Time-Dependent Features of Complications Following Surface Replacement Arthroplasty for Primary Osteoarthritis of Proximal Interphalangeal Joint.

Background: The extended dorsal central splitting approach (EDCSA) in surface replacement arthroplasty (SRA) for primary osteoarthritis of the proximal interphalangeal (PIP) joint contributes to overcoming problems associated with the volar approach. However, new issues associated with the dorsal approach have become apparent. This study aimed to evaluate the time-dependent features of complications following SRA performed using the EDCSA for primary osteoarthritis of the PIP joint. Methods: This retrospective case series study included 188 patients (140 women and 48 men; 240 fingers) who unde

Collagenase Clostridium Histolyticum for the Treatment of Dupuytren Disease: A Delphi-Based Consensus Study.

PURPOSE: The aim of this study was to establish the consensus recommendations among hand surgeons who were experts in the use of collagenase clostridium histolyticum (CCH) on the appropriate treatment of Dupuytren disease in well-defined patient populations with varying degrees of disease severity and functional impairment. METHODS: A three-round, blinded, modified Delphi process examined panelists' approaches to CCH treatment of metacarpophalangeal (MP) or proximal interphalangeal (PIP) joint contractures involving one or two fingers with varying degrees of severity. Clinical scenarios relat

Boutonniere Deformity of the Proximal Interphalangeal Joint in a Young, Active Female Patient: A Nonstandard Surgical Procedure.

One of the key elements of post-traumatic osteoarthritis treatment is not only pain relief but also the return of joint function close to pre-injury levels. This is particularly important for young and physically active individuals. Unfortunately, the ability to return to satisfactory proximal intraphalangeal joint mobility after surgical treatment is often limited. This article describes the treatment of a young, active female patient who, several years ago, suffered a proximal interphalangeal (PIP) joint injury in her fifth finger following a fall from a height. Initially, the patient was tr

Motor cortical measures of use-dependent plasticity are graded from distal to proximal in the human upper limb.

In humans, it is well established that practicing simple, repetitive movements with the distal upper limb induces short-term plasticity in the neural pathways that control training. It is unknown how the neural response to similar training at more proximal joints differs. The purpose of this study was to quantify how ballistic training at proximal and distal upper limb joints influences measures of corticomotor plasticity. To accomplish this goal, we had subjects repetitively practice simple movements for 30 min using the index finger, wrist, or elbow. Before and after training, transcranial m

Amputation of the little finger.

We review 17 patients who had 21 amputations of the little finger. Transmetacarpal amputations gave satisfactory results in older patients, but in young patients loss of breadth of the palm caused significant disability. Disarticulation through the metacarpophalangeal joint is recommended in younger patients.

Humanoid finger with rigid-flexible-soft structure.

Soft robotic grippers offer adaptable and safe manipulation across diverse objects, but they often lack the output force and stability of rigid grippers. Developing fingers that deliver high force while delicately handling small or fragile objects remains a key challenge. Here, we design a humanoid finger with a rigid-flexible-soft structure, composed of rigid tubular bones, flexible joint springs with automatic reset and resistance to deformation under external forces, and a soft silicone skin. A pneumatic membrane actuator is placed between the bones and skin. To test its performance, we bui

The rheumatoid swan-neck deformity.

It is important to evaluate each swan-neck deformity to determine the mobility and radiographic condition of the PIP joint. This information forms a basis for logical treatment. In those fingers with little or no loss of motion, the treatment alternatives include DIP joint fusions, dermadesis, or flexor tenodesis. With significant loss of PIP joint motion, an attempt is made to first restore passive motion by manipulation and lateral band or skin releases. It then becomes essential to restore flexor tendon excursion. In those patients with destroyed joint surfaces the salvage procedures of fus

Arthroplasty for destroyed proximal interphalangeal joint in hand trauma surgery: Silicone hinged NeuFlex® or gliding Tactys®?

The treatment of proximal interphalangeal joints (PIP) after a traumatic injury has produced disappointing outcomes. The objective of this study was to evaluate the functional results of emergency hinged or gliding arthroplasty for destroyed PIP joints. A two-center retrospective study was carried out in 24 patients with open and closed trauma of the PIP joint of the fingers from 2007 to 2019. Fifteen hinged silicone implants (NeuFlex®) and nine gliding implants (Tactys®) were used. Pain on a visual analog scale (VAS), stability and total active motion (TAM), grip and pinch strength (Jamar®),

Comparison of the Thickness of Pulley and Flexor Tendon Between in Neutral and in Flexed Positions of Trigger Finger.

