Contact sensitivity to gentamicin-hydrocortisone ear drops.

Patch testing of ten patients who had been treated with gentamicin-hydrocortisone ear drops for active chronic suppurative otitis media showed evidence of contact sensitivity in five. Positive skin tests correlated with failure to respond to treatment.

Late ear sequelae in cleft palate patients.

This retrospective study looks at the incidence and nature of ear disease in 50 adolescent patients who had cleft palates repaired in infancy. Half of these patients had a history of grommet insertion. We found that most patients had normal hearing (81%) and middle-ear pressures (86%), although about half had tympanic membrane abnormalities. Grommet insertion did not result in better long-term hearing in this study but was strongly associated with tympanosclerosis. Cleft type did not influence the degree of ear disease although more patients with complete clefts had a history of repeated gromm…

Correlation between morbid adenoid and atelectatic ear.

We observed the nasopharynx of 30 patients with atelectatic ears (AE), and 10 with healthy ear drums and mouth breathing by CT scan, to examine the relation between adenoid and otitis media. The adenoids of those with healthy ear drums were of the large posterior type with free space in the pharyngeal fossa, while those with AE were of the large anterior type in 18 patients and of the pendulous projection type over the choana in the other 12. The anatomical location of the adenoids should be considered when one discusses the relation between the adenoid and otitis media. The "Toynbee phenomeno…

Ventilation tube surgery and middle ear irrigation.

Tympanostomy and insertion of ventilation tubes has become one of the most commonly performed operations in the United States. Most authors … in general. However, this practice is not routinely performed when incising and draining the middle ear. We have completed a prospective controlled double blind study on post-tympanostomy tube otorrhea utilizing irrigation of the middle ear. In 220 consecutive cases, the use of middle ear irrigation reduced postoperative infections in the first 6 mont …

Animal models of inner ear vascular disturbances.

Animal models of vascular disorders are identified or developed for the evaluation of functional deficits and morphologic alterations. This information will serve a useful purpose for a better understanding of sudden deafness, Meniere's disease, and presbycusis. The study of microcirculation of the inner ear vessels reveals that their responses to various stimuli, such as anoxia, sympathetic nerve stimulation, hypothermia, and drugs, are different from those of the middle ear vessels. In sudden occlusion of the major vascular supply to the inner ear, the cochlea is found to be more vulnerable …

Inner ear fluids dynamics and endolymphatic hydrops.

The inner ear possesses remarkably stable homeostatic mechanisms for the maintenance of the functional integrity of the inner ear end-organs. The inner ear fluid maintains its homeostasis by a variety of subtle regulatory mechanisms, both locally and systemically. Any disturbance in one of these mechanisms can induce the disruption of homeostasis expressed by ionic, osmotic, or metabolic imbalance between the compartments. This can be manifested as membrane displacement or abnormal functions of the inner ear, depending on the degree of the disturbance. Further studies are necessary to clarify …

Inner ear pathology associated with Reye's syndrome.

Severe pathological changes were observed in the inner ear tissues of a 2-month-old patient who died of Reye's syndrome after 5 days of hospitalization. In the organ of Corti, the inner hair cells appeared to be more severely damaged than the outer hair cells. Various degrees of degeneration were observed in all non-sensory epithelial cells lining the cochlear duct. In most turns of the cochlear duct, Reissner's membrane was ruptured and/or collapsed onto the organ of Corti. Likewise, both sensory and non-sensory cells of the vestibular end organs were markedly degenerated. These observations …

Unilateral cystic inner ear anomalies in siblings.

Unilateral cystic inner ear anomalies were diagnosed in two siblings, a 9 year old boy and a 6 year old girl. X-ray examination of the temporal bone was performed, together with audiological examinations and vestibular function tests. The common tomographic X-ray findings consisted of an enlarged solitary sac type deformity of the vestibule with narrowing of the internal auditory canal, severe hypoplasia of the anterior semicircular canal and no visualized cochlea. Pure-tone audiometry revealed severe mixed type of hearing loss in the right ear in both children. The test for vestibular functio…

Pituitary carcinoma occurring as middle ear tumor.

Pituitary gland tumors are most commonly adenomas of the glandular tissue. They are benign, slow-growing tumors confined to the pituitary fossa region. Pituitary tumors that become aggressive, involving contiguous bony structures and brain, as well as metastasizing out of the confines of the skull, are referred to as pituitary carcinomas. This case of pituitary carcinoma is the first reported in the literature occurring as a middle ear tumor. The literature documents many cases of pituitary carcinoma. The history and medical treatment of this case are presented.

Positive middle ear pressure shown by tympanometry.

The incidence and implications of positive middle ear pressure shown by tympanometry have been sparsely reported in the literature. A series of 13 patients (16 ears) exhibited middle ear pressures that exceeded +49 mm H2O. Clinical, otoscopic, audiologic, and pathologic findings are reviewed. Otalgia or fullness and pharyngitis were the most commonly reported symptoms. Acute middle ear pathologic conditions were otoscopically confirmed in 13 ears. Effusion was present in five of six ears that were selected for myringotomy. The clinical implications of positive middle ear pressure would be part…

An unusual congenital middle ear ossicular anomaly.

Unilateral congenital absence of the stapes suprastructure occurred in association with displaced attachment of the stapedial tendon on an abbreviated incudal lenticular process. The stapes footplate was intact and mobile, and the middle ear was otherwise normal. This is interpreted to be a developmental aberration involving partial absence of a second branchial arch derivative. The preoperative history of no trauma or surgery and the audiologic analysis correlated with the occurrence of this anomaly. This appears to be the first reported case of this unusual congenital anomaly. In addition, i…

Inner ear damage induced by bacterial endotoxin.

Repeated i.d. injection doses of endotoxin lipopolysaccharide of E. coli at certain time intervals were found to give rise to the following changes in the inner ear in mice: In the organ of Corti of the cochlea, there were noted degeneration and disappearance of the inner and outer hair cells of the lower turn, partly with a partial disappearance of nerve fibers in the osseous spiral lamina. Degenerative changes seen in the upper turn were of milder degree. The stria vascularis exhibited enlarged intercellular spaces, the appearance of lysosomes in the constituent cells, and degeneration there…

Viral causes of sudden inner ear deafness.

Clinical, pathologic, and laboratory studies suggesting a viral etiology for sudden deafness have been presented. Although a cause and effect relationship has not been proven, the frequent close association between viral infection and sudden deafness constitutes a strong argument in favor of a viral etiology of sudden deafness in about one of three patients with sudden deafness.

Sensorineural hearing loss in chronic ear surgery.

The author's series of 3000 consecutive operations of tympanoplasty from 1960 to 1975 were reviewed in regard to the occurrence of sensorineural hearing loss as a consequence of the surgical procedure. Worsening of bone conduction thresholds by 10 dB through the frequencies 500 to 4000 cps, or a 10% reduction in speech discrimination scores were considered significant. Whereas in transcanal tympanoplasty the incidence of cochlear damage was greater in ears when initially the ossiculare chain was incomplete, by contrast in combined approach tympanoplasty the risk was greater when the chain was …

[Surgery of inner ear diseases (author's transl)].

In the treatment of Menière's disease, surgical procedures are available when conservative measures have failed. The most distressing symptom of vertigo can frequently be arrested, tinnitus reduced and hearing improved. Sudden deafness due to round window rupture is treatable. The most important disorder with labyrinthe symptoms is the acoustic neuroma, the smaller ones of which can be operated on by otologists in collaboration with neurosurgeons using the transtemporal or translabyrinthine approach with preservation of the facial nerve.
