Acceptability, Feasibility, and Effectiveness of Concurrent High-Definition Transcranial Direct Current Stimulation and Cue Exposure in Cannabis Use Disorder: A Pilot Double-Blind Randomized Controlled Trial.

OBJECTIVES: We studied the acceptability, feasibility, tolerability, and preliminary effectiveness of combined cue exposure and anodal high-definition transcranial direct current stimulation (HD-tDCS) on the right dorsolateral prefrontal cortex (DLPFC) on cannabis craving in cannabis use disorder (CUD). METHODS: We randomly assigned 48 participants equally in 4 groups: A, tDCS and cannabis cue exposure; B, tDCS and neutral image; C, sham tDCS and cue exposure; and D, sham tDCS and neutral image. The images were validated by Delphi consensus. All participants received 6 sessions. We explored t…

Neural response to betel quid cues in chewers: a functional magnetic resonance imaging study.

The World Health Organization regards betel quid (BQ) as a human carcinogen. The current study analyzes whether the BQ cues can elicit activity in the chewers' craving-related brain areas. We adopted a cue-reactivity paradigm to examine the changes in the brain activities. The urge intensity was also included to examine whether it can moderate the brain areas stimulated by BQ cues. Sixteen male BQ chewers and 16 healthy male controls were recruited and analyzed. Four types of cues were adopted: BQ cues, matched food cues, visual control cues, and resting crosshair cued. The most direct and imp…

Questioning the validity of food addiction: a critical review.

Food cravings (FC) are closely associated with behaviors such as loss of control, binge eating, and emotional eating. Although FC is among the symptoms proposed for food addiction (FA), we argue that the distress associated with eating, managing cravings, and experiencing loss of control may not, in itself, constitute a framework consistent with addiction or addiction-like eating. Grouping these concepts under the FA label may contribute to conceptual confusion, potentially leading to diagnostic inaccuracies. This integrative review aimed to explore the concepts of FA and FC, as well as their …

Shared and reinforcer-specific alterations in the CRH and noradrenergic systems following short- and long-term withdrawal from cocaine, heroin, and sucrose self-administration.

Stress is known to play a critical role in relapse to drug use as well as in food craving. Craving itself is a key determinant of relapse, and cue-induced drug craving has been shown to increase, or 'incubate', over time for certain drugs such as cocaine and nicotine, though this effect is less consistent for others such as opiates. However, the modulations of stress-related biochemical systems after early or protracted withdrawal that could contribute to this incubation phenomenon have not yet been systematically examined in animal models, nor has the specificity of these mechanisms been test…

White matter neural substrates in alcohol dependence with genetic risk and their role in pathological reward process.

Studies have revealed significant evidence of white matter (WM) microstructural and network connectome abnormalities in alcohol use disorder (AUD). However, the neuroimaging characteristics of alcohol dependence (AD) patients with a family history of AUD and the role of these changes in pathological craving remain unclear. Investigating the heritability of AUD is crucial for identifying genetic predispositions and informing targeted prevention strategies. The study recruited 51 patients for AD, 21 patients with a family history positive (FHP), 30 patients with a family history negative (FHN), …

A Systematic Review of the Effect of Short Term Transcranial Direct Current Stimulation Therapy in Methamphetamine Use Disorder.

Background: Methamphetamine use disorder (MUD) has become more and more common. Some studies have shown that Transcranial Direct Current stimulation (tDCS) may reduce craving when stimulating the dorsal lateral prefrontal cortex. Objectives: The aim of this systematic review was to evaluate the effect of transcranial direct current stimulation (tDCS) on MUD. Databases were searched through May 2022. Randomized Controlled Trials (RCT) and pre-post studies investigating the efficacy of tDCS in MUD were included. The Cochrane Manual of Systematic Evaluation 6.3 bias risk assessment tool was used …

Long-term Changes in the Central Amygdala Proteome in Rats with a History of Chronic Cocaine Self-administration.

The central nucleus of the amygdala (CeA) is a striatum-like structure that contains mainly inhibitory circuits controlling a repertoire of (mal)adaptive behaviors related to pain, anxiety, motivation, and addiction. Neural activity in the CeA is also necessary for the expression of persistent and robust drug seeking, also termed 'incubation of drug craving.' However, neuroadaptations within this brain region supporting incubated drug craving have not been characterized. Here, we conducted a comprehensive analysis of protein expression in the CeA of male rats after prolonged (45-day) abstinenc…

Atomoxetine Efficacy in Methamphetamine Dependence during Methadone Maintenance Therapy.

BACKGROUND: Co-occurring methamphetamine (METH) use during methadone maintenance therapy (MMT) is a highly prevalent and progressive problem in Iran. There are no registered pharmacological treatments for treating METH use disorder. The present study investigates the potential efficacy of atomoxetine in the treatment of these patients. METHODS: In a double-blind, controlled clinical trial, 86 METH-dependents on MMT randomly received either atomoxetine (40 mg/d) or placebo. We measured the craving scores with visual analog scale (VAS) on a weekly basis, and evaluated depression, anxiety and st…

One-way or two-way sweet link between theobromine and depression?

Theobromine is an abundant methylxanthine in cocoa/chocolate. A recent article in BMC Psychiatry concludes that theobromine consumption incr … nefit from the intake of products containing theobromine. Since some antidepressant drugs alter the craving for sweet products, it would be interesting to try to correlate the data on theobromine intake with the kind of therapy used in depressed individuals.

Naltrexone in patients with bipolar disorder and alcohol dependence.

Bipolar disorder is associated with very high rates of substance abuse. However, few clinical trials are reported in this population. Naltrexone is effective for alcohol dependence, but its safety and efficacy are not established in patients with bipolar disorder and alcohol dependence. A 16-week, open-label, add-on pilot study of naltrexone was conducted in 34 outpatients with bipolar disorder and alcohol dependence. Assessments included the 17-item Hamilton Rating Scale for Depression (HRSD-17), Young Mania Rating Scale (YMRS), Brief Psychiatric Rating Scale (BPRS), and an alcohol craving sc…

Black Pepper (Piper nigrum) for Tobacco Withdrawal: A Case Report.

Tobacco use continues to be one of humanity's most significant public health concerns, causing more than 8-million deaths annually. Existing treatments for tobacco use disorder are limited in efficacy and there is a strong need for identifying effective novel treatments. Small clinical trials indicate that black pepper (Piper nigrum) essential oil may be helpful for treating nicotine withdrawal and craving. However, we are unaware of any cases reporting the use of black pepper for these purposes in nonresearch settings. Here we present the case of a patient who inhaled combusted black pepper t…
