Dysfunctional Brain Networking among Autonomic Regulatory Structures in Temporal Lobe Epilepsy Patients at High Risk of Sudden Unexpected Death in Epilepsy.

BACKGROUND: Sudden unexpected death in epilepsy (SUDEP) is common among young people with epilepsy. Individuals who are at high risk of SUDEP exhibit regional brain structural and functional connectivity (FC) alterations compared with low-risk patients. However, less is known about network-based FC differences among critical cortical and subcortical autonomic regulatory brain structures in temporal lobe epilepsy (TLE) patients at high risk of SUDEP. METHODS: 32 TLE patients were risk-stratified according to the following clinical criteria: age of epilepsy onset, duration of epilepsy, frequenc

Using spectral and temporal filters with EEG signal to predict the temporal lobe epilepsy outcome after antiseizure medication via machine learning.

Epilepsy is a neurological disorder in which the brain is transiently altered. Predicting outcomes in epilepsy is essential for providing feedback that can foster improved outcomes in the future. This study aimed to investigate whether applying spectral and temporal filters to resting-state electroencephalography (EEG) signals could improve the prediction of outcomes for patients taking antiseizure medication to treat temporal lobe epilepsy (TLE). We collected EEG data from a total of 46 patients (divided into a seizure-free group (SF, n = 22) and a non-seizure-free group (NSF, n = 24)) with T

Amplitude synchronization of spontaneous activity of medial and lateral temporal gyri reveals altered thalamic connectivity in patients with temporal lobe epilepsy.

In this study, we examined whether amplitude synchronization of medial (MTL) and lateral (LTL) temporal lobes can detect unique alterations in patients with MTL epilepsy (mTLE) with mesial temporal sclerosis (MTS). This was a retrospective study of preoperative resting-state fMRI (rsfMRI) data from 31 patients with mTLE with MTS (age 23-69) and 16 controls (age 21-35). fMRI data were preprocessed based on a multistep preprocessing pipeline and registered to a standard space. Using each subject's T1-weighted scan, the MTL and LTL were automatically segmented, manually revised and then fit to a

Putative Transcriptomic Endotypes Reveal Neuronal, Immune, and Metabolic Heterogeneity in Temporal Lobe Epilepsy with Hippocampal Sclerosis.

Temporal lobe epilepsy with hippocampal sclerosis (TLE-HS) is a biologically heterogeneous disorder in which neuronal loss, gliosis, neuroinflammation, and metabolic stress may coexist but are unlikely to be uniformly represented across all patients. To investigate this heterogeneity at the molecular level, we analysed publicly available hippocampal transcriptomic data from individuals with TLE-HS. Our analyses identified three transcriptomically defined putative endotypes within this shared neuropathological diagnosis. These states were associated with distinct molecular programs, including a

Comparing the cognitive-motor performance of individuals with temporal lobe epilepsy versus healthy controls using robotics.

Cognitive impairments are common in individuals with temporal lobe epilepsy (TLE). Interactive Kinarm robotic systems provide a novel approach to quantify cognitive impairment. The Object Hit and Avoid (OHA) and Reverse Visually Guided Reaching (RVGR) tasks examine higher order neurocognitive domains (executive function, attention, visuospatial function). This study explores differences in OHA and RVGR performance between patients with TLE and healthy controls, and measures their sensitivity to impairment. Participants completed the Kinarm assessment. Task parameter performance quantifying dif

Wnt activation prevents epileptogenic hippocampal remodeling in animal models of unilateral and bilateral temporal lobe epilepsy.

Temporal lobe epilepsy (TLE) is a heterogeneous disorder with most clinical presentations involving unilateral or bilateral hippocampal seizure onsets. Antiseizure medications are often ineffective for TLE, and epilepsy surgery can have variable outcomes. Risk factors for TLE are readily identifiable and typically precede chronic epilepsy, providing a window of opportunity for preventative treatments. However, there are currently no clinically approved anti-epileptogenic therapies. In this study, we investigate the role of Wnt signaling in epileptogenesis using two mouse TLE models, the intrah

Comparative Review of Seizure and Cognitive Outcomes in Resective, Ablative, and Neuromodulatory Temporal Lobe Epilepsy Surgery.

Resective surgery for drug-resistant temporal lobe epilepsy remains underutilized in the United States. While anteromesial temporal lobectomy consistently achieves the highest rates of long-term seizure freedom, it comes with greater risks for memory and language decline. Magnetic resonance imaging-guided laser interstitial thermal therapy and neuromodulation have gained popularity due to perceived lower surgical risk and faster recovery, although they yield lower rates of sustained seizure freedom. Neuromodulation with vagus nerve, deep brain, or responsive neurostimulation provides an option

From Stumbling Blocks to Stepping Stones: Progress in Treating Temporal Lobe Epilepsy With Stem Cell Transplantation.

