Statewide collaborative partnerships among American Indian and Alaska Native (AI/AN) communities in California to target the opioid epidemic: Preliminary results of the Tribal Medication Assisted Treatment (MAT) key informant needs assessment.

American Indian and Alaska Native (AI/AN) communities have disproportionately been impacted by the opioid epidemic with the second highest opioid-related overdose death rates compared to other ethnic groups. The diversity among California AI/AN tribes, including regional differences in economic opportunities, tribal affiliation and organization, resources and infrastructure, requires a strong community-based partnership approach to assess global statewide patterns in service availability, acceptability, and utilization, as well as capturing the unique challenges and service needs within each r

Going home with baby: innovative and comprehensive support for new mothers.

Shorter length of stay for postpartum mothers and their newborns necessitates careful community follow-up after hospital discharge. The vast amount of information given during the initial postpartum period can be overwhelming. New parents often need considerable support to understand the nuances of newborn care including newborn feeding. Primary health care and community services need to ensure there is a seamless continuum of care to support, empower, and educate new mothers and their families to prevent unnecessary hospital readmission and other negative health outcomes. The Healthy & Home p

A mixed-methods approach to understanding water use and water infrastructure in a schistosomiasis-endemic community: case study of Asamama, Ghana.

BACKGROUND: Surface water contaminated with human waste may transmit urogenital schistosomiasis (UGS). Water-related activities that allow skin exposure place people at risk, but public health practitioners know little about why some communities with access to improved water infrastructure have substantial surface water contact with infectious water bodies. Community-based mixed-methods research can provide critical information about water use and water infrastructure improvements. METHODS: Our mixed-methods study assessed the context of water use in a rural community endemic for schistosomia

Screening for fetal alcohol spectrum disorders by nonmedical community workers.

BACKGROUND: South Africa has the highest prevalence of Fetal Alcohol Spectrum Disorders (FASD) in the world yet many women have no access to clinic care or to physicians in their communities. The shortage of physicians trained in the diagnosis of FASD is even more severe. Thus there is a need to train community workers to assist in the delivery of health care. OBJECTIVES: This study reports on the effectiveness of training community workers to screen for a possible diagnosis of a FASD. METHODS: Community workers in Cape Town, South Africa were trained to screen for FASD in 139, 18-month-old

Active shooters: The underutilisation of emergency operations centres - the need for a checklist to guide response and recovery.

The emergency operations centre (EOC) is a critical emergency response and recovery component that provides information management and resource allocation. EOCs are often used during all hazards; however, after reviewing over 25 after-action reports for active shooter incidents, they are frequently underutilised. Not activating or delaying activation can slow recovery efforts and lead to chaos for the first responders and the public due to a lack of situational awareness. Historical active shooter incidents, such as the San Bernardino attack, Uvalde school shooting and Aurora theatre shooting,

In Situ Diagnosis of Scabies Using a Handheld Digital Microscope in Resource-Poor Settings-A Proof-of-Principle Study in the Amazon Lowland of Colombia.

Scabies is a neglected tropical disease associated with important morbidity. The disease occurs worldwide and is particularly common in resource-poor communities in the Global South. A validated technique for the diagnosis of scabies in resource-poor settings does not exist. The objective of the study was to determine the practicability and accuracy of handheld digital microscopy in three indigenous communities in the Amazon lowland of Colombia, where scabies is the most common parasitic skin disease. One-hundred-and-eleven children and adults from three indigenous communities with a presumpti

A Multiphase Implementation of Ring Treatment: A Community-Engaged Control Program for T. Solium taeniasis/cysticercosis in Northern Peru.

Background Cysticercosis (CC) is a zoonotic disease transmitted between pigs and humans, caused by the larval stage of T. solium that leads to neurological morbidity in humans and economic losses in pig-farming communities in low-resource rural regions. Ring Treatment (RT) is an evidence-based intervention that integrates a community participatory approach to provide antiparasitic treatment to households within 100-meters of an infected pig. This study aimed to evaluate the multiphase implementation of RT as a public health program in northern Peru, with a focus on feasibility, adoption, and s

Community-Emergent Advance Care Planning Programming for LGBTQ+ Older Adults.

The percent of the American population that self-identifies as LGBTQ+ is currently doubling generation after generation. Advance Care Planning and Health Care Proxy completion have unique challenges in this growing, aging queer community due to nontraditional family structures, as well as risk factors from historical and current discrimination. LGBTQ+ older adults benefit from a systematized and provider-led approach to assistance with making difficult decisions and mitigating their vulnerability in health settings. This article highlights the implementation of a novel, community-emergent, and

Giant mucinous cystadenoma: a case report.

INTRODUCTION: Giant ovarian cysts are rarely described in the literature, owing to the availability of advanced imaging technologies in developed countries leading to early treatment. In resource-limited settings, various factors lead to late presentation. CASE PRESENTATION: We present a case of a 48-year-old black African woman with a giant mucinous cystadenoma who presented to a tertiary hospital with massive abdominal distention 5 years after being referred from a district hospital for the same problem. Surgical management resulted in fatal complications. CONCLUSIONS: The surgical managem

Communities organizing to promote equity: engaging local communities in public health responses to health inequities exacerbated by COVID-19-protocol paper.

BACKGROUND: The COVID-19 pandemic has disproportionately impacted rural and under-resourced urban communities in Kansas. The state's response to COVID-19 has relied on a highly decentralized and underfunded public health system, with 100 local health departments in the state, few of which had prior experience engaging local community coalitions in a coordinated response to a public health crisis. METHODS: To improve the capacity for local community-driven responses to COVID-19 and other public health needs, the University of Kansas Medical Center, in partnership with the Kansas Department of

Principles, practices and knowledge of clinicians when assessing febrile children: a qualitative study in Kenya.

BACKGROUND: Clinicians in low resource settings in malaria endemic regions face many challenges in diagnosing and treating febrile illnesses in children. Given the change in WHO guidelines in 2010 that recommend malaria testing prior to treatment, clinicians are now required to expand the differential when malaria testing is negative. Prior studies have indicated that resource availability, need for additional training in differentiating non-malarial illnesses, and lack of understanding within the community of when to seek care play a role in effective diagnosis and treatment. The objective of

Communal mastery and associations with depressive and PTSD symptomatology among urban trauma-exposed women.

Objective: Racial and ethnic minority women from low-resource urban communities experience disproportionately high rates of trauma exposure. Higher rates of lifetime trauma exposure are strongly associated with subsequent psychological sequela, specifically depression and posttraumatic stress disorder (PTSD). Communal mastery is the ability to cope with challenges and achieve goals by being closely interconnected with friends, family, and significant others. Yet, it is unknown if communal mastery is protective specifically against PTSD and depressive symptoms. Method: Participants (N = 131) we
