Gonadal steroid preservation of central synaptic input to hamster facial motoneurons following peripheral axotomy.

In recent work, we have demonstrated that testosterone propionate accelerates recovery from facial nerve injury in the adult male hamster. Central synaptic stripping following peripheral motor neuron damage is a well-established component of the injury response. Gonadal steroids regulate synaptogenesis in the normal nervous system. In this study, we tested the hypothesis that testosterone propionate administration at the time of facial nerve transection alters the synaptic connectivity of injured facial motoneurons. Adult hamsters were subjected to right facial nerve transection at the level o

Facial Nerve Surgery in the Rat Model to Study Axonal Inhibition and Regeneration.

This protocol describes consistent and reproducible methods to study axonal regeneration and inhibition in a rat facial nerve injury model. The facial nerve can be manipulated along its entire length, from its intracranial segment to its extratemporal course. There are three primary types of nerve injury used for the experimental study of regenerative properties: nerve crush, transection, and nerve gap. The range of possible interventions is vast, including surgical manipulation of the nerve, delivery of neuroactive reagents or cells, and either central or end-organ manipulations. Advantages o

Gunshot injuries of the temporal bone.

Despite an increasing incidence of gunshot wounds to the temporal bone, there is little in the literature regarding management of survivors of these serious injuries. Twelve patients were treated for such wounds between 1986 and 1994. The most frequent presentations were cranial nerve injury, especially facial paralysis (9 patients), hearing loss (7), vascular injury (4), and vestibular dysfunction (3). Persistent cerebrospinal fluid otorrhea was uncommon (1 patient) in this series. Computed tomography and audiovestibular testing were helpful in evaluating the severity of injury and guiding th

Cost and Inpatient Burden of Orbital Fracture Management: A 14-year Analysis.

INTRODUCTION: The complexity of orbital fracture management necessitates systematic investigation of the financial burden of these fractures and the cost-effectiveness of management. Here, we analyze the cost and inpatient burden of patients presenting with orbital fractures at an academic medical center. METHODS: A retrospective chart review of 1,449 patients presenting with orbital fractures between 2008 to 2022 was performed. Patient characteristics, treatment approach, length of stay (LOS), and hospital costs were collected. Linear regression and pairwise comparison tests were performed t

[Thierry's spatulas or forceps--a comparison between maternal-fetal morbidities].

OBJECTIVE: To compare maternal and fetal morbidity during forceps or Thierry spatulas operative deliveries. METHODS: Retrospective and desc … ted with severe perineal lesions. As far as fetal morbidity is concerned, there were more newborn's facial injuries using the forceps tool (18.18% vs 3.08%; P=0.0046). The remaining data were comparable. CONCLUSIONS: Thierry spatulas are less harmful than forceps to maternal tissues and reduce facial injuries of the newborn. Consequently, spatulas could benefit …

Life-threatening expansive sublingual hematoma: a stab wound with lingual artery injury.

Vascular injuries are a constant risk in facial trauma, although bone and soft tissues of the face have provided some protection to the larger blood vessels. However, penetrating injuries usually do not have this type of protection and can damage significant vascular arteries. This article presents a case of a stab wound, which led to airway obstruction arising to a large sublingual hematoma due to lingual artery injury. A healthy 44-year-old man was stabbed in the submandibular region and admitted with an airway obstruction. He was subjected to an emergency tracheotomy and evolved with progre

Ocular reconstruction after zygomatic complex fracture with retention of a foreign body.

Facial injuries with the retention of foreign bodies inside the tissues, both in soft and hard ones, can cause major functional and aesthetic damage. Among the different etiological agents, cutting tools, fragments of a firearm, the splinter of wood, steel, or iron, launched by misuse, or even caused by defects in equipment, are the main cause of these injuries. The aim of this study was to discuss the peculiarity of the multidisciplinary approach in caring of a 33-year-old man, victim of an accident at work, by the rupture of an emery disc and consequent penetration of the fragments in violat…

Orbital fractures in children.

In children, differences in the properties and proportions of bone in the craniofacial skeleton and the lack of development of the paranasal sinuses result in orbital fractures that present differently from those in adults. Facial growth may be disturbed by such injuries and also by surgical intervention, which should therefore be as conservative as possible. However, urgent operation is needed to prevent irreversible changes when fractures of the orbital floor involve entrapped muscle. We present an approach to such injuries.

Sequelae of unrecognized, untreated mandibular condylar fractures in the pediatric patient.

The mandibular condyle is a commonly involved site of maxillofacial trauma in children. However, this injury is often overlooked on initial physical examination in the emergency department. In cases involving blunt trauma to the chin it is important to suspect possible injury to the condyle of the mandible, as this type of facial injury can result in a spectrum of facial growth disturbances during the patient's later years. General practitioners and pediatric dentists are often the first clinicians to recognize the late complications of condylar trauma years after the injury. With a good clini

Boxing participation by children and adolescents: A joint statement with the American Academy of Pediatrics.

