Advancements in diagnostic techniques for scorpion venom identification: A comprehensive review.

Scorpion envenomation's ignored public health problem in tropical and subtropical countries is alarming. Particularly dangerous for small children and the elderly, it can cause severe problems and even death. Recent studies have proposed the creation of rapid, easy, species-specific, and sensitive detection kits as an alternative to the methods currently used to identify scorpions. Unfortunately, there is currently no commercially available technology for detecting scorpion envenomation in clinical settings, especially in remote tropical health centres. This study delineates the most dangerous

Emerging therapeutic strategies for optic nerve regeneration.

The optic nerve, comprising axons from retinal ganglion cells (RGCs), is a component of the central nervous system (CNS) that generally exhibits a limited regeneration capacity following injury in mature mammals, resulting in permanent vision loss. Here, we summarize recent advances in interventions targeting cell-intrinsic and cell-extrinsic mechanisms to enhance RGC axon regeneration. Additionally, we summarize strategies for guiding the reconnection of regenerating axons with brain visual targets, aiming to restore partial visual function. Given the advent of high-throughput screening techn

Specifications of the ACMG/AMP guidelines for ACADVL variant interpretation.

Very long-chain acyl-CoA dehydrogenase (VLCAD) deficiency (VLCADD) is a relatively common inborn error of metabolism, but due to difficulty in accurately predicting affected status through newborn screening, molecular confirmation of the causative variants by sequencing of the ACADVL gene is necessary. Although the ACMG/AMP guidelines have helped standardize variant classification, ACADVL variant classification remains disparate due to a phenotype that can be nonspecific, the possibility of variants that produce late-onset disease, and relatively high carrier frequency, amongst other challenge

48-hour Induction of Transdermal Buprenorphine to Extended-release Buprenorphine.

Buprenorphine extended-release (BUP-XR) provides sustained delivery of buprenorphine to control withdrawal and craving symptoms in the form of a monthly injectable and has been shown to improve health outcomes in patients with opioid use disorder. It is recommended that patients are stabilized with a transmucosal buprenorphine product, for at least 7 days per the product monograph; however, clinically, this timeline may be expedited. We report a case of a hospitalized patient with unregulated fentanyl use who underwent a successful transdermal buprenorphine induction for 48 hours to initiate B

Dry eye disease in astronauts: a narrative review.

Long-duration spaceflight can have adverse effects on human health. One of the most common ocular conditions experienced by astronauts is dry eye disease (DED). Symptoms of DED include feelings of eye irritation, eye strain, foreign body sensation and blurred vision. Over 30% of International Space Station expedition crew members reported irritation and foreign body sensation. We reviewed the current literature on the prevalence and mechanisms of DED in astronauts and its potential implications for long-duration spaceflight, including the influence of environmental factors, such as microgravit

Performance and Interpretation of Sonography in the Practice of Nephrology: Core Curriculum 2024.

Ultrasonography is increasingly being performed by clinicians at the point of care, and nephrologists are no exception. This Core Curriculum illustrates how ultrasonography can be incorporated into clinical decision making across the spectrum of kidney disease to optimize the care nephrologists provide to patients. Sonography is valuable in outpatient and inpatient settings for the diagnosis and management of acute and chronic kidney disease, evaluation of cystic disease, urinary obstruction, pain, hematuria, proteinuria, assessment of volume status, and in providing guidance for kidney biopsy

Treating opioid use disorder with rapid micro induction technique of sublingual buprenorphine/naloxone in an outpatient setting-a case report.

In this case report, we present a patient on Medication for Opioid Use Disorder (MOUD) for opioid use disorder (OUD) enrolled in an outpatient addictions clinic located inside a metropolitan academic hospital. He had been stable on methadone for more than six years, and had requested to be transitioned to buprenorphine due to economic constraints. He had initially been unsuccessful completing traditional induction due to withdrawal symptoms. By using the rapid micro induction technique, which benefits from the use of ancillary medications for comfort, the patient successfully transitioned to b

Swallowing and laryngeal complications in lung and heart transplantation: Etiologies and diagnosis.

