Cutaneous leishmaniasis in travellers: a focus on epidemiology and treatment in 2015.

Imported cutaneous leishmaniasis (CL) is a growing problem with increasing global travel to endemic areas. Returned travellers seeking care encounter significant barriers to treatment, including diagnostic delays and difficult access to anti-leishmanial drugs. Treatment recommendations in non-endemic settings are a moving target, reflecting recent developments in Leishmania diagnostics and therapeutics. Accumulating experience with molecular-based species identification has enabled species-directed therapy. Clinicians are reevaluating more toxic traditional regimens in light of newly approved

Building an evidence base for DSM-5 conceptualizations of oppositional defiant disorder and conduct disorder: introduction to the special section.

The DSM-5 ADHD and Disruptive Behavior Disorders Work Group recently outlined a research agenda designed to support possible revisions to the diagnostic criteria for oppositional defiant disorder (ODD) and conduct disorder (CD). Some of the areas in need of further investigation include (a) examining the clinical utility of the current diagnostic system in girls, (b) further clarifying the developmental progression from ODD to CD, (c) determining whether facets of ODD symptoms can help explain heterotypic continuity and enhance predictive validity, (d) evaluating the clinical utility of a new

Investigation of bromide ion effects on disinfection by-products formation and speciation in an Istanbul water supply.

Recent epidemiological studies reported that brominated DBPs may be more carcinogenic than their chlorinated analogs. Thus, this research was designed to investigate the role of bromide ion in the formation and speciation of disinfection by-products (DBPs) during chlorination of Buyukcekmece Lake Water (BLW) in Istanbul. Chlorination of BLW samples was carried out at pH 7.0 with 5 and 12 mg/l chlorine dosages. For each chlorine dosage, six bromide concentrations ranging from 0.05 to 4.0mg/l were added to form a 2 x 6 experimental matrix. In general, increasing bromide concentration gradually s

A risk-factor model of epistatic interaction, focusing on autism.

Research to date on the genetics of autism has not uncovered a major susceptibility locus and indications are that a number of genes, perhaps as many as 15-20, may play detectable but minor roles in the etiology of the condition. To cope with this situation, a risk-factor model based on standard epidemiologic designs is proposed. The model supposes that adding a factor to a fixed set of existing factors always increases the total risk. Thus, according to the model genetic contributions cumulate but are not necessarily additive. A threshold, hence, epistasis is required. The model is applied to

Vitamin E 80th anniversary: a double life, not only fighting radicals.

Recent research on alpha-tocopherol has revealed specific cellular functions of this compound belonging to the vitamin E family. Alpha-tocopherol can act as a radical scavenger, as a pro-oxidant, as an anti-alkylation agent and, most important, by mechanisms that are independent of the above properties. To the last group belong protein kinase C and 5-lipoxygenase inhibition at post-translational level, as well as alpha-tocopherol activation of protein phosphatase 2A and diacylglycerol kinase. Furthermore, at transcriptional level, several genes (CD36, alpha-TTP, alpha-tropomyosin, and collagen

[Early-summer meningo-encephalitis (ESME) and ESME-vaccination: status 2000].

Tick-borne encephalitis (TBE) is a public health problem very well under control in Austria because of a vaccination programme using a safe, efficient and well tolerated vaccine and a carefully designed social marketing concept. The Austrian vaccine underwent another technological updating and is now marketed under a new brand name (TicoVac) on the basis of an EU registration. A second product is also available (Encepur), but some limitations of use have to be taken into account. To improve the epidemiological situation even further (only 41 hospital cases in 1999) special attention has to be

Expulsions in immediate postpartum insertions of Lippes Loop D and Copper T IUDs and their counterpart Delta devices--an epidemiological analysis.

In this paper, an epidemiological analysis was performed, using an international data set, exclusively on the expulsion problem associated with postpartum IUD insertions. The inserter's experience in postplacental insertions is probably an important determining factor for IUD expulsions. Immediate insertions (within 10 minutes after placental delivery) are possibly associated with lower expulsion rates than later insertions (eg. two to 72 hours after placental delivery) during the woman's postpartum hospitalization. No significant differences were detected between the standard Lippes Loop D an

Bisphenol A and cancer: a critical review of the epidemiologic literature with an emphasis on exposure assessment.

