Anomalies of the course of the facial nerve have been reported in association with middle and inner ear malformations. Bifurcation of its intratemporal portion is a rare malformation in which focal splitting of one or more facial nerve segments occurs. We describe the CT appearance of this anomaly and discuss its possible embryology. Facial nerve bifurcation is important to recognize in patients undergoing evaluation for congenital hearing loss and other congenital ear malformations.

Isolation of the left innominate artery arising from the ductus arteriosus is a rare clinical entity with only nine cases being reported in the literature [5, 7, 8]. We describe a case consisting of tetralogy of Fallot, right aortic arch, and isolation of the left innominate artery arising from a left-sided ductus arteriosus which, to the best of our knowledge, has not been previously reported. The embryology of the aortic arch anomaly is reviewed.

A wide spectrum of sonographic findings can be seen in the first trimester of pregnancy. One must have an appreciation of embryology to recognize normal and abnormal early pregnancy development. In this article we emphasize discriminatory criteria for identification of normal structures, as well as ultrasound findings in threatened abortion, failing pregnancy, multiple gestations, ectopic pregnancy, and trophoblastic disease. Sonoembryology, the identification of embryonic anatomy, is discussed, as are examples of early identification of anomalies.

Although urachal remnants rarely are observed clinically, they often give rise to a number of problems such as fatal infection and late malignant changes. Therefore, a total assessment of the disease with a particular focus on embryology, anatomy, and clinical symptoms, as well as the most advisable treatment, is called for. The authors report on 33 previously experienced cases and discuss the problems involved in both the diagnosis and treatment of these anomalies.

The infant with a congenital diaphragmatic hernia presents many challenges to those caring for him. A thorough understanding of both anatomic and physiologic components can enable the caregiver to accurately assess the patient and employ interventions which will minimize complications. This article discusses the incidence, mortality, embryology, diagnosis, pathophysiology, and conventional management. Part II will discuss alternatives to conventional management, outcome predictors, current research, and follow-up of the infant with a congenital diaphragmatic hernia.

Neonatal obstruction is a radio-clinical syndrome, common to a wide range of malformative anomalies which alter the normal anatomy or function of the digestive tract at any level. The main etiologies are reviewed with emphasis on specificity due to embryology, topography and clinical presentation. It is relatively easy to diagnose neonatal obstruction and relate it to a definite cause. The successful treatment of such obstruction requires a pluridisciplinary collaboration of specialized teams.

We report a case of an asymptomatic pelvic mass that was found during examination for a left ureteral stone. After stone management preoperative computerized tomography demonstrated a 6 by 6 cm. midline pelvic mass posterior to the bladder. Lumbar angiography was normal. A liver/spleen scan and splenic angiography were not performed. At exploration the mass was found to be an accessory pelvic spleen. The embryology, presentation, diagnosis and treatment are discussed.

Laryngeal atresia is a rare, life-threatening anomaly. A case is reported with histological analysis of the deformity. Although there are different theories of glottic development, all authors agree that the primitive glottis is occluded at one time by an epithelial plug. Laryngeal atresia is felt to represent a lack of recanalization of the embryonic larynx. Laryngeal embryology is reviewed and the various types of laryngeal atresia are compared to the stages of laryngeal development.

Two cases of horseshoe lung are described; one was suspected and the other was diagnosed preoperatively. Both underwent successful surgical treatment. The embryology of this anomaly is briefly reviewed with reference to the closely related scimitar syndrome (anomalous venous return of right lung to inferior atriocaval junction). Diagnostic studies are discussed with stress on the need for a thorough functional evaluation of both the heart and lungs before the surgical indication is made.

Several studies show that portions of intramyocardial coronary arteries are spared of arteriosclerosis, involving morphological, embryological, biochemical and pathophysiological aspects. Endothelial function is significantly affected in the segment of transition, as estimated by the vasoactive response to Ach. These findings suggest that myocardial bridge can provide protection against arteriosclerosis by counteracting the negative effects of endothelial dysfunction. OBJECTIVE: Two cases of first branchial cleft fistula with internal opening on the Eustachian tube are reported and the diagnosis, management and embryological hypothesis are discussed.
DESIGN: Retrospective study and review of the literature.
RESULTS: Both patients were young boys with first branchial cleft anomaly clearly identified by computed tomography fistulography scan and direct Methylene Blue dye injection. In both cases, surgical removal revealed a fistula with internal opening located on the Eustachian tube near the nasopharynx.