Despite continued surgical advancements in the field of cardiothoracic transplantation, post-operative complications remain a burden for the patient and the multidisciplinary team. Lesser-known complications including swallowing disorders (dysphagia), and voice disorders (dysphonia), are now being reported. Such disorders are known to be associated with increased morbidity and mortality in other medical populations, however their etiology amongst the heart and lung transplant populations has received little attention in the literature. This paper explores the potential mechanisms of oropharyng

A Prolonged Neurological Presentation of Legionnaire's Disease.

Legionnaire's disease is an uncommon pneumonic disease that can carry a mortality rate up to 20%. It commonly presents as an atypical lower respiratory illness. However, it is important to be mindful of the various extra-pulmonic presentations of the infection. Here, we present a case of a 51-year-old female who presented to the emergency department with lethargy, slurred speech, and confusion. Legionella has been reported to present with neurological symptoms but it is not a common occurrence in each hospitalization. These neurological symptoms often lead to an extensive workup and the comple

Large-scale RNAi screening uncovers therapeutic targets in the parasite Schistosoma mansoni.

Schistosome parasites kill 250,000 people every year. Treatment of schistosomiasis relies on the drug praziquantel. Unfortunately, a scarcity of molecular tools has hindered the discovery of new drug targets. Here, we describe a large-scale RNA interference (RNAi) screen in adult Schistosoma mansoni that examined the function of 2216 genes. We identified 261 genes with phenotypes affecting neuromuscular function, tissue integrity, stem cell maintenance, and parasite survival. Leveraging these data, we prioritized compounds with activity against the parasites and uncovered a pair of protein kin

In Vivo Bioluminescence Imaging to Assess Compound Efficacy Against Trypanosoma brucei.

Traditional animal models for human African trypanosomiasis rely on detecting Trypanosoma brucei brucei parasitemia in the blood. Testing the efficacy of new compounds in these models is cumbersome because it may take several months after treatment before surviving parasites become detectable in the blood. To expedite compound screening, we have used a Trypanosoma brucei brucei GVR35 strain expressing red-shifted firefly luciferase to monitor parasite distribution in infected mice through noninvasive whole-body bioluminescence imaging. This protocol describes the infection and in vivo biolumin

Antibody-dependent enhancement and SARS-CoV-2 vaccines and therapies.

Antibody-based drugs and vaccines against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) are being expedited through preclinical and clinical development. Data from the study of SARS-CoV and other respiratory viruses suggest that anti-SARS-CoV-2 antibodies could exacerbate COVID-19 through antibody-dependent enhancement (ADE). Previous respiratory syncytial virus and dengue virus vaccine studies revealed human clinical safety risks related to ADE, resulting in failed vaccine trials. Here, we describe key ADE mechanisms and discuss mitigation strategies for SARS-CoV-2 vaccines and

Lung Point-of-Care Ultrasound in Pediatric COVID-19: A Case Series.

Lung point-of-care ultrasound (POCUS) has been shown to be useful for identifying pulmonary pathology in adult patients with coronavirus disease 2019 (COVID-19). However, pediatric literature for POCUS in COVID-19 is limited. The objective of this case series was to describe lung POCUS findings in pediatric patients with COVID-19. Three patients with COVID-19 who had lung POCUS performed in a pediatric emergency department were included. Point-of-care ultrasound revealed bilateral abnormalities in all patients, including pleural line irregularities, scattered and coalescing B-lines, consolidat

Holistic Patient Assessment: Dermatology in Dental Practice.

