Hematoma refers to the collection or extravasation of blood, usually clotted, in a closed tissue space. It is caused by leakage from local vessels damaged by blunt trauma, local injury, or surgical dissection. In the postoperative phase, a hematoma often results in edema, pain, wound dehiscence, infection, and scarring of the surgical wound. We describe a 44-year-old woman who developed severe complications, including hematoma, abscess, failure of internal fixation, and loss of soft-tissue structures, after hallux abducto valgus surgery. Hospitalization was required for infection control, soft

BACKGROUND: Comminuted fractures of the patella are traditionally treated by internal fixation. The modified tension-band technique is widely accepted for internal fixation. The management of comminuted fractures often requires additional K-wire and/or circlage material and wide surgical exposure. Therefore, symptoms and complications related to the stainless steel wire are not uncommon in these cases. We present a new alternative treatment technique for comminuted patellar fractures.
METHODS: Five comminuted patellar fractures of four patients were treated by circular external fixator (CEF)

BACKGROUND: Although the use of a sliding hip screw is considered to be the preferred treatment for pertrochanteric femoral fractures, we theorized that external fixation could produce clinical outcomes equal to, if not better than, the outcomes obtained with conventional treatment. Furthermore, because external fixation is minimally invasive, we expected a lower rate of morbidity and a reduced need for blood transfusions. Therefore, we compared the two treatments in a clinical trial of elderly patients with pertrochanteric fracture.
METHODS: Forty consecutive elderly female patients who had

OBJECTIVES: To compare the mechanical stability of external fixation with and without spanning of the ankle joint with a foot plate in an in vitro model of extra-articular distal tibia fractures.
DESIGN: A laboratory investigation was performed to evaluate the mechanical behavior of external fixation of extra-articular distal tibia fractures using a fixator with and without a foot plate. Ten fresh-frozen lower extremities (5 pairs) with a simulated OTA 43-A3.3 fracture were stabilized with an Ilizarov hybrid fixator with and without a foot plate.
SETTING: All mechanical testing was performed

The present study compares a recent approach for congenital pseudarthrosis of the tibia, combining a free vascularized fibula graft and an Ilizarov external fixator. Between 1984 and 1993, seven vascularized fibula grafts were performed. In the first six cases, casts or unilateral external fixators were applied, and five of these cases were complicated by fractures of the graft, necessitating secondary procedures or long-term immobilization. The mean follow-up was 12.1 years. Five patients can now walk unassisted, and one uses a brace only outdoors. On average, a brace was required for 18 mont…

The advantages of Ilizarov external fixation, allowing early motion of adjacent joints during fixation of periarticular fractures, have not yet been applied to distal radius fractures. A magnetic resonance imaging study of 10 normal volunteers evaluated the safety of passing percutaneous transfixion pins across the distal radius in 3 forearm positions. Even in the optimal forearm position, the safe zones between the transfixion pin, vessel, nerve, or tendon was small, suggesting that open placement would be required. A cadaver study in 8 specimens demonstrated that the pins could be placed wit

BACKGROUND: The advantages of early fracture fixation in patients with multiple injuries have been challenged recently, particularly in patients with head injury. External fixation (EF) has been used to stabilize pelvic fractures after multiple injury. It potentially offers similar benefits to intramedullary nail (IMN) in long-bone fractures and may obviate some of the risks. We report on the use of EF as a temporary fracture fixation in a group of patients with multiple injuries and with femoral shaft fractures.
METHODS: Retrospective review of charts and registry data of patients admitted t

PURPOSE: To report capsular block syndrome with external blockage of the capsular opening by a posterior chamber lens fixated in the ciliary sulcus.
METHOD: Case report.
RESULTS: In an 89-year-old man who had undergone cataract surgery, a posterior chamber lens was accidentally fixated in the ciliary sulcus after continuous curvilinear capsulohhexis and phacoemulsification/aspiration. The next day, capsular block syndrome was noted along the posterior chamber lens optic, which was blocking the capsular opening from the outside.
CONCLUSION: In all previously reported eyes with capsular block

We compared Norian SRS, an injectable calcium phosphate bone cement, with external fixation in the treatment of redisplaced distal radial fractures by a prospective randomized study in 40 patients (women 50-80 years or men 60-80 years). After rereduction, the fracture was either stabilized by injection of SRS and immobilized with a cast for 2 weeks, or externally fixed with Hoffman's bar for 5 weeks. Each patient was evaluated at 2, 5, 7 weeks and at 3, 6 and 12 months. Functional parameters were grip strength, range of motion and pain. Radiographic parameters were radial angle, ulnar variance

