[Primary, definitive treatment of the nasoethmoidorbital-midface-frontobasal injuries in the maxillofacial practice].

Nasoethmoid-orbital (NEO) fractures are typically resulted by a drastic, forceful blow to the central aspect of the head and nose, associated injuries to adjacent structures occur and involve the midface, anterior cranial fossa and the eye. Dislocated bony parts of the nose may be impacted into the intraorbital region and lead to severe complications: cerebrospinal fluid leak, optic nerve lesion, anosmy, telecanthus and diplopy. In the complex management of the craniofacial lesions, a cooperative team-work of maxillofacial surgeon, neurosurgeon, otolaryngologist and ophthalmologist is required

Infectious aneurysm of the intracavernous carotid artery occurring concomitantly with sphenoid sinusitis; an autopsy case report.

A previously healthy 56-year-old woman presented with right-sided ophthalmic pain and diplopia following headache and fever. A neurological examination revealed 3rd and 6th right cranial nerve palsies. Brain magnetic resonance imaging (MRI) and 3D-computed tomography (CT) angiography (CTA) showed right-sided sphenoid sinusitis, cavernous sinus thrombophlebitis, and aneurysms in the right intracavernous carotid artery and in a portion of internal carotid-posterior communicating artery. We diagnosed her condition as cavernous sinus syndrome with an infectious aneurysm secondary to sphenoiditis;

Age-related size reduction of foramina in the cribriform plate.

Anecdotal evidence suggests that the foramina of the cribriform plate which transmit cranial nerve I decrease in size with age, but this finding has never been supported with quantitative data. It has also been observed that olfactory function declines with increasing age. It has been hypothesized that the cribriform plate foramina closure may be responsible for the olfactory performance decrease with age. We gathered quantitative data to test an age-related decline in cribriform plate foramina area. We report data for the area of patent foramina in the posterior 1 cm of 57 cribriform plates f

Microsurgical interhemispheric approach to dural arteriovenous fistulas of the floor of the anterior cranial fossa.

Six patients with a dural arteriovenous fistula (DAVF) of the floor of the anterior cranial fossa underwent microsurgical treatment. Two of them were operated using a conventional frontobasal approach, and four using an interhemispheric approach. The interhemispheric approach offers the advantages of sparing the frontal sinus, minimizing frontal lobe retraction, and providing a visual angle perpendicular to the floor of the anterior fossa and an excellent view of the fistula located on the cribriform plate at the level of the foramen caecum. Using the interhemispheric route, the malformation w

[Treatment of osteo-dural injuries of the anterior segment of the skull base through the trans-lesion approach. Apropos of 25 cases surgically treated by a dual neurosurgical and maxillofacial team].

The authors describe a technique of mobilisation of the frontal sinus using the fractures of anterior and posterior walls. The translesional approach is adapted to cranio-facial borders surgery of traumatisms. It's a skull base approach which minimizes the cerebral retraction and enables or perfect viewing of the top of the ethmoid. The advantages, drawbacks and indications are discussed about 25 cases.

Surgical nuances for removal of olfactory groove meningiomas using the endoscopic endonasal transcribriform approach.

Olfactory groove meningiomas represent 10% of intracranial meningiomas and arise in the midline of the anterior cranial fossa along the dura of the cribriform plate and planum sphenoidale. Hyperostosis of the adjacent underlying bone is common, and further extension into ethmoid sinuses and nasal cavity can occur in 15%-25% of cases. Radical tumor resection including the involved dural attachment and underlying hyperostotic bone offers the best chance of a Simpson Grade I resection to minimize recurrence. Incomplete removal of involved hyperostotic bone can result in tumor recurrence at the cr

Sinonasal Glomangiopericytoma: Review of Imaging Appearance and Clinical Management Update for a Rare Sinonasal Neoplasm.

