The role of coinfections in HIV epidemic trajectory and positive prevention: a systematic review and meta-analysis.

OBJECTIVES: Recurrent or persistent coinfections may increase HIV viral load and, consequently, risk of HIV transmission, thus increasing HIV incidence. We evaluated the association between malaria, herpes simplex virus type 2 (HSV-2) and tuberculosis (TB) coinfections and their treatment on HIV viral load. DESIGN: Systematic review and meta-analysis of the association of malaria, HSV-2 and TB coinfections and their treatment with HIV viral load. METHODS: PubMed and Embase databases were searched to 10 February 2010 for studies in adults that reported HIV plasma and/or genital viral load by …

Incidence and risk factors for steatosis progression in adults coinfected with HIV and hepatitis C virus.

BACKGROUND & AIMS: Hepatic steatosis is a common histologic finding in patients coinfected with human immunodeficiency virus (HIV) and hepatitis C virus (HCV), although little is known about its natural history. We prospectively examined the natural history of steatosis in patients coinfected with HIV and HCV who attended an urban HIV clinic. METHODS: The study cohort consisted of 222 coinfected patients (87% black, 94% with HCV genotype 1 infection) who had at least 2 liver biopsies performed between 1993 and 2008. Biopsy specimens were scored by a single pathologist; samples were classified

HIV-specific health care utilization and mortality among tuberculosis/HIV coinfected persons.

Persons coinfected with tuberculosis (TB) and HIV are at high risk of death, in part due to suboptimal utilization of HIV-specific health care. We sought to better understand HIV-associated health care utilization and mortality in a retrospective cohort of TB/HIV coinfected cases reported in North Carolina 1993-2003. In this cohort, HIV was newly diagnosed during TB presentation for 34.2% of coinfected patients. Patients had advanced HIV (median CD4 104 cells/mm(3)) at TB diagnosis. Of 260 patients previously known to be HIV positive, 32.3% had seen a physician for HIV care in the previous 6 m

Highly active antiretroviral therapy in HIV and hepatitis C coinfected individuals: friend or foe?

PURPOSE OF REVIEW: Highly active antiretroviral therapy dramatically reduces HIV-related morbidity and mortality. Liver disease has subsequently emerged as a major cause of death in HIV/hepatitis C virus-coinfected individuals. Whether HIV therapy also has a favourable impact on the course of liver disease needs to be explored and counterbalanced with toxicity issues related to the extended duration of antiretroviral therapy. RECENT FINDINGS: Recent cohort studies have shown that highly active antiretroviral therapy is associated with a reduced rate of progression of hepatitis C virus liver d

Unilateral syphilitic perioptic neuritis in a patient coinfected with human immunodeficiency virus type 1.

Perioptic neuritis caused by secondary syphilis is a rare ophthalmic manifestation in the HIV-infected host. Early diagnosis and treatment of this condition is required to prevent further visual damage. We report a case of unilateral syphilitic perioptic neuritis in a patient coinfected with HIV-1.

Nevirapine plasma concentrations and concomitant use of rifampin in patients coinfected with HIV-1 and tuberculosis.

OBJECTIVES: In countries with high numbers of HIV/tuberculosis coinfection nevirapine and rifampin are used extensively. However, limited data are available about whether or not nevirapine and rifampin can be safely coadministered without the plasma concentration of nevirapine falling below therapeutic levels. METHODS: Blood samples for determination of nevirapine plasma concentrations were collected from patients using nevirapine 200 mg twice daily with or without concomitant rifampin. Bivariate and multivariate linear regression models were used to investigate factors possibly related to ne…

Immune complex glomerulonephritis in patients coinfected with human immunodeficiency virus and hepatitis C virus.

Human immunodeficiency virus-associated nephropathy (HIVAN), characterized by heavy proteinuria, rapidly progressive renal failure, "collapsing" glomerulopathy, and tubulointerstitial abnormalities, is the most common finding in HIV-infected patients undergoing a renal biopsy and predominantly affects blacks. We describe the clinical features and renal pathologic findings of 12 intravenous drug users (IVDUs) coinfected with HIV and hepatitis C virus (HCV) who were selected for renal biopsy because they presented with features different from typical HIVAN, including hypertension, microscopic he

Impact of Respiratory Coinfections on Outcomes of Pneumocystis jirovecii Pneumonia in Solid Organ Transplant Recipients.

BACKGROUND: Pneumocystis jirovecii pneumonia (PCP) is a significant opportunistic infection in solid organ transplant (SOT) recipients. However, the impact of respiratory coinfections on outcomes in this population remains unclear. METHODS: We retrospectively analyzed 112 SOT recipients diagnosed with PCP from April 2018 to March 2024. Respiratory coinfections were defined as identification of significant pathogens in respiratory specimens within 3 days of PCP diagnosis. RESULTS: Among 112 SOT recipients with PCP, 50.0% required intensive care unit (ICU) admission, and the 30-day and 90-day …

Assessment of the role of Haemophilus ducreyi coinfection on outcomes of Yaws treatment in the southwestern part of Ghana.

