Subcutaneous peripheral nerve stimulation with inter-lead stimulation for axial neck and low back pain: case series and review of the literature.

OBJECTIVES: While pain in the extremities often responds to treatment using spinal cord stimulation (SCS), axial pain is notoriously refractory to SCS. Interest in subcutaneous peripheral nerve stimulation (SQ PNS) as an alternative to SCS has emerged, but the most appropriate electrode locations and neurostimulator programming techniques are not yet clear. METHODS: A retrospective review was conducted of consecutive patients evaluated from August 2009 to December 2010 who had undergone trial of SQ PNS with inter-lead stimulation for axial spine pain. Patients proceeding to implant were follo

Cochlear implantation in chronic otitis media and previous middle ear surgery: 20 years of experience.

Cochlear implantation in the setting of chronic otitis media or previous middle ear surgery poses several problems for the surgeon: possible spread of infection to the cochlea and the subarachnoid spaces with consequent meningitis, risk of electrode array extrusion and possible recurrence of the original disease. Several surgical strategies have been proposed to overcome these problems. In the present study, clinical and functional results of cochlear implantation in 26 patients with chronic otitis media (8 cases) or previous middle ear surgery (18 cases) in the ear most suitable for implantat

[Atrial sensing with fixed and floating electrodes at identical activities].

Despite advantages of single-lead VDD systems, the frequency of implantation of these pacemakers in patients with symptomatic atrioventricular block and normal sinus rhythm continues to decline. Therefore, data comparing the atrial sensing performance of floating and fixed electrodes at identical activities are rare. The aim of the study was to investigate atrial sensing via floating and fixed atrial electrodes at identical activity levels (supine position, left side position, seated position, hyperventilation, standing, walking, fast walking, walking up and down stairs, for 1.5 min each) by b

A new system for continuous recurrent laryngeal nerve monitoring.

Existing nerve monitoring devices in thyroid surgery are - except for one - mainly intermittently working nerve identification tools. We present a new vagal electrode which allows true continuous monitoring of the recurrent laryngeal nerve (RLN). The electrode was designed as a tripolar hybrid cuff electrode consisting of polyimide, gold and platinum layers embedded in a flexible silicon cuff which can be opened at the long side for introducing the nerve. It is fully implantable and atraumatic. The evoked potentials are sensed by standard thyroid electrodes. Real-time signal analysis and audio

Beta-Band and Aperiodic Activity Across New and Chronic Subthalamic Nucleus Deep Brain Stimulation in Parkinson's Disease.

BACKGROUND: Neurophysiological biomarkers in Parkinson's disease (PD) are increasingly used for deep brain stimulation (DBS). Their long-term utility is not clear, especially when transitioning to sensing-guided DBS years post-surgery. OBJECTIVES: Beta-band power, bursting activity, and periodic and aperiodic components of local field potentials recorded from DBS electrodes in the subthalamic nucleus (STN) were compared between a group of newly and chronically (6.7 ± 4.0 years) implanted PD patients. Clinical severity was rated using the Movement Disorder Society-Unified Parkinson's Disease R

Transforming the Anthropomorphic Passive Free-Flow Foot Prosthesis Into a Powered Foot Prosthesis With Intuitive Control and Sensation (Bionic FFF).

INTRODUCTION: Approximately 89% of all service members with amputations do not return to duty. Restoring intuitive neural control with somatosensory sensation is a key to improving the safety and efficacy of prosthetic locomotion. However, natural somatosensory feedback from lower-limb prostheses has not yet been incorporated into any commercial prostheses. MATERIALS AND METHODS: We developed a neuroprosthesis with intuitive bidirectional control and somatosensation and evoking phase-dependent locomotor reflexes, we aspire to significantly improve the prosthetic rehabilitation and long-term f

Spatially selective stimulation of the pig vagus nerve to modulate target effect versus side effect.

Electrical stimulation of the cervical vagus nerve using implanted electrodes (VNS) is FDA-approved for the treatment of drug-resistant epilepsy, treatment-resistant depression, and most recently, chronic ischemic stroke rehabilitation. However, VNS is critically limited by the unwanted stimulation of nearby neck muscles-a result of non-specific stimulation activating motor nerve fibers within the vagus. Prior studies suggested that precise placement of small epineural electrodes can modify VNS therapeutic effects, such as cardiac responses. However, it remains unclear if placement can alter t

Simultaneous thalamic and posterior subthalamic electrode insertion with single deep brain stimulation electrode for essential tremor.

OBJECTIVES: The optimal target location of deep brain stimulation (DBS) is the subject of some controversy. We implanted electrodes that could stimulate both posterior subthalamic area (PSA) and ventralis intermedius nucleus of thalamus (Vim), and examined the benefits of bilateral DBS of Vim, PSA, and Vim + PSA in patients with essential tremor (ET). MATERIALS AND METHODS: Electrodes were inserted into the Vim and PSA in ten hemispheres of five consecutive patients. All patients were assessed for action tremor, including postural and kinetic tremors, both preoperatively and at six months and

Cortical brain mapping of peripheral nerves using functional magnetic resonance imaging in a rodent model.

