ED echo of reverse Tako-tsubo cardiomyopathy: a rare and misleading finding.

Tako-tsubo transient cardiomyopathy (TCM) is a syndrome of specific myocardial wall motion abnormalities that appear in response to extreme physiological or emotional stress. The patients have normal coronary arteries and usually present as new-onset heart failure or acute coronary syndrome. We report on a 48-year-old woman with post-community cardiac arrest presenting as an acute myocardial infarction. An emergency physician–performed echocardiogram showed significant wall motion abnormalities, suggesting myocardial infarction. Cardiac catheterization revealed patent epicoronary circulation, …

[A diagnosis of venous anomaly with dextrocardia by magnetic resonance imaging and transesophageal echocardiography].

A 56-year-old woman was admitted with the chief complaint of dyspnea. Chest radiograph and echocardiography disclosed mirror image dextrocardia with massive pleural effusion. Magnetic resonance imaging (MRI) and transesophageal echocardiography (TEE) revealed dilated coronary sinus and persistent right-sided superior vena cava which entered the coronary sinus. These findings were compatible with those of digital subtraction angiography. Anomalies of the venous system must be recognized before surgery because adequate venous drainage is essential for extra-corporeal circulation. MRI and TEE are

Benefits of exercise training on coronary blood flow in coronary artery disease patients.

Every 34 seconds an American experiences a myocardial infarction or cardiac death. Approximately 80% of these coronary artery disease (CAD)-related deaths are attributable to modifiable behaviors, such as a lack of physical exercise training (ET). Regular ET decreases CAD morbidity and mortality through systemic and cardiac-specific adaptations. ET increases myocardial oxygen demand acting as a stimulus to increase coronary blood flow and thus myocardial oxygen supply, which reduces myocardial infarction and angina. ET augments coronary blood flow through direct actions on the vasculature that

Primary congenital anomalies of the coronary arteries: a coronary arteriographic study in Western Turkey.

Coronary artery anomalies are found in 0.6% to 1.5% of coronary angiograms. Angiographic recognition of these vessels is important because of their clinical significance and importance in patients undergoing coronary angioplasty or cardiac surgery. We reviewed the database of the Cardiac Catheterization Laboratory of Uludag Medical University in Bursa, Turkey. All patients who were subjected to coronary angiography from 1994 to 2001 were included. The study included 12,059 patients who underwent diagnostic coronary arteriography during the 8 year period. One hundred patients had primary congen

[Treatment of unstable coronary disease].

The clinical presentations of ischemic heart disease comprises the term of acute ischaemia syndromes, that include unstable angina pectoris, non-Q-wave myocardial infarction, Q-wave-myocardial infarction and sudden death. Among the different presentations of acute ischemic syndromes, the unstable angina and non-Q-wave myocardial infarction can be regarded together. In both pathologic entity, the plaque rupture, or erosion signifies the primary event, which is the source of the highly thrombogenic substances coming out from the core of the atherosclerotic plaque and entering to the coronary and

Anomalous origin of a posterior descending artery from the right pulmonary artery: report of a rare case diagnosed by multidetector computed tomography angiography.

The case of a mildly symptomatic 43-year-old man with evidence of reversible inferior wall ischemia by radioisotope stress testing is described. Invasive catheterization showed abnormal connections between the coronary arteries and the pulmonary circulation. Multidetector computed tomography elucidated the previously undescribed finding of a posterior descending artery originating from the right pulmonary artery.

Therapeutic targets for atherosclerotic coronary artery disease: druggable genome-wide Mendelian randomization analysis.

BACKGROUND AND AIMS: Despite advances in treating atherosclerotic coronary artery disease (ACAD), the risk of major adverse cardiovascular events remains high. We aimed to identify potential therapeutic targets for ACAD by integrating genome-wide association studies (GWAS) and Mendelian randomization (MR) analyses. METHODS: Leveraging druggable genome data, cis-eQTL/cis-pQTL from blood and coronary artery tissue, alongside GWAS summary data for coronary atherosclerosis, we performed drug-target MR to identify causal genes for ACAD at both transcript and protein levels. Phenome-wide associatio

Anomalous aortic origin of a coronary artery (AAOCA): A case report.

Anomalous Aortic Origin of a Coronary Artery (AAOCA) is a rare anomaly of the coronary artery with a considerable risk of sudden cardiac death due to ischaemia of the heart. Symptoms may include chest pain on exertion, breathlessness or dizziness. We encountered a case of a 46- year-old female who complained of exertional chest pain with a positive-stress test and subsequently diagnosed with AAOCA through CT angiography (CTA). She successfully underwent a coronary artery bypass graft (CABG) surgery using a saphenous vein graft with uneventful recovery. Right internal mammary artery (RIMA) was

[Subepicardial hematoma (haemorrhagia per rhexin) after elective PTCA with consecutive compression of the distal RIVA].

