Fixation of Fresh Femoral Neck Fractures Using Fibular Strut Graft Along with Cannulated Screws.

BACKGROUND: Intracapsular femoral neck fractures can result from trivial trauma in the elderly or high-energy trauma in younger age-groups. In younger patients aged <60 years, the femoral head should be conserved to avoid the long-term complications of replacement arthroplasty. The options of osteosynthesis include closed/ open reduction and internal fixation with/without bone grafting. Internal fixation alone does not provide rigid fixation, owing to cavities in the posterior part of the femoral head and neck. Fibular grafts augment union and provide strength to the posterior cortex during

Failed Proximal Humerus Osteosynthesis Using Intramedullary Fibular Strut Allograft Conversion to Reverse Shoulder Arthroplasty.

Comminuted proximal humerus fractures are more frequently being treated utilizing an intramedullary fibular strut allograft to assist with open reduction internal fixation. Although this technique has led to improved outcomes, avascular necrosis, rotator cuff failure, and/or prominent hardware may still occur necessitating conversion to reverse shoulder arthroplasty. The presence of an intramedullary strut allograft makes this conversion technically demanding and creates certain potential complications unique to this group. This manuscript details the conversion of a proximal humerus open redu

Acute calcaneus tendon rupture associated with ipsilateral malleolar fracture: case report and literature review.

Malleolar fractures are rarely associated with lesions of the adjacent tendons or neurovascular structures. The association of ankle fractures with Achilles tendon rupture is even more infrequent, although both of these injuries are very common in and of themselves. To our knowledge, fracture of the lateral malleolus in association with an acute rupture of the ipsilateral calcaneus tendon has not been previously described. In this article, we describe a female patient who sustained an acute rupture of the Achilles tendon in conjunction with fracture of the ipsilateral lateral malleolus.

Hyperimmunoglobulinemia E syndrome: association with osteoporosis and recurrent fractures.

Hyperimmunoglobulinemia E (Job or Buckley) syndrome is an immunodeficiency disease associated with recurrent pyogenic infections, chronic eczematoid dermatitis, and mucocutaneous candidiasis. Association of this condition with osteoporosis and recurrent fractures is presented by a case report and by review of other reports describing similar findings. Other immunodeficiency syndromes with associated skeletal abnormalities are discussed.

[Kidney function disorders in "uncomplicated" skeletal trauma in children].

The renal function was studied in 198 children with non-complicated skeletal trauma (fraction of the femur, vertebral column and others). The renal function was found to be disturbed in 34,3% of the cases. The suitable treatment results in normal functioning of the kidneys within the 2nd week after trauma.

[Treatment of fractures of the ankle in the elderly].

BACKGROUND: Osteoporotic fractures of the ankle were observed three times more often in the year 2000 than in the year 1970 and it is predicted that this will increase another three times by the year 2030. The most important predictive values for ankle fractures in the elderly are smoking, multipharmacy and poor mobility. INJURY PATTERNS: Conservative treatment only seems to be successful in stable ankle fractures with good surrounding soft tissue. Pronation-abduction (PA) fractures most commonly affect elderly females and 90% of the cases present as the very unstable type III. Unstable fract

A 17-Year-Old Male With Hypoxemia After Long-Bone Fracture.

An otherwise healthy 17-year-old male patient presented to a periphery hospital with a compound fracture of the right distal tibia and fibula after a traumatic accident on a ski trip. He was treated empirically with IV cefazolin before undergoing open reduction with internal fixation with intramedullary nail for surgical fixation. Postoperatively, he became febrile, tachypneic, and hypoxemic, requiring up to 6 L/min supplemental oxygen by nasal prongs. He reported mild chest discomfort but denied productive cough, hemoptysis, or calf tenderness. Because of nonresolving oxygen demands, on posto

Treatment of ankle varus deformity due to physeal bar formation: A case report.

Treatment of ankle deformities caused by the physeal bar after a fracture sequel is complicated. Each patient should be treated individually depending on of the severity of the deformity and bar location. We presented a case report of a successful treatment of an 11-year-old male patient with progressive varus deformity and bar formation due to trauma in the distal tibial physis. Tibia distal open wedge osteotomy, fibula closed wedge osteotomy, bar resection, and autogenic adipose tissue interposition were performed. The patient is at 4 year follow-up without any complication. We think that ba

Three cross leg flaps for lower leg reconstruction of Gustilo type III C open fracture.

