OBJECTIVE: To identify factors for development of dehydration in under five year olds with acute watery diarrhoea.
DESIGN: Hospital based unmatched case-control study.
SETTING: Diarrhoea Treatment Unit, Government Medical College Hospital, Nagpur, India.
PARTICIPANTS: The study included 387 cases of diarrhoea having severe or moderate dehydration and 387 controls suffering from diarrhea with mild or no dehydration.
RISK FACTORS: The study included infancy, female sex, religion, residing in urban slums or rural area, under nutrition, cessation of breast feeding during diarrhoeal episode, fl

Food allergy (FA) is a very important problem affecting numbers of infants and children with protean manifestations which are frequent challenges to the pediatrician and other specialists working with children. Adverse reactions to food are very complex, frequently mediated bu IgE mechanisms, and often by other mechanisms. To make the correct diagnosis and to arrive at a proper therapeutic approach requires all the skill a physician can gather. Only an extensive knowledge of the various mechanisms and pharmacologic agents that can be used to prevent or treat these adverse reactions will allow

Eighty-five stools collected from 50 children with diarrhea at an evacuation site on the Thai-Kampuchean border were (1) examined microscopically for fecal leukocytes, (2) tested after 24 hr enrichment in brain/heart infusion broth by a latex slide agglutination test for detection of Salmonella and Shigella, and (3) examined with microbiological techniques to identify bacterial, viral, and parasitic pathogens. If the 65 specimens in which one or no pathogens are considered, 6 or more fecal leukocytes/hpf were found on microscopic examination of stools in both children infected with Shigella sp

PURPOSE OF REVIEW: Enterotoxigenic Escherichia coli (ETEC) remain major causes of infantile diarrhea in the developing world and of travelers' diarrhea in visitors to these areas from industrialized countries, although the relative importance of these pathogens in these settings may be decreasing. The pathogenesis of ETEC infections has been well understood for almost two decades, and there is good evidence for acquired immunity after infection. Nevertheless, currently there is no effective ETEC vaccine. This review evaluates the current status of ETEC vaccine development.
RECENT FINDINGS: ET

TROP1 (EpCAM) and TROP2 are homologous cell surface proteins that are widely expressed, and often co-expressed, in developing and adult epithelia. Various functions have been ascribed to EpCAM and TROP2, but responsible mechanisms are incompletely characterized and functional equivalence has not been examined. Adult intestinal epithelial cells (IEC) express high levels of EpCAM, while TROP2 is not expressed. EpCAM deficiency causes congenital tufting enteropathy (CTE) in humans and a corresponding lethal condition in mice. We expressed TROP2 and EpCAM in the IEC of EpCAM-deficient mice utilizi

In order to assess the epidemiological and clinical characteristics and changing nutritional status of infants suffering from acute diarrhoea, 103 infants with such diarrhoea and the same number of age-matched controls were investigated at the Universidade Federal do Rio Grande do Norte's Paediatric Hospital, in north-eastern Brazil. Each child with diarrhoea was given oral rehydration or, in the severe cases, intravenous rehydration. Each subject was checked for enteropathogens and his or her weight, height and weight-for-height, weight-for-age and height-for-age Z-scores were evaluated immed

(1) Each year in France, diarrhoea caused by rotavirus kills about 10 infants under three years of age and leads to about 210 hospitalisations per 100 000 children under 5 years of age. (2) Two new oral rotavirus vaccines designed for immunisation during infancy are marketed or shortly to be marketed in France for the first time; one is based on a human strain and the other on both bovine and human strains. (3) The vaccine based on a human strain has been tested in a double-blind placebo-controlled trial involving about 63 000 infants. It had no statistically significant impact on mortality bu

Enteropathogenic Escherichia coli (EPEC) is a leading cause of diarrhea in infants less than two years of age in developing countries. To induce diarrhea EPEC uses several virulence factors acting on a still unknown and mysterious mechanism. The hallmark of EPEC infection is a histological intestinal alteration known as the attaching and effacing (A/E) lesion. The bacterium attaches intimately to the enterocyte and induces assembly of cytoskeleton intracellular actin on the cellular surface. Rearrangements of the actin cytoskeleton form a pedestal-like structure where bacterium tightly cups th

The interactions between diarrhoeal disease and nutritional status are complex and synergistic. These are serious issues globally because they affect hundreds of millions of young children and annually cause > 3 million deaths in children aged under 5 y. Despite intensive field-based and laboratory studies over three decades, many questions remain unanswered about the causes, pathophysiology and best approaches to management and prevention of this "diarrhoea-malnutrition" syndrome. Oral rehydration therapy (ORT) has been a major advance and has saved many lives from acute diarrhoea. However, p

In a case-control study among the urban poor of Dhaka, Bangladesh, the association of maternal education and family income with severity of disease due to diarrhoea in children was examined. After adjusting for family income, 7 or more years of school education was associated with 54% reduced risk of severe disease as indicated by the presence of dehydration. Income in the uppermost quartile of this population, independently of maternal education, was associated with 41% reduced risk of severe disease compared to the lowest quartile. In the logistic regression model the effect of maternal educ

Five children (three boys, two girls) presenting in the first year of life with intractable diarrhoea had a number of features in common. All had ulcerating stomatitis, four had partial villous atrophy on small intestinal biopsy, all had colitis characterised by large ulcers with overhanging edges, and four had severe perianal disease; no stool pathogens were detected. Treatment with steroids, sulphasalazine, and azathioprine was unsuccessful. All five required subtotal colectomy. Four were children of consanguinous marriages, two were siblings of Pakistani origin, two were cousins of Arab ori

Thirteen infants with intractable diarrhea were classified as having severe or moderate malabsorption on the basis of D-xylose absorption. Within each group (designated severe and moderate), patients were randomly assigned to continuous enteral nutrition (CEN) with an elemental formula or to an alternative therapy: total parenteral nutrition (TPN) for the severe patients or intermittent oral nutrition (ION) with the elemental formula for the moderate patients. Within the severe group, CEN and TPN produced similar correction of malnutrition (6.0 +/- 2.5 weeks vs 6.5 +/- 2.5 weeks for weight to