BACKGROUND: Bisphenol A (BPA) is a high production volume chemical that has been used for decades in numerous consumer and industrial applications. Daily exposure to BPA is likely; it is readily detected in >90% of the general population despite being rapidly metabolized and excreted. BPA's toxicity, including endocrine disrupting activity, has sparked public health concern. We comprehensively reviewed the epidemiologic literature on the carcinogenicity of BPA and highlighted exposure assessment considerations that impact study interpretation. METHODS: Multiple biomedical databases were searc

Assessment of the CDC hypospadias atlas for epidemiological research - A survey of experts.

BACKGROUND: Hypospadias is a common congenital malformation. There are several classification systems for hypospadias. However, the phenotypic subtypes of hypospadias are inconsistently reported across databases due to the inherently subtle variability of the penile anatomy. Consistent classification of hypospadias would enable more accurate data regarding etiological correlates of specific subtypes as well as perioperative care and surgical outcomes. Existing classifications are designed for use by hypospadias surgeons, although there is a need for a system for epidemiologic data abstractors,

[Epidemiological surveillance of pulmonary tuberculosis treated at the specialized care level based on 2 data sources, Valladolid; Spain].

BACKGROUND: Pulmonary tuberculosis is still more frequent that it should be in Spain given the degree of Spain's social and healthcare-related development. Apart from some individual studies, such as the Multicenter Tuberculosis Research Project, the incidence of tuberculosis is known by way of the Compulsory Notifiable Disease System, in which some degree of under-notification has been detected. The question has been raised as to whether this data can be improved through the additional use of another registry, specifically the Minimum Basic Data Set (MBDS). METHODS: This is a retrospective s

Issues in filariasis--a century of enquiry and a century of failure.

A great deal is known about the biology of the filarial parasites infecting man and of their transmission, their clinical manifestations, their treatment and their control but many more people are infected with Bancroftian filariasis and onchocerciasis today than more than 100 years ago when Manson discovered the first live cycle. There has been some success in the control of the Simulium vectors of onchocerciasis in parts of Africa and mass chemotherapy has helped to reduce Bancroftian filariasis in a few Pacific Islands but the main endemic areas have seen no benefit from all the advance in

A systematic literature review and critical appraisal of epidemiological studies on outdoor air pollution and tuberculosis outcomes.

Ambient air pollution is the leading environmental risk factor for disease globally. Air pollutants can increase the risk of some respiratory infections, but their effects on tuberculosis (TB) are unclear. In this systematic literature review, we aimed to assess epidemiological studies on the association between outdoor air pollutants and TB incidence, hospital admissions and death (collectively referred to here as 'TB outcomes'). We sought to consolidate available evidence on this topic and propose recommendations for future studies. Following PRISMA guidelines, we searched PubMed, Web of Sci

Febrile illness in Asia: gaps in epidemiology, diagnosis and management for informing health policy.

BACKGROUND: Increasing evidence is becoming available on the aetiology and management of fevers in Asia; the importance of these fevers has increased with the decline in the incidence of malaria. AIMS: To conduct a narrative review of the epidemiology and management of fevers in South and South-East Asia and to highlight gaps in our knowledge that impair evidence-based health policy decisions. SOURCES: A narrative review of papers published since 2012 on developments in fever epidemiology, diagnosis and treatment in South and South-East Asia. The papers that the authors felt were pivotal, fr

Risk stratification in hypertension: new insights from the Framingham Study.

Five decades of epidemiologic research have established that blood pressure elevation is a common and powerful contributor to all of the major cardiovascular diseases, including coronary disease, stroke, peripheral artery disease, renal disease, and heart failure. The common variety of hypertension designated benign essential hypertension was not shown to be either benign or essential. Although clinicians favor the diagnosis and treatment of hypertension in terms of diastolic blood pressure elevation and categoric cut points, epidemiologic data show a more important influence of systolic blood

Associations between criteria air pollutants and asthma.

The evidence that asthma is increasing in prevalence is becoming increasingly compelling. This trend has been demonstrated not only in the United States, but also in the United Kingdom, New Zealand, Australia, and several other Western countries. In the United States, the increase is largest in the group under 18 years of age. There is mounting evidence that certain environmental air pollutants are involved in exacerbating asthma. This is based primarily on epidemiologic studies and more recent clinical studies. The U.S. Clean Air Act of 1970 provides special consideration to the class of outd