A 60 year old male had Gustilo type III C open fracture of the right lower leg. After radical debridement, the large open defect including certain loss of the bone tissue was successfully augmented and covered, by consecutive three cross-leg flaps, which consisted of the free rectus abdominis musculocutaneous flap, the fibula osteocutaneous flap and the conventional sural flap. Although indication for amputation or preservation is decided with multiple factors in each case, a strategic combination of cross-leg flap, free flap, external fixation and vascular delay could increase the potential o

Cefazolin-sodium has no adverse effect on fracture healing in an experimental rabbit model.

OBJECTIVES: This study aims to investigate whether cefazolin-sodium has any adverse effect on fracture healing in an experimental rabbit model. MATERIALS AND METHODS: The study was performed on 50 male New-Zealand white rabbits. Under general anesthesia, closed double fracture of middle one-third of the tibia-fibula of the left lower extremity of the subjects was produced by manual compression followed by closed reduction of fracture and long leg circular cast was applied. Subjects were divided randomly into five groups including 10 rabbits in each group. The first and second group were admin

Superficial Deltoid Ligament and Deep Deltoid Ligament Play Equally Important Roles in the Stability of Isolated Lateral Malleolus (OTA/AO 44-B1) Fractures: A Biomechanical Study.

OBJECTIVE: To evaluate the individual contributions to stability of the superficial and deep deltoid ligaments in the setting of SER IV ankle fractures. METHODS: Nineteen total cadaveric specimens were used. SER IV injuries were created with the rupture of either the superficial (SER IV-S) (n = 9) or deep deltoid (SER IV-D) (n = 10). These were tested by applying an external rotation force (1 Nm, 2 Nm, 3 Nm, and 4 Nm). Changes in the position of the talus were recorded with a 3D motion tracker. Injury conditions were compared with a 4-step general linear model with repeated measures. Injury c

Intramedullary Foreign Bodies within Fresh-Frozen Fibular Cortical Allograft: A Case Report.

CASE: A 71-year-old man with a proximal humerus fracture nonunion underwent surgery augmented by a fibular cortical strut allograft. On placing the allograft within the proximal humerus, fluoroscopic images showed 2 foreign bodies found to be broken drill bits located within the allograft. The drill bits were extracted, and the allograft was reprepared for use. CONCLUSION: After 2 years of follow-up, no significant sequela related to the foreign bodies was noted. To the best of our knowledge, this case report represents the first-of-its-kind occurrence of broken drill bits found within bone a

Fibular Fracture in a Female Rugby Player.

A 20-year-old female rugby player was injured when an opponent landed on her leg during a match. Twelve days after injury, the team's certified athletic trainer referred the patient to a physical therapist due to continued pain. Following fluoroscopic imaging, which was utilized by the physical therapist because standard radiographs were unavailable in close proximity, she was referred for radiographs, which demonstrated a midfibular diaphyseal fracture. J Orthop Sports Phys Ther 2016;46(7):608. doi:10.2519/jospt.2016.0411.

Corrective ankle osteotomies.

Corrective ankle osteotomies are periarticular osteotomies of the fibula, distal tibial metaphysis, or distal tibial metaphyseal diaphyseal junction. These osteotomies are indicated when angular, rotational, or translational malalignment is present. The author presents a review of the literature and in-depth surgical technique for each procedure, as well as a review of how to prevent and address the most common complications encountered.

Clinicoradiological Outcome of Concomitant Fractures of Proximal Femur and Femoral Shaft Treated With Second-Generation Cephalomedullary Nailing.

Background The ideal modality of treatment of concomitant fractures of the hip (intertrochanteric/femoral neck) and the femoral shaft is still evolving. The aim of our retrospective study was to assess the clinicoradiological outcome of such fractures managed by closed second-generation cephalomedullary nailing. Methodology The study was conducted among skeletally mature patients presenting within one week of injury who underwent closed second-generation cephalomedullary nailing (proximal femoral nail). Those presenting beyond one week or those who had pathological fractures, incomplete follow