Introduction: Glomangiopericytoma (GPC) is a rare tumor in the nasal cavity or paranasal sinuses with low malignant potential. Initially deemed a hemangiopericytoma, in 2005 it was classified as a distinct entity by the World Health Organization (WHO). Case Presentation: A male patient in his early 60s presented with new-onset right arm and leg weakness/numbness, who was incidentally found to have a left ethmoid sinus mass with extension in the olfactory fossa. On CT and MRI, the mass enhanced with well-defined borders and eroded the bone, but without dural enhancement. The mass was surgically

Nasal hemangiopericytoma presenting with oncogenic osteomalasia: A case report and literature review.

Sinonasal type hemangiopericytoma is a rare soft tissue tumor. Oncogenic osteomalacia (tumor-induced osteomalacia) is a rare syndrome that develops especially due to benign mesenchymal tumors. Nonspecific general bone pain and weakness delay the diagnosis and treatment of oncogenic osteomalacia, and it is difficult to determine the localization of the primary tumor causing oncogenic osteomalacia. A 43-year-old male patient with nasal hemangiopericytoma with symptoms of oncogenic osteomalacia is presented. The patient had musculoskeletal complaints at first and was diagnosed with lumbar disc he

A 3-Year-Old Girl with Recurrent Infections and Autoimmunity due to a STAT1 Gain-of-Function Mutation: The Expanding Clinical Presentation of Primary Immunodeficiencies.

We report a 3-year-old Peruvian girl, born to non-consanguineous parents. At the age of 8 months, she had a severe pneumonia complicated with empyema that required thoracic drainage and mechanical ventilation. Although no microorganisms were isolated, the patient recovered with broad-spectrum antibiotics. Since that date, she has presented multiple episodes of pneumonia and recurrent episodes of bronchospasm. At 1 year 5 months of age, the patient began with recurrent episodes of oropharyngeal, vaginal, and skin candidiasis, which improved transiently after using oral azole drugs. At 2.5 years

Medial and Superior Orbital Decompression: Improving Access for Endonasal Endoscopic Frontal Sinus Surgery.

OBJECTIVE: Visualization by Draf I-III endoscopic access to the frontal sinus via drainage pathways is sometimes inadequate. We compare lateral frontal sinus exposures by Draf approaches versus our modification of removing the medial-superior wall of the orbit while preserving the periorbita. METHODS: Twenty cadaveric heads dissected using Draf IIB, III, and modified Draf III with medial and superior orbital decompression (MSOD) underwent thin-cut computed tomography (CT) scanning. Under image guidance, measurements extended from the midline crista gali to the most lateral point of the fronta

Drainage of cells and soluble antigen from the CNS to regional lymph nodes.

Despite the absence of conventional lymphatics, there is efficient drainage of both cerebrospinal fluid (CSF) and interstitial fluid (ISF) from the CNS to regional lymph nodes. CSF drains from the subarachnoid space by channels that pass through the cribriform plate of the ethmoid bone to the nasal mucosa and cervical lymph nodes in animals and in humans; antigen presenting cells (APC) migrate along this pathway to lymph nodes. ISF and solutes drain from the brain parenchyma to cervical lymph nodes by a separate route along 100-150 nm wide basement membranes in the walls of cerebral capillarie…

Treatment of anterior skull base defects by a transnasal endoscopic approach in children.

OBJECT: The object of this study was to assess the efficacy and complications of endoscopic management of anterior skull base defects. METHOD: The authors reviewed the medical records of 28 children (20 boys and 8 girls) undergoing endoscopic repair of anterior skull base defects in their tertiary referral center between 2001 and 2008; 18 cases were congenital and 10 cases posttraumatic. During the endoscopic procedure, rigid telescopes--2.7 or 4 mm in diameter, with 0° or 30° lenses--were used. In 23 patients the anterior skull base defect was sealed with fragments of middle turbinate (bone

[Endoscopic treatment of sphenoid aspergilloma].

In recent years there has been an increase in fungal infections of the nose and paranasal sinuses, even though it is difficult to ascertain the incidence and prevalence of such disorders. The only information which can be drawn from the literature is that such infections are diagnosed in 6% to 13% of surgically treated chronic sinusitis. Aspergillus is the etiologic agent in approximately 80% of cases. Fungal infections are classified into two groups: non-invasive and invasive. The former includes mycetoma and allergic fungal sinusitis. The latter comprises the chronic indolent form, which pre