The last three decades of scientific research provided a wealth of data on the brain origins, development, and functional roles of GABAergic interneurons and new insights into GABAergic interneuron dysfunction in different types of epilepsy. A stumbling block in treating GABAergic interneuron dysfunction in acquired temporal lobe epilepsy (TLE) has been the incapacity of the adult human brain to replace interneurons through adult neurogenesis. Recent advances in the field of stem cell biology led to the development of pluripotent stem cells (iPSCs), and this technology has been used in combina

Responsive neurostimulation for patients with refractory mesial temporal lobe epilepsy: A systematic review and meta-analysis.

Responsive neurostimulation (RNS) is a well-established adjuvant therapy for reducing seizure frequency in adults with medically refractory partial-onset seizures, particularly in individuals who are either not candidates for surgical resection or remain seizure-prone post-surgery. However, its effectiveness in patients with mesial temporal lobe epilepsy (MTLE) remains unclear. This meta-analysis evaluates the efficacy of RNS therapy in individuals with medically refractory MTLE. A systematic search of PubMed, Cochrane, and Scopus databases was conducted to identify eligible studies. Outcomes

Individualized structural network deviations predict surgical outcome in mesial temporal lobe epilepsy: a multicenter validation study.

BACKGROUND: Surgical resection is an effective treatment for medically refractory mesial temporal lobe epilepsy (mTLE), however, more than one-third of patients fail to achieve seizure freedom after surgery. This study aimed to evaluate preoperative individual morphometric network characteristics and develop a machine learning model to predict surgical outcome in mTLE. METHODS: This multicenter, retrospective study included 189 mTLE patients who underwent unilateral temporal lobectomy and 78 normal controls between February 2018 and June 2023. Postoperative seizure outcomes were categorized a

Sleep-related disruptions potentially synergistically exacerbate attentional impairments in temporal lobe epilepsy, especially the alerting network.

AIM: To explore the potential synergistic effect of sleep-related disruptions (SRD) on attention dysfunction in temporal lobe epilepsy (TLE), focusing on how SRD impacts specific attention networks, including alertness, orientation, and executive control. METHODS: Ninety-three patients with TLE and matched healthy controls participated. Attention was assessed using the Attention Network Test (ANT) and eye-tracking data, while SRD severity was determined via the Pittsburgh Sleep Quality Index. Participants were categorized based on SRD severity, and attention performance across the three netwo

Stronger pre-surgical functional connectivity networks are associated with improved surgical outcome in temporal lobe epilepsy.

OBJECTIVE: This study investigates the network differences in pre-surgical resting-state EEG between individuals with unilateral temporal lobe epilepsy who become seizure-free and those who continue to have seizures after epilepsy surgery. METHODS: We retrospectively identified 15 individuals with unilateral TLE who were seizure-free (SF) after epilepsy surgery and 13 who continued to have seizures post-surgery (nSF). Functional connectivity was measured using cross-correlation techniques on clips of resting-state scalp EEG that were free of any visually apparent epileptiform abnormalities.

Individualized Target Selection of Closed-Loop Electrical Stimulation for the Treatment of Spontaneous Temporal Lobe Epilepsy.

Closed-loop neuromodulation holds significant promise for treating refractory epilepsy, but the lack of specificity and individualization considerably limits its clinical efficacy. Given the inherent complexity of epilepsy, which involves multiple brain regions and significant interindividual variability, a network-guided, personalized approach is essential. This study aims to develop precise, individualized neuromodulation strategies by leveraging unique brain network characteristics. Using a closed-loop system in chronic temporal lobe epilepsy (cTLE) rats, continuous neural signals were anal

Diffusion Tensor Imaging Versus Conventional MRI in Temporal Lobe Epilepsy: A Comparative Study of Diagnostic Performance.

BACKGROUND: Conventional magnetic resonance imaging (MRI) may appear normal in a proportion of patients with temporal lobe epilepsy (TLE). Diffusion tensor imaging (DTI) provides quantitative measures of white matter microstructure and may reveal subtle changes not visible on routine MRI. This study compared DTI-derived diffusion metrics with standard MRI epilepsy protocols in patients with unilateral TLE to assess the extent and pattern of white matter alterations. METHODS: A cross-sectional observational study was conducted in the Department of Radiodiagnosis at Sree Balaji Medical College
