One-year structural and functional outcomes of accelerated epithelium-off corneal cross-linking in keratoconus.

AIM: To evaluate the structural and functional outcomes of accelerated epithelium-off corneal collagen cross-linking (CXL) in patients with keratoconus (KC) 1y after treatment. METHODS: This retrospective study included patients who underwent an accelerated epi-off CXL for progressive KC at the Department of Ophthalmology, Sveti Duh University Hospital, Zagreb, Croatia, between January and December 2023. The protocol consisted of epithelial removal, riboflavin 0.1% instillation for 3min, and ultraviolet-A (UVA) irradiation at 9 mW/cm2 for 10min. Pentacam HR was used to assess keratometric par

Transepithelial accelerated versus conventional corneal collagen crosslinking in patients with keratoconus: a comparative study.

PURPOSE: To systematically compare the efficacy of transepithelial accelerated corneal collagen crosslinking (TE-ACXL) with conventional corneal collagen crosslinking (C-CXL) in patients with progressive keratoconus. METHODS: Eyes of patients with progressive keratoconus who were treated with C-CXL (3 mW/cm2 for 30 minutes) were compared with those who underwent TE-ACXL (6 mW/cm2 for 15 minutes). Best-corrected visual acuity (BCVA), keratometry values, corneal thickness, and topometric indexes were compared before CXL, and at 2 months, 6 months, and 12 months postoperatively. RESULTS: The st

PURPOSE: To compare clinical outcomes of conventional corneal cross-linking (C-CXL) and diluted alcohol and iontophoresis-assisted corneal cross-linking (DAI-CXL) for the treatment of progressive keratoconus (KC). METHODS: Ninety-three eyes of 80 patients with KC were treated by C-CXL (n = 47) or DAI-CXL (n = 46). Visual acuity, keratometry, KC indexes, pachymetry, and aberrations were recorded before treatment and 1, 3, 6, and 12 months after treatment. The demarcation line was assessed 1 month after treatment. RESULTS: A significant improvement in visual acuity was observed at month 3 and

A mouse model of corneal endothelial decompensation using cryoinjury.

PURPOSE: To develop a mouse model of bullous keratoplasty and evaluate the safety and efficacy of cryoinjury-induced corneal endothelial decompensation. METHODS: Transcorneal freezing was performed on the right eye of each mouse. One cycle of cryoinjury was performed in 18 eyes (group A), and three cycles were performed in 17 eyes (group B). Pachymetry and intraocular pressure (IOP) measurements were done preoperatively, as well as at 1, 3, 7, 14, and 21 days after cryoinjury. At each post-cryoinjury time point, three mice from each group were euthanized, and the corneas underwent histology a

Improved field scanner incorporating parabolic optics. Part 2: Experimental verification and potential for volume scanning.

PURPOSE: To report the visual and topographic outcomes of two pulsed-light-accelerated CXL (A-CXL) protocols at a 12-month follow-up and their correlation with the corneal stromal demarcation line (DL) depth. PATIENTS AND METHODS: Retrospective comparative cohort of patients with documented progressive keratoconus were included. Two epi-off pulsed-light [1s on-1s off] A-CXL protocols were compared: irradiance 30*8 and 45*5:20 (fluence 7.2 J/cm2). Five-year clinical outcomes of 107 consecutive DMEK surgeries.

PURPOSE: The long-term clinical outcomes, postoperative complications, and graft survival of Descemet-membrane endothelial keratoplasty (DMEK) remain poorly understood. We retrospectively assessed these variables in all consecutive eyes that underwent DMEK for any indication in 2014-2018. The findings were compared to the long-term DMEK studies of five other groups (3-10-year follow-up). METHODS: Patients underwent ophthalmological tests preoperatively, at 1, 3, 6, and 12 postoperative months, and then annually. Five-year graft survival was determined by Kaplan-Meier estimator. Repeatability and Reproducibility of Slit Lamp, Optical Coherence Tomography, and Scheimpflug Measurements of Corneal Scars.

Purpose: To determine the repeatability and reproducibility of anterior segment optical coherence tomography (AS-OCT) and Scheimpflug photography for several measurements of corneal scars, including scar size, scar depth, and corneal thickness. Methods: A series of patients treated for fungal keratitis at a tertiary eye care center in South India were recalled two years after successful treatment. Eyes with corneal scars had a slit lamp examination performed by two ophthalmologists masked to the other's examination. For AS-OCT and Scheimpflug photography, each eye had two scans taken by one te

Evaluation of keratoconus progression.

AIM: To define variables for the evaluation of keratoconus progression and to determine cut-off values. METHODS: In this retrospective cohort study (2010-2016), 265 eyes of 165 patients diagnosed with keratoconus underwent two Scheimpflug measurements (Pentacam) that took place 1 year apart ±3 months. Variables used for keratoconus detection were evaluated for progression and a correlation analysis was performed. By logistic regression analysis, a keratoconus progression index (KPI) was defined. Receiver-operating characteristic curve (ROC) analysis was performed and Youden Index calculated t

[Keratoconus: correlations between clinical aspects and Orbscan evaluations].

PURPOSE: To look for potential correlations between clinical and topographical aspects of keratoconus evaluated by Orbscan (Chauvin-Bausch & Lomb). METHODS: We analyzed 228 keratoconic eyes in a retrospective study over a period of 16 months. The best corrected visual acuity (BCVA) and the corneal opacities were correlated with 26 topographical parameters evaluated by Orbscan. RESULTS: The maximal keratometry is the parameter that correlated best with clinical aspects. The other well-correlated keratometric parameters were the SimKmax and the Mean Power in the 3-mm zone. The distance between

Midstromal isolated Bowman layer graft for reduction of advanced keratoconus: a technique to postpone penetrating or deep anterior lamellar keratoplasty.

Midstromal implant of an isolated Bowman layer graft is a new approach to reduce ectasia in eyes with advanced keratoconus. The procedure should postpone penetrating or deep anterior lamellar keratoplasty. Ten eyes of 9 patients with progressive, advanced keratoconus and contact lens intolerance underwent the procedure with no intraoperative adverse events. Throughout the study period, we observed no complications related to stromal dissection and/or the Bowman layer graft. Maximum corneal power decreased from a mean (SD) of 74.5 (7.1) diopters (D) before to 68.3 (5.6) D after surgery (P = .00