A transmural coronary dissection occurred immediately after elective angioplasty of a 90% stenosis of the proximal left anterior descending coronary artery (LAD) within the dilated segment and was followed by subepicardial bleeding (haemorrhagia per rhexin), although no perforation by the guide-wire or the balloon catheter took place. A subepicardial blood bolster led to rapid extravasal compression of the LAD distal from the PTCA site resulting in acute ischemia. None of the conventional rescue-interventions such as stents or autoperfusion catheters were likely to resolve this critical situat

Coronary arterial angiographic findings during transient myocardial ischemia.

Coronary arteriography remains the most precise clinical tool for diagnosis and therapeutical decision concerning patients with coronary artery disease; when performed during a short transient episode of myocardial ischemia, this investigation can afford interesting information. The first part of the paper describes the coronary arterial angiographic findings during ischemia related to increase in myocardial requirements. Thus, a vasoconstrictor reflex can decrease the area of narrowing during handgrip exercise. Other reports have described coronary arterial spasm during exercise in highly sel

Causal effects of the gut microbiota, circulating metabolites, and cardiometabolic diseases: A Mendelian randomization study.

The gut microbiota is associated with cardiometabolic disorders (including coronary artery disease, type 2 diabetes, stroke, heart failure, and hypertension) through the gut-heart or gut-brain axis, among which metabolic processes play crucial roles. However, the exact causal mechanisms remain unknown. Our study sought to uncover the causal relationships between the gut microbiota, circulating metabolites and cardiometabolic diseases via Mendelian randomization (MR) analysis. Forward MR was utilized to investigate the causal effects of the gut microbiota and circulating metabolites on the risk

Study protocol for the epigenetic characterization of angor pectoris according to the affected coronary compartment: Global and comprehensive assessment of the relationship between invasive coronary physiology and microRNAs.

BACKGROUND: MicroRNAs (miRNAs) are noncoding RNAs involved in post-transcriptional genetic regulation with a proposed role in intercellular communication. miRNAs are considered promising biomarkers in ischemic heart disease. Invasive physiological evaluation allows a precise assessment of each affected coronary compartment. Although some studies have associated the expression of circulating miRNAs with invasive physiological indexes, their global relationship with coronary compartments has not been assessed. Here, we will evaluate circulating miRNAs profiles according to the coronary pattern o

High plasma levels of macrophage migration inhibitory factor are associated with adverse long-term outcome in patients with stable coronary artery disease and impaired glucose tolerance or type 2 diabetes mellitus.

OBJECTIVES: MIF is proatherogenic and is highly expressed in unstable atherosclerotic plaques. Circulating levels of MIF are increased in patients with impaired glucose tolerance or type 2 diabetes mellitus (IGT/T2DM). We examined whether high circulating levels of macrophage migration inhibitory factor (MIF) are related to increased risk of future coronary events in patients with coronary artery disease (CAD) and IGT/T2DM. METHODS: Plasma MIF levels after overnight fast were measured by ELISA in 617 patients with stable CAD including 79 patients with IGT and 215 patients with T2DM. All patie

[Right coronary artery issuing from the pulmonary artery: review of the literature apropos of a case].

A new case of right coronary artery abnormally issued from the pulmonary trunk is reported. The patient was an asymptomatic young man in whom a continuous cardiac murmur was discovered by chance and an angiographic examination disclosed the coronary abnormality. A review of the literature yielded 37 cases of this anomalous origin which was either isolated, as in the case reported here, or associated with another congenital or acquired cardiopathy. The diagnosis may be suspected on the presence of a continuous murmur in an usually asymptomatic subject and is confirmed by angiography. The abnorm

Association of Circulating Monocyte Chemoattractant Protein-1 Levels With Cardiovascular Mortality: A Meta-analysis of Population-Based Studies.

IMPORTANCE: Human genetics and studies in experimental models support a key role of monocyte-chemoattractant protein-1 (MCP-1) in atherosclerosis. Yet, the associations of circulating MCP-1 levels with risk of coronary heart disease and cardiovascular death in the general population remain largely unexplored. OBJECTIVE: To explore whether circulating levels of MCP-1 are associated with risk of incident coronary heart disease, myocardial infarction, and cardiovascular mortality in the general population. DATA SOURCES AND SELECTION: Population-based cohort studies, identified through a systema

Cardiovascular evaluation before circulatory arrest for removal of vena caval extension of renal carcinoma.

The use of cardiopulmonary bypass, deep hypothermia and circulatory arrest has decreased the risks of hemorrhage, tumor embolization, incomplete thrombus resection, and warm hepatic and renal ischemia associated with resection of renal cell carcinoma extending into the inferior vena cava above the hepatic veins. Patients about to undergo this operation frequently have significant coronary artery and carotid artery disease, and are at risk for perioperative myocardial infarction and stroke. Preoperative evaluation of the coronary artery and carotid artery circulation by coronary angiography, du