Thousands of boys and girls younger than 19 years of age participate in boxing in North America. Although boxing provides benefits for participants, including exercise, self-discipline and self-confidence, the sport of boxing encourages and rewards deliberate blows to the head and face. Participants in boxing are at risk of head, face and neck injuries, including chronic and even fatal neurological injuries. Concussions are one of the most common injuries occurring in boxing. Because of the risk of head and facial injuries, the Canadian Paediatric Society and the American Academy of Pediatrics…

Are Facial Gunshot Wounds More Fatal When They Are Self-Inflicted or Other-Inflicted?

STUDY DESIGN: This was a retrospective cohort study of the 2014 Nationwide Emergency Department Sample (NEDS). OBJECTIVE: Intraoral and submental projectile entry points may be less fatal than other facial entry points due to the indirect access to the intracranial structures and the protection offered by the intervening maxillofacial complex. Because intraoral and submental trajectories are almost always present in the setting of attempted suicide, this study sought to determine if intent (self-harm versus other-harm) influenced mortality in facial gunshot wound (GSW) patients. METHODS: All

Functional and Molecular Characterization of a Novel Traumatic Peripheral Nerve-Muscle Injury Model.

Traumatic injuries to human peripheral nerves are frequently associated with damage to nerve surrounding tissues including muscles and blood vessels. Currently, most rodent models of peripheral nerve injuries (e.g., facial or sciatic nerve) employ surgical nerve transection with scissors or scalpels. However, such an isolated surgical nerve injury only mildly damages neighboring tissues and weakly activates an immune response. In order to provide a rodent nerve injury model accounting for such nerve-associated tissue damage and immune cell activation, we developed a drop tower-based facial ner

Risk factors for death in elderly patients with facial fractures secondary to falls.

BACKGROUND: Craniomaxillofacial trauma is associated with severe injuries and disability. There are no recent reports analyzing the demographics of facial trauma in elderly patients. Our objective was to review the demographics, outcomes, and risk factors associated with death in elderly patients with facial fractures in a single-institution level I trauma center. METHODS: We performed a retrospective review of the University of Pittsburgh Medical Center Trauma Registry from January 1999 to December 2008 in an institutional review board-approved protocol. The search query included all types o

Etiology and patterns of facial lacerations and their possible association with underlying maxillofacial fractures.

BACKGROUND: This study was designed to analyze the etiology and patterns of soft-tissue facial lacerations associated with maxillofacial fractures and to identify associations between facial lacerations and underlying fractures. METHODS: Of 1960 patients who had been admitted for maxillofacial fractures between 2001 and 2010, only patients with complete clinical records presenting with facial lacerations were considered for this study. Facial lacerations were classified according to the MCFONTZL system. RESULTS: Of the 1960 patients admitted with maxillofacial fractures, 637 had 836 associat

Management of acute soft tissue injury to the auricle.

The external ear is commonly involved in facial trauma. Injuries to the ear can range from simple lacerations to complete avulsions. We review the normal auricular anatomy and vascular supply, as well as the initial management of any auricular injury. Furthermore, we review the literature on soft tissue injuries of the ear and present a simple algorithm for classifying injuries. The classification is based on whether or not cartilage is involved. Injuries to the lobule do not involve cartilage and thus are more easily repaired by simple closure or Z-plasty. Injuries involving cartilage are fur

Discerning a smile - The intricacies of analysis of post-neck dissection asymmetry.

INTRODUCTION: Iatrogenic facial nerve palsy is distressing to the patient and clinician. The deformity is aesthetically displeasing, and can be functionality problematic for oral competence, dental lip trauma and speech. Furthermore such injuries have litigation implications. Marginal mandibular nerve (MMN) palsy causes an obvious asymmetrical smile. MMN is at particular risk during procedures such as rhytidoplasties, mandibular fracture, tumour resection and neck dissections. Cited causes for the high incidence are large anatomical variations, unreliable landmarks, an exposed neural course an

A new approach to recording nasal fracture in skeletonized individuals.

OBJECTIVE: This work describes a new method for recording nasal fracture in skeletonized individuals, suitable for use in biocultural studies of violence and fracture in past societies. METHODS: The method consists in recording the 'side of fracture', 'side of deviation', 'type of fracture', 'other facial fractures', and stage of 'bone remodeling'. RESULTS: A lateral impact force to the facial area is typical of interpersonal violence. This may result in a unilateral nasal fracture and/or a laterally deviated nose. Given the predominance of right-handedness in human populations, side of frac
