A 29-year-old athlete suffered from vague hip pain during the preparation for a marathon competition. During the competition, severe pain forced him to drop out after 29 kilometres. He underwent conventional x-rays immediately, which showed a displaced lateral fracture of the femoral neck. The fracture was immediately treated with a dynamic hip screw. A few days later, the patient was discharged. He returned to sport upon completion of treatment. Vague hip pain after exercise may be an indicator for a rare stress fracture of the femoral neck and should be taken into consideration in athletes.

BACKGROUND: Patients presenting in the emergency department (ED) with a hip fracture are often in significant pain. Opioids are frequently prescribed as part of standardized pain protocols, but are associated with many adverse events, especially in the elderly. Regional anaesthesia with a femoral nerve block (FNB), Fascia Iliaca compartment block (FICB) or Pericapsular nerve block (PENG) may provide a reasonable alternative to these patients. We designed a prospective multicentre study to compare the efficacy and safety of these blocks in the ED setting, which has been registered in the Centra

OBJECTIVES: The Femoral Neck System (FNS) is a fixed-angle side plate device approved for use in fixation of femoral neck fractures. The FNS perforates the lateral cortex of the subtrochanteric femur, which may increase the risk of postoperative subtrochanteric fractures compared with the inverted triangle cannulated screw (CS) construct. The purpose of this biomechanical study was to compare forces required to create subtrochanteric fractures in FNS and CS constructs in a synthetic bone biomechanical model.
METHODS: Osteopenic sawbones were divided into five groups: Noninstrumented femurs, C

We examined the remains of an individual who was unearthed from the Tuchengzi site and was believed to be from the Warring States period in China. The remains exhibited segmental femoral fracture. We aimed to deduce the cause of fracture, medical interventions, healing process, and motion behavior after fracture healing using several techniques, including macroscopic observation, computed tomography (CT), and finite element analysis. Based on the morphology of the long bones, it appeared that the individual was male. The fractures resulted in an adduction angle of 5.47° and an anterior flexion

Femoroacetabular impingement syndrome (FAIS) is caused by a repetitive mechanical conflict between the acetabulum and the proximal femur, occurring in flexion and internal rotation. In cam impingement, bony prominences of the femoral head-neck junction induce chondrolabral damage. The acetabular type of FAIS, termed pincer FAIS, may be either due to focal or global retroversion and/or acetabular overcoverage. Combinations of cam and pincer morphology are common. Pathological femoral torsion may aggravate or decrease the mechanical conflict in FAI but can also occur in isolation. Of note, a hig

BACKGROUND AND OBJECTIVES: Cemented hip arthroplasty is considered the standard of care for treating both osteoporotic femoral neck fractures and pathologic disease of the proximal femur due to the ability to achieve strong fixation in poor quality bone. There is minimal literature evaluating uncemented arthroplasty for pathologic disease of the proximal femur. This objective of this study is to compare outcomes of patients undergoing cemented and uncemented arthroplasty of the proximal femur for an oncologic indication.
METHODS: Patients who underwent hip arthroplasty procedures in one healt

It is not clear how bone mineral density (BMD) measurements from several regions of lumbar spine and proximal femur should be utilized in assessing fracture risk. We examined how well the newest ISCD recommendations differentiate subjects with and without prevalent vertebral fractures in 187 postmenopausal women presenting for routine bone densitometry. The association between T-scores from proximal femur and lumbar spine sites and the probability of having a vertebral fracture was modeled via logistic regression with adjustment for age. The lowest T-score of any hip or spine sites (the curren

PURPOSE: Fracture union after osteosynthesis of a fracture neck femur (FNF) occurs by compression of the fracture ends and potential neck shortening. Selection of an implant for fixation of a femoral fracture of the neck can be challenging when making management decisions. Femoral neck shortening after internal fixation of FNFs using a femoral neck system (FNS) or multiple cannulated cancellous screws (MCS) was compared.
MATERIALS AND METHODS: This prospective interventional single-blinded randomized controlled trial was conducted at a university teaching hospital. Sixty patients undergoing i

BACKGROUND: Women with premature ovarian insufficiency (POI) lack oestrogen, which is a key determinant of bone growth, epiphyseal closure, and bone tissue organisation. Although dual-energy X-ray absorptiometry (DXA)-derived areal bone mineral density (BMD) remains the gold standard for fracture risk evaluation, it does not fully characterise the skeletal abnormalities present in these women. Hence, we aimed to assess hip/femur anatomy, strength, and geometry and femoral alignment using advanced hip analysis (AHA).
METHODS: We conducted a cross-sectional, case-control study including 89 wome

UNLABELLED: Trabecular bone is an intricate 3D network of struts and plates. Although the structure-function relations in trabecular bone have been studied since the time of Julius Wolff, controversy still exists regarding the architectural parameters responsible for its stability and resilience. We present a parameter that measures the angle between two connected trabeculae - the Inter-Trabecular Angle (ITA). We studied the ITA values derived from μCT scans of different regions of the proximal femora of 5 individuals of different age and sex. We show that the ITA angle distribution of nodes w

Cannulated screws within the femoral neck/head often need not be removed. Identifying and removing them can be a technical struggle, in some cases requiring hip dislocation or excessive operative time. In this article, we describe a novel technique that orthopedic surgeons can use to more effectively identify and percutaneously remove cannulated screws. Over the past 6 years, we have percutaneously removed 10 screws (from 7 patients) in an average time of 21 minutes per screw.

In situ fixation has been the gold standard for the treatment of slipped capital femoral epiphysis for some time. This technique has been popular despite obligate residual proximal femoral deformity due to the increased risk for catastrophic avascular necrosis of the femoral head with closed manipulation and historical open reduction techniques. As the body of evidence regarding long-term outcome has grown, it has become evident that early osteoarthritis is common after in situ or conservative treatment because of femoroacetabular impingement of the deformed femoral neck on the acetabular rim.

Persistent femoral neck cysts in an adult with healed Legg-Perthes disease had the radiological features of which are rather like those of simple bone cyst. To the author's knowledge such persistent cysts have not been reported previously. Existence of such cysts in healed Legg-Perthes' disease may provide a clue to the etiology of simple bone cyst.

Ten patients were treated for anterior or posterior displacement of the hip after an innominate osteotomy for congenital dislocation of the hip. All required a repeated open reduction with an additional procedure, either at the same time as reduction or as a second stage. Stable reduction was achieved in eight cases, but in two the initial attempt failed and the operation had to be repeated. The clinical features of hips with this complication are described and the technique of the salvage operation is discussed.

Tc-99m diphosphonate bone scans were performed on 11 children with slipped capital femoral epiphysis. On pinhole hip images, seven hips in seven patients had increased radionuclide uptake in the physis and adjacent proximal femoral metaphysis where the slip had occurred. Three hips in three patients had decreased radionuclide uptake in the femoral head on the side of the slipped epiphysis, indicating compromise of the femoral head blood supply. Three or more months following internal fixation, three children had scintigraphy that showed loss of the usual focal uptake in the physis and adjacent