Three types of surfaces for external fixation pins were compared. One hundred and eight stainless-steel tapered 5/6-millimeter pins were divided into three groups: thirty-six pins remained uncoated (Group A), thirty-six were plasma-sprayed with hydroxyapatite (Group B), and thirty-six were plasma-sprayed with titanium (Group C). The pins were implanted in the left tibia of eighteen sheep, with each sheep receiving six pins from the same group. A unilateral fixator then was assembled on the pins. The medial aspect of the mid-part of the tibial diaphysis was exposed, and a five-millimeter-long c

This study was undertaken to document whether a basic set of a new unilateral external fixation device could provide sufficient stability in complex musculoskeletal injuries involving upper and lower extremities and in patients with unstable pelvic ring disruptions. The initial clinical evaluation was performed in Sweden (stage 1) followed by field evaluation at the Swedish Field Hospital during the Mogadishu conflict in Somalia (stage 2). In stage 1, there were 90 patients with 116 acute limb injuries and six patients with an unstable pelvic ring disruption. Ninety limb injuries and all six p

Three calves, 3 to 6 months old, were admitted for evaluation and treatment of fractures of the forelimb that had resulted from use of forced extraction at birth. Each of these calves had clinical and radiographic evidence of delayed union of a fractured metacarpus. All calves were treated, using a type II external skeletal fixator consisting of 0.25-inch-diameter transcortical pins and acrylic connecting bars. Two calves required open reduction of the fracture to obtain anatomic alignment of the limb. On the basis of the 3 calves described here, it would appear that type II external skeletal

90 unstable fractures of the distal radius were studied in a randomized, prospective manner. Follow-up consisted of patient questionnaire, medical record review, therapist evaluation and radiography at 4 months, 1 year and 2 years post-operatively. Overall results were good or excellent in 94%. No significant differences were found between treatment groups (external fixation and pins and plaster) in final results, range of motion, intrinsic tightness, grip strength, or the presence of arthritis. The complication rate was high for both groups (45%), and half of the complications were major. Ext

A randomized, prospective study comparing Ender nailing with external fixation for open fractures of sixty-three tibiae (sixty patients) was undertaken. Ender nailing proved to be at least as effective as external fixation with respect to seven parameters: time to union, tibial alignment, total number of operations, ranges of motion of the knee and ankle, pain, presence of infection, and complications. Ender nailing is a safe alternative to external fixation for grade-I and grade-II open fractures. It should not be used for comminuted fractures, which might shorten about the nails.

INTRODUCTION: Circular frame fixation is a management method often used for complex fractures, or fractures with soft tissue compromise. Tibia fractures in older adults can be difficult to manage due to an association with osteoporosis, poor quality soft tissues, and a requirement for post-operative weight bearing to maintain function and independence. There is a paucity of evidence for the use of circular frames for tibia fractures in older adults. The available evidence suggests it is an effective treatment option, however, no study has compared treatment outcomes between older and younger p

BACKGROUND: Circular external fixation frames are widely used in limb reconstruction, however determining the optimal time for frame removal remains challenging. Current approaches rely on clinical and radiological assessment, both of which have recognised limitations. Mechanical assessment of construct stability offers a potential alternative, but existing approaches have not translated widely into clinical practice, in part due to reliance on absolute measurement thresholds.
OBJECTIVE: To determine whether force measurement within a circular external fixation frame varies systematically wit

In this retrospective, single-center, before-and-after comparison study, we report a simple and reliable method for securing cisternal or external ventricular drain catheters to the scalp to prevent accidental catheter displacement. This method involves fixation of the catheter at 2 points with loop sutures. A 10-year retrospective review was conducted in 39 consecutive patients with aneurysmal subarachnoid hemorrhage who underwent placement of either a cisternal drainage catheter (n = 15) or an external ventricular drainage catheter (n = 24) at our institution. Conventional single-point sutur

BACKGROUND: External fixation (EXFIX) is commonly used for managing unstable pelvic ring fractures, but its efficacy and safety remain unclear. This meta-analysis seeks to assess the efficacy and safety of EXFIX in comparison to alternative surgical interventions in patients diagnosed with unstable pelvic ring fractures.
METHODS: A systematic search identified relevant studies from inception to August 2023 in PubMed, EMBASE, the Cochrane Library, and Web of Science. Data on postoperative functional and imaging evaluations were extracted as primary outcomes, with postoperative complications an

Background: Displaced intra-articular calcaneal fractures are difficult to treat due to their complex anatomy and the high risk of soft-tissue complications. Although open reduction and internal fixation (ORIF) enables accurate anatomical reconstruction, it can be associated with substantial wound morbidity. Minimally invasive techniques have been developed to mitigate these risks. This study aims to describe a modified external fixation technique and report its clinical and radiographic outcomes in consecutive cases of patients with displaced intra-articular calcaneal fractures, with a minimu