As part of the holistic approach to their patients, General Dental Practitioners are well placed to identify common skin lesions. Awareness and recognition of worrying lesions allow timely and appropriate referrals for further investigation and treatment. In this paper, we review benign, premalignant and malignant skin lesions, as well as genetic skin conditions. Past medical, family and social history (including sun exposure and previous cutaneous malignancy) is important. Examination includes the lesion, the skin type and the regional lymph nodes. The different common lesions are described,

Successful Distal Urethral Stone Removal in the Emergency Department.

Nephrolithiasis is a common pathology encountered in the primary care and emergency department (ED) setting. In 2009 alone, there were over one million ED visits related to nephrolithiasis Higa et al. (2017) [1]. Emergent treatment options range from non-invasive pain control and patient education to lithotripsy and other invasive urologic procedures depending on stone location and related pathology. Urethral calculi are estimated to represent 0.3% of all urinary stone diseases Verit et al. (2006) [2]. There are very few case reports documenting distal urethral stone removal in the ED. Here we

Robotic Resection of Ectopic Parathyroid Glands in the Superior Posterior Mediastinum.

Background: Ectopic parathyroid glands can present in a challenging location in the superior posterior mediastinum. Methods: Two patients with primary hyperparathyroidism were operated on for ectopic paraesophageal parathyroid glands in the superior posterior mediastinum. Sestamibi scan, computed tomography (CT) scan, and photon emission CT were used to identify the exact location of these glands. We describe a minimally invasive resection using a three-arm robotic-assisted thoracoscopic technique. Results: Both lesions were completely resected with using the port-based robotic approach with e

A successful use of therapeutic plasma exchange in a fulminant form of acute fatty liver of pregnancy.

Acute fatty liver of pregnancy (AFLP) continues to raise special concerns since its first post-mortem description by Sheehan in 1940. While early diagnosis and expedite delivery are the cornerstone of management, this condition remains fatal for both mother and fetus. Acute fulminant liver failure is the most serious and life-threatening AFLP-related complication and can require liver transplant despite aggressive supportive management. In lieu of transplant, therapeutic plasma exchange (PE) has emerged as a life-saving alternative and has, in few reports, demonstrated efficacy for the reversa

Cross-Coupling of Alkyl Redox-Active Esters with Benzophenone Imines: Tandem Photoredox and Copper Catalysis.

Alkyl amines are an important class of organic compounds in medicinal and materials chemistry. Until now very have been very few methods for the synthesis of alkyl amines by metal-catalyzed cross-coupling of alkyl electrophiles with nitrogen nucleophiles. Described here is an approach to employ tandem photoredox and copper catalysis to enable the cross-coupling of alkyl N-hydroxyphthalimide esters, readily derived from alkyl carboxylic acids, with benzophenone-derived imines. Hydrolysis of the coupling products furnish alkylated primary amines. Primary, secondary, and tertiary alkyl groups can

Oral manifestations of lipoid proteinosis in a 10-year-old female: A case report and literature update.

Lipoid proteinosis (LP) is a rare autosomal recessive disorder characterized by the deposition of amorphous hyaline material in the dermis and submucosal connective tissue. Here, we present a case of LP with significant oral, dermatologic, and neurologic manifestations occurring in a 10 year-old female of Asian descent. In addition to the more typical oral findings of restricted tongue movement and labial and buccal mucosal involvement, this case highlights an unusual pattern of gingival enlargement infrequently reported in the literature. As LP almost always involves the oral cavity, often be

Orbital mantle cell lymphoma presenting as myasthenia gravis.

A 69-year-old man, previously treated with pyridostigmine for myasthenia gravis (manifesting as ptosis and diplopia) was evaluated for several concomitant bilateral anterior orbital masses. Imaging revealed 3 discrete, solid masses within and around the orbits. An incisional biopsy demonstrated atypical lymphocytes positive for CD20 and Cyclin-D1, consistent with mantle cell lymphoma. The patient received induction chemotherapy with a rituximab-based regimen. He experienced resolution of his diplopia and ptosis after one cycle of chemotherapy and achieved complete remission of the orbital mass