OBJECTIVE: This study aims to compare the morphology of the A1 pulley and flexor tendons in idiopathic trigger finger of digits other than the thumb between in neutral position and in the position with the interphalangeal joints full flexed and with the metacarpophalangeal (MP) joint 0° extended (hook grip position). METHOD: A total of 48 affected digits and 48 contralateral normal digits from 48 patients who initially diagnosed with idiopathic trigger finger were studied sonographically. Sonographic analysis was focused on the A1 pulley and flexor tendons at the level of the MP joint in the

Comparison of three local frame definitions for the kinematic analysis of the fingers and the wrist.

Because the hand is a complex poly-articular limb, numerous methods have been proposed to investigate its kinematics therefore complicating the comparison between studies and the methodological choices. With the objective of overcoming such issues, the present study compared the effect of three local frame definitions on local axis orientations and joint angles of the fingers and the wrist. Three local frames were implemented for each segment. The "Reference" frames were aligned with global axes during a static neutral posture. The "Landmark" frames were computed using palpated bony landmarks.

Common sports hand injuries. An overview of aetiology, management and prevention.

Injuries to the hand are among the most common in all of sports. Appropriate care should include prompt diagnosis and treatment. Dislocations of the digits should be reduced promptly, particularly the carpometacarpal joint of the thumb. Volar dislocations of the proximal interphalangeal joint need to be splinted in full extension, and the more common dorsal dislocations in slight flexion. Collateral ligament injuries of the fingers respond well to initial immobilisation followed by early motion. Although ligament injuries to the thumb metacarpophalangeal joint may be treated closed, they gener

"Selective Fasciotomy with Local Flap Interposition for Dupuytren Contracture: A Retrospective Analysis".

BACKGROUND: Dupuytren contracture is a fibroproliferative disorder that causes progressive flexion deformities of the fingers. Various traditional procedures have distinct benefits and limitations. We developed a novel surgical technique combining selective fasciotomy with local flap interposition to restore extension while preventing pathological reconnection. METHODS: This study adhered to the STROBE guidelines. A retrospective review was conducted on 64 patients (90 fingers; average age, 68.1 years) treated between 2011 and 2024 at two institutions with expertise in hand surgery. Pathologi

Fractures of the neck of the fifth metacarpal bone. Medium-term results in 28 cases treated by percutaneous transverse pinning.

The purpose of this study was to report the medium-term results in 28 patients affected by closed displaced fractures of the neck of the fifth metacarpal bone (boxer's fracture) with an associated severe swelling of the hand, who were treated with percutaneous transverse K-wire pinning, to verify the effectiveness of this surgical treatment. We opted for this treatment in all cases in which malrotation of the fifth finger and volar angulation of the metacarpal head greater than 30° were associated with a severe swelling of the hand. All the patients were reviewed clinically and radiologically

Bilateral trigger finger in a 5-year-old child: case report.

Trigger finger is rare in children. Herein, we report on a 5-year-old boy with trigger digits in several of his fingers since 1 month before which got worse. Triggering occurred in decreasing order of severity in proximal interphalangeal joints of the left middle, index, ring fingers and thumb. There was also triggering of the right index finger. The patient had no history of trauma, metabolic disorders and viral infections. The examination revealed triggering of the above-mentioned digits and was otherwise unremarkable. The patient was treated with physiotherapy (eight sessions) and non-stero

Long oblique/spiral mid-shaft metacarpal fractures of the fingers: treatment with cerclage wire fixation and immediate post-operative finger mobilisation in a wrist splint.

The cerclage wire technique of internal fixation for displaced long oblique/spiral metacarpal shaft fractures has not gained popularity for two reasons: many believe that wire migration is a real possibility and that the fixation technique is not rigid enough to allow immediate postoperative finger mobilisation. In this report, the authors review the results of 19 cases of long oblique/spiral mid-shaft metacarpal shaft fractures of the fingers treated by cerclage wires and immediate postoperative mobilisation of all finger joints. The study included 17 men and two women with a mean age of 35 (…

Wrist EMG-based Gestures Recognition for Finger and Wrist Motions.

Gesture recognition is a relatively natural humanmachine interface (HMI). Electromyography (EMG) based gesture recognition methods have been extensively investigated in upper limb prostheses as a special HMI, in which EMG sensors are mostly mounted on the proximal part of the forearm. However, for more general applications beyond upper-limb prosthetics, the wrist may be a more suitable position for more intuitive HMIs, in which independent finger movements in addition to wrist gestures can be realized. In this study, we propose to investigate the recognition performance for gestures of the ind