The regions of the body have cortical and subcortical representation in proportion to their degree of innervation. The rat forepaw has been studied extensively in recent years using functional magnetic resonance imaging (fMRI), typically by stimulation using electrodes directly inserted into the skin of the forepaw. Here we stimulate the nerve directly using surgically implanted electrodes. A major distinction is that stimulation of the skin of the forepaw is mostly sensory, whereas direct nerve stimulation reveals not only the sensory system but also deep brain structures associated with moto

Long-term recording of olfactory and vomeronasal stimulant-induced waves from the turtle main olfactory bulb and accessory olfactory bulb.

Recording of stimulant-induced waves (bulbar responses) from the main olfactory bulb is a useful tool for measuring quantitative stable olfactory responses. There is a good relationship between the olfactory bulbar response, olfactory nerve response and electroolfactogram (EOG), suggesting that the bulbar response reflects events in receptor cells. The modern whole-cell recording technique offers direct information on olfactory transduction in single cells, but it requires long experimental periods and many animals. On the other hand, analysis of bulbar responses provides useful information an

The effect of cervical sympathectomy on cochlear electrophysiology.

The purpose of this study was to examine the electrophysiologic responses to sound from guinea pig cochleas, 3--6 days after a unilateral cervical sympathectomy. After recovery from surgery the guinea pigs developed a Horner's syndrome on the sympathectomized side. Some days later their bullas were opened and electrodes were placed bilaterally on the round windows. Most of the sympathectomized cochleas showed signs of decreased sensitivity to sound. They had a smaller dynamic range for the click- and tone-burst evoked compound action potential (CAP) compared with the non-sympathectomized cochl

Enhancement of osmotic thirst elicited by electric stimulation of the limbic system in dogs.

Seven dogs were chronically implanted with 30 electrodes directed at various structures of the basal forebrain. Osmotic thirst threshold (expressed as an increase in plasma osmolality and a decrease in cellular water content caused by an intravenous infusion of 5% NaCl, necessary to induce drinking) was measured during electric stimulation of the basal forebrain and compared to that found under control conditions. Stimulation through 11 electrodes located in the lateral part of the septum (5 locations) and other parts of the limbic system markedly decreased the osmotic thirst threshold. The in

[Motor coordination of the gastro-duodenal junction in the dog].

To record mechanical and electrical activities of the gastro-duodenal junction in dogs, before feeding after feeding and after drugs, four extraluminal strain-gauges and four bipolar electrodes were surgically implanted, one set on the pylorus, one in antrum and one on the duodenum. All the records were analyzed on a Nova 830 minicomputer for the frequency content of signals. The pylorus behaved primarily as an extension of the antrum in the sense that it had electrical and mechanical frequencies mostly in the range of 4-5 c/m, same as antrum. It did not show any relaxation in response to feed

Subcontinuous epileptiform activity after failed hippocampal radiosurgery.

PURPOSE: Although gamma-knife radiosurgery (GKS) has proved efficacious in temporal lobe epilepsy (TLE), its antiepileptic mechanism of action remains elusive. Human and experimental data suggest that subnecrotic radiation-induced tissue changes might contribute to the antiepileptic effect of GKS. However, there are no data regarding the evolution of electroencephalography (EEG) activity within GKS-treated hippocampus, information that aid in better understanding both the mechanism of action and the reason for failure of GKS treatment. METHODS: Two patients with disabling seizures 4 and 6 yea

Cochlear implants and electrical brainstem stimulation in sensorineural hearing loss.

Cochlear implants and multichannel auditory brainstem implants enable patients with bilateral total or profound hearing loss to receive at least acoustic information. Both types of prosthesis are based on electrical stimulation of the auditory pathway. Different speech coding strategies and the number of electrodes used may influence the postoperative results. The preoperative evaluation of patients is of utmost importance. The cochlear implant is suitable for patients with hearing loss due to inner ear disorders, but who have functioning hearing nerve. Patients with a defect of the hearing ne

Targeting grasp-related cortical areas for intracortical brain-machine interfaces.

This study aimed to improve intracortical microelectrode array implantation sites for grasp-related motor decoding by integrating anatomical, functional, and vascular imaging with preoperative 3D modeling. A participant with C5 tetraplegia underwent anatomical MRI, diffusion-weighted imaging, and task-based fMRI to identify grasp-related cortical regions while avoiding vasculature and speech-critical areas. Quicktome software was used to refine target selection by integrating structural connectivity and functional activation data. A 3D-printed skull and cortical model enabled preoperative plan
