[Cutaneous coccidioidomycoses: Six Mexican cases report].

Coccidioidomycosis is a systemic airborne mycosis that may involve secondarily other organs through systemic dissemination. Fungi Coccidioides immitis and C. posadasii are the etiologic agents. The former is ubiquitous from the area of California in North America, and the latter is found elsewhere in the world. Primary cutaneous infection is rare. We present six Mexican male cases, residents of Tijuana B.C. Three of them with primary pulmonary infection and further cutaneous dissemination, and three cases of primary cutaneous coccicioidomycosis. In half the cases C. posadasii was isolated. The…

Coccidioidomycosis: Recent Updates.

Coccidioidomycosis manifests as a variety of clinical manifestations and ranges in severity from asymptomatic exposure with resultant immunity to reinfection, to fulminant, and life-threatening disseminated disease. Primary coccidioidal pneumonia represents the most common clinical form of infection, and the incidence continues to increase. Within the endemic region, primary pulmonary coccidioidomycosis represents up to 29% of all community-acquired pneumonia emphasizing the frequency with which clinicians encounter this endemic mycosis. Chronic infection develops in 3 to 5% of patients, and a

A case of fever, eosinophilia, and pneumonia.

There is a broad differential for patients presenting with fever, eosinophilia, and pneumonia. We present a case of a 48-year-old man who presented with recurrent fever, pleuritic chest pain, and cough. His medical history was significant for a recent trip to Arizona. A chest X ray showed a right lower lobe infiltrate and CT examination of the chest showed extensive mediastinal lymphadenopathy. Tissue culture from a biopsy specimen of the mediastinal lymph nodes revealed growth of Coccidioides immitis and a diagnosis of coccidioidomycosis was made. He was treated with a total of a 9-month cour…

Coccidioidomycosis in India: report of a second imported case.

We describe a fatal case of imported coccidioidomycosis in India in a 22-year-old male who worked in Tucson, Arizona, approximately four years prior to his illness. The diagnosis was based on the presence of characteristic spherules with endospores in biopsy tissue of lymph nodes, bone and pus from a chronic discharging sinus in the left gluteal region and isolation of Coccidioides immitis in culture. C. immitis is one of the most infectious and virulent fungal pathogens and poses a serious occupational hazard for laboratory personnel, especially in areas where the disease is not endemic. To r…

Coccidioidomycosis in Northern California. An outbreak among archeology students near Red Bluff.

An outbreak of coccidioidomycosis occurred among 39 archeology students in the summer of 1972. The students excavated Indian ruins near Red Bluff in Tehama County, California, 20 miles north of the previously recognized northernmost limit of endemicity. At least 17 persons contracted an illness clinically compatible with a diagnosis of coccidioidomycosis. Coccidioidomycosis was documented by skin test conversion as well as by specific serologic reactions. Coccidioides immitis was also isolated from two soil samples taken at the excavation site. In light of its ecological requirements, it is do…

Coccidioidomycosis as a complication of pregnancy.

Records of 33 cases of coccidioidomycosis occurring during pregnancy were reviewed. In this group the incidence of dissemination of the disease was considerably greater than the reported incidence of dissemination in all cases of coccidioidomycosis. The incidence of dissemination was higher in the patients who contracted coccidioidomycosis late in pregnancy than it was in those in whom onset of the disease occurred earlier in gestation; but dissemination occurred in all Negro patients in the group, regardless of the time of onset during pregnancy. The chief complicating effect when onset was i

Use of Epoetin Alfa for Anemia of Chronic Disease in a Jehovah's Witness Teenager With Disseminated Coccidioidomycosis: A Case Report.

Anemia of chronic disease is a common complication of prolonged infection and systemic inflammation and can be difficult to manage in patients who decline blood transfusion, such as Jehovah's Witnesses. Erythropoiesis-stimulating agents are widely used in renal and oncologic settings but are rarely reported in infection-associated anemia among pediatric or young adult patients. We describe a 19-year-old Jehovah's Witness male with disseminated Coccidioides immitis infection complicated by severe anemia of chronic disease. Because blood transfusion was declined, epoetin alfa was administered as…

Endemic Fungi Presenting as Community-Acquired Pneumonia: A Review.

In endemic areas, dimorphic fungal infections due to Histoplasma capsulatum, Blastomyces dermatitidis, and Coccidioides posadasii/immitis account for up to 30% of cases of community-acquired pneumonia. Because respiratory manifestations are often indistinguishable from common bacterial causes of pneumonia, the diagnosis of pulmonary histoplasmosis, blastomycosis, and coccidioidomycosis is often delayed and associated with antibiotics overuse. In addition to being highly endemic to certain regions of North America, dimorphic fungi have global significance due to established areas of endemicity …

Preventing and treating biologic-associated opportunistic infections.

A variety of opportunistic pathogens have been reported to infect patients receiving tumor necrosis factor (TNF) antagonists for the treatment of autoimmune diseases. These pathogens are numerous, and include coccidioides, histoplasma, nontuberculous mycobacteria, Mycobacteria tuberculosis, and others of public health concern. Accordingly, TNF antagonists should be used with caution in patients at risk for tuberculosis, and screening for latent tuberculosis infection should be undertaken before anti-TNF therapy is initiated. Although screening and prevention efforts have decreased the risk of …

Secondary intracerebral blastomycosis with giant yeast forms.

Secondary central nervous system (CNS) blastomycosis is an unusual manifestation of blastomycosis. We report a case of recurrent intracerebral blastomycosis that presented histopathologically with giant yeast-like cells and multinucleation that mimicked Coccidioides immitis. The yeast forms of Blastomyces dermatitidis usually range in size from 8 to 20 microm in diameter. Large or giant yeast forms (20-40 microm) are rare. The four cases previously reported in the literature involving giant yeast cell forms of B. dermatitidis are reviewed here. Intracerebral blastomycosis should be suspected i…

Coccidioidomycosis in workers at an archeologic site--Dinosaur National Monument, Utah, June-July 2001.

Coccidioidomycosis is a fungal infection caused by inhalation of airborne Coccidioides immitis spores that are present in the arid soil of the southwestern United States, California, and parts of Central and South America. Infection with C. immitis previously has not been diagnosed in patients outside these areas, except in travelers returning from areas where the disease is endemic. This report describes an outbreak of coccidioidomycosis in workers at an archeologic site in northeastern Utah during June-July, 2001, and represents the first identification of coccidioidomycosis in northern Utah…

Coccidioidomycosis--Arizona, 1990-1995.

Coccidioidomycosis is a systemic infection caused by the inhalation of airborne arthroconidia from Coccidioides immitis, a soil-dwelling fungus found in the southwestern United States, parts of Mexico, and Central and South America. Clinical manifestations occur in approximately 40% of infected persons and may include mild influenza-like illness; severe pneumonia; and rarely, disseminated disease and death. During 1990-1995, the number of reported cases of coccidioidomycosis in Arizona increased by 144%. To characterize trends in and the impact of coccidioidomycosis in Arizona, the Arizona Dep…

Coccidioidomycosis in a patient with lymphoma. Spherulation in human pleural fluid medium.

In a Caucasian woman who had had a giant follicular lymphoma for 21 months a small lesion developed in the upper lobe of the left lung. Excision, histologic study, and culture of the lesion were performed. Of the many stained sections, only one revealed endospore-containing spherules within the caseous necrosis of a granuloma, and only four spherules were identified. On agar medium the same tissue yielded a fast-growing fungus that produced arthrospores alternating with empty cells, typical of Coccidioides immitis. The arthrospore suspension was inoculated intraperitoneally into a mouse and in…
