with chronic pain in HIV-infected patients on ART. METHODS: We conducted a prospective, survey study, including consecutive HIV-infected patients under specialist care at the HIV Outpatient Clinic of the Hospital for Infectious Disease in Warsaw between Februa

Interventions for the unhoused individual with schizophrenia: a civilized plan.

This article provides an overview of individuals with schizophrenia who become unhoused and explores current approaches to managing this severe illness in those who often do not want care or believe they need it. Individuals with schizophrenia and who are unhoused face numerous adverse consequences including premature mortality and increased rates of suicide. There is a dearth of research evidence demonstrating efficacy of the Housing First (HF) model and harm reduction approach in decreasing psychotic symptoms in individuals with schizophrenia. Ensuring medication adherence in individuals wit

Sudden increase in skin temperature predicts venous ulcers: a case study.

Venous ulcers affect approximately 2.5 million adults with chronic venous disease (CVD). Venous ulcers are a significant health problem with a reoccurrence rate as high as 72%. There is a critical need for a prediction/prevention model of venous ulcers that includes objective methods to assess the skin. Among individuals affected by CVD, skin temperature is elevated in the lower extremities. This case study of a patient with CVD highlights the potential predictive usefulness and feasibility of measuring skin temperature with an infrared dermal thermometer as part of the standard of care for ve

Experiences of an internet-based rehabilitation program: An interview study among individuals with chronic whiplash-associated disorders.

INTRODUCTION: Given the rapid development of digital healthcare interventions, incorporating patients' perspectives is essential. However, experiences of internet-based exercise programs for chronic whiplash-associated disorders (WAD) remain unexplored. The aim of this study was to explore and describe experiences of an internet-based rehabilitation program combined with four physiotherapy visits for individuals with chronic WAD from a quality-of-care perspective. METHODS: This study employed a qualitative explorative and descriptive study design. Individual semi-structured interviews were co

Chronic respiratory diseases are predictors of severe outcome in COVID-19 hospitalised patients: a nationwide study.

BACKGROUND: Influenza epidemics were initially considered to be a suitable model for the COVID-19 epidemic, but there is a lack of data concerning patients with chronic respiratory diseases (CRDs), who were supposed to be at risk of severe forms of COVID-19. METHODS: This nationwide retrospective cohort study describes patients with prior lung disease hospitalised for COVID-19 (March-April 2020) or influenza (2018-2019 influenza outbreak). We compared the resulting pulmonary complications, need for intensive care and in-hospital mortality depending on respiratory history and virus. RESULTS:

Recent challenges facing patients with preexisting chronic liver disease in the era of the COVID-19 pandemic.

COVID-19 pandemic has resulted in a growing number of beds in common hospital wards and intensive care units being occupied by COVID-19 patients and the majority of medical and nursing staff being dedicated to their care. The present review summarizes the impact of COVID-19 on patients with underlying chronic liver diseases (CLD). Deferrals of all non-urgent activities in healthcare facilities, including a decrease in liver-clinic visits for patients with CLD, inadequate hepatocellular carcinoma (HCC) surveillance, and postponement of liver transplant activities are the most important conseque

Virtual health coaching to improve self-care among informal caregivers of hospitalized older adults during a transition in care: Randomized controlled trial study protocol.

BACKGROUND: Informal caregivers play a critical role in supporting older adults with multiple chronic conditions (MCCs), particularly during transitions from hospital to home. These transitions are often marked by increased stress and reduced self-care among caregivers and heightened risk for poor outcomes in both caregivers and patients. The Virtual Caregiver Coach for You (ViCCY) intervention, grounded in the Transactional Model of Stress and Coping, aims to improve caregiver self-care through tailored virtual health coaching. METHODS: This single-site, three-hospital, Phase III randomized

[From acute care to long-term management: reflections on models for stroke recurrence prevention].

Stroke is the leading cause of death and disability in China. In recent years, acute stroke care in China has improved substantially with the continued development of stroke centers and rapid advances in emergency reperfusion therapy technology. Nevertheless, the risk of stroke recurrence remains high, and long-term management has become a critical determinant of long-term prognosis of patients. Currently, a large amount of evidence has established the efficacy of secondary prevention strategies, including antithrombotic therapy, risk factor control, and lifestyle modification. The main challe

Comparison of perceived versus actual care complexity in HIV-positive patients receiving antiretroviral treatment: The STRATPATIENT study.

INTRODUCTION: HIV is now considered a chronic disease due to advances in antiretroviral therapy (ART), which have improved survival but have also increased both comorbidities and polypharmacy. This underscores the need for personalized care strategies such as the Capacity-Motivation-Opportunity (CMO) model and patient stratification tools. This study aimed to identify the discrepancy between patient-perceived and actual care complexity in HIV-positive patients on ART, as assessed by a stratification tool. METHODS: A retrospective, observational study at Valme Hospital (April-June 2024) includ

[Diagnosis and Therapeutic Management of Bone Disease in Patients with Chronic Kidney Disease or Kidney Transplant Recipients].

Osteoporosis is a chronic skeletal disease characterized by reduced bone mineral density and deteriorated bone microarchitecture, which increases fracture risk. In patients with chronic kidney disease (CKD), osteoporosis management is complicated by disturbances in mineral and bone metabolism (CKD-MBD) that adversely affect bone health. Diagnosis requires a thorough clinical evaluation, including bone mineral density measurement via DEXA, bone microarchitecture assessment with TBS, and analysis of bone turnover biomarkers. Therapeutic management must be personalized and may include anti-resorp

Angelica sinensis isolate SBD.4: composition, gene expression profiling, mechanism of action and effect on wounds, in rats and humans.

This report characterizes an aqueous isolate (SBD.4) of one of the most broadly used Chinese medicinal herbs, Angelica sinensis, from the perspective of its application in skin and wound care. SBD.4 has been chemically defined and was found to increase the strength of healed wounds in retired breeder (older) rats. Furthermore, the mechanism of action of this Angelica sinensis isolate was tested in the zebrafish angiogenesis model, and in human skin substitutes by DNA microarray, revealing a bioactivity profile consistent with skin repair and regeneration. When combined with several types of wo

Identifying the need for care in rheumatoid arthritis: A Candidacy 2.0 analysis of lived experiences.

This study explores how individuals with rheumatoid arthritis (RA) come to identify themselves as candidates for medical care, using the newly developed Candidacy 2.0 model. Candidacy 2.0 extends the original Candidacy Framework by emphasizing the role of the embodied intersectional relational self in healthcare access, providing a theoretical framework that transcends specific healthcare systems. Through semi-structured interviews with 33 individuals living with RA across six Canadian provinces, we examined how embodied experiences, social identities, and relational contexts shape initial rec

A deep learning framework for recognizing skin changes secondary to chronic venous insufficiency in clinical photographs: a multicentre validation study.

BACKGROUND: Chronic venous insufficiency (CVI) produces heterogeneous lower-extremity skin changes that often mimic inflammatory dermatoses, complicating the differentiation between venous etiologies and conditions requiring dermatologic care. Coexisting venous signs further confound visual interpretation, leading to diagnostic variability. To address this unmet clinical need, we developed and externally validated a pose-guided, high-resolution deep learning pipeline to classify CVI-related skin lesions from routine clinical photographs. MATERIALS AND METHODS: A deep learning framework was de