BACKGROUND: Yaws, caused by Treponema pallidum subsp. pertenue (TP) is targeted for eradication by 2030 under the World Health Organization's (WHO) initiative, which relies on mass drug administration (MDA) of azithromycin. Evidence of persistent lesions after treatment has been reported. While the occurrences of Haemophilus ducreyi (HD) in yaws-like lesions has been widely documented, there is limited evidence regarding its influence on treatment outcomes assessed at the WHO-recommended 4-week follow-up. In this study, we sought to detect the occurrence of H. ducreyi in yaws-confirmed cases a

Community-acquired coinfections in adults with severe imported malaria: a systematic review and meta-analysis.

BACKGROUND: Severe imported malaria poses diagnostic challenges due to its overlap with bacterial sepsis, often prompting empirical antibiotic use. However, the burden of community-acquired coinfections in adults for non-endemic settings remains uncertain, leading to inconsistent recommendations. DATA SOURCES: MEDLINE, Embase, LILACS, and the Cochrane Central Register of Controlled Trials were searched from inception to March 2025, without language or date restrictions, with additional screening of reference lists. STUDY ELIGIBILITY CRITERIA: We included experimental and observational studie…

Fungal Pulmonary Coinfections in COVID-19: Microbiological Assessment, Inflammatory Profiles, and Clinical Outcomes.

Background and Objectives: Secondary pulmonary fungal infections in coronavirus disease 2019 (COVID-19) remain underexplored despite emerging reports linking them to heightened morbidity. Comorbidities, steroid use, and prolonged hospital stays can predispose patients to opportunistic fungi. This study aimed to evaluate the impact of fungal coinfection on inflammatory markers, disease severity, antifungal resistance profiles, and outcomes in hospitalized COVID-19 patients. Methods: This retrospective observational study enrolled 280 adults (≥18 years) with real-time polymerase chain reaction (…

A Double Threat in an Immunocompromised Patient: Coinfection With Pneumocystis jirovecii Pneumonia and Cytomegalovirus in Stomach Cancer.

Pneumocystis jirovecii pneumonia (PCP) and cytomegalovirus (CMV) present significant threats as opportunistic infections among immunocompromised patients. The combination of these two infections can lead to dire clinical scenarios, particularly in those with weakened immune systems. We document a case of a patient battling advanced stomach cancer who was admitted with alarming symptoms, including persistent fever and severe respiratory distress. Having undergone multiple chemotherapy and immunotherapy cycles, patient's condition had deteriorated significantly. Initial management involved high-

Occult hepatitis B: prevalence and clinical significance. Role in liver pathology and in viral coinfections.

The review examines issues related to occult hepatitis B virus infection (OBI), which occurs at a late stage of chronic hepatitis B (CHB) after HBsAg clearance. In clinical practice, OBI is detected by the absence of HBsAg and the presence of antibodies to HBcAg in the blood serum and is often referred to as «past» or «resolved» hepatitis B. However, hepatitis B virus (HBV) DNA remains in liver cells, is poorly detected by routine diagnostic methods, and cannot be removed by existing therapies. Data on the prevalence of OBI vary, but it is found in all regions of the world, much more often in

Cohabitating in the City: A Case of Hemolytic Anemia in a Patient Coinfected With Babesiosis, Lyme Disease, and Mononucleosis.

In the past decades, the number of tick-borne illnesses reported in the United States has increased exponentially, leading to significant human morbidity and mortality. Among tick-borne illnesses, the most frequent in the United States is Lyme, a disease caused by the spirochetal bacteria Borrelia burgdorferi. Coinfection with Lyme disease and Babesia spp. is common among ticks, rodent reservoirs, and human hosts. At an early stage, both Lyme disease and babesiosis present with flu-like symptoms, such as fever, myalgias, fatigue, headache, and malaise. Due to the nonspecific presentation of th…

Persistent T cell unresponsiveness associated with chronic visceral leishmaniasis in HIV-coinfected patients.

A large proportion of HIV-coinfected visceral leishmaniasis (VL-HIV) patients exhibit chronic disease with frequent VL recurrence. However, knowledge on immunological determinants underlying the disease course is scarce. We longitudinally profiled the circulatory cellular immunity of an Ethiopian HIV cohort that included VL developers. We show that chronic VL-HIV patients exhibit high and persistent levels of TIGIT and PD-1 on CD8+/CD8- T cells, in addition to a lower frequency of IFN-γ+ TIGIT- CD8+/CD8- T cells, suggestive of impaired T cell functionality. At single T cell transcriptome and c
