Aorto-caval fistulas.

The surgical repair of 16 aorto-caval (A-C) fistulas (15 male and one female patient; average age of 61.3 years) is reviewed. Fourteen fistulas were caused by aneurysm's erosion, one by iatrogenic injury, while one followed abdominal blunt trauma. The interval from presumed occurrence to diagnosis ranged from 6 h to 2 years. The presence of an abdominal bruit (87.5%) was the most reliable physical finding. Congestive heart failure was prominent in three (18.7%) cases, while severe lower extremity edema in five (31.2%). Two patients (12.5%) had hematuria, two (12.5%) renal insufficiency, while …

Vaginal reconstruction is required in congenital absence of the vagina in the paediatric population and in patients with surgical (anterior … he field of vaginal reconstruction, especially when large bowel segments are utilized. Most vaginal fistulae occur after gynaecological surgery. There is a multitude of established techniques for closure of vaginal fistulae with comparable success rates, and two different approaches (vaginal or abdominal) may be used. [Tracheobronchial injuries and fistulas].

In association with perforating or blunt trauma, isolated injuries of the trachea and the bronchi are rarely seen. More frequently, however, … s of transmural lacerations are dyspnea, soft tissue emphysema, and pneumothorax. Tracheoesophageal fistulas result from decubital necrosis caused by long-term intubation. With spontaneous healing never occurring, surgical repair is carried out ideally by closing both openings in chronic fistulas, while simultaneous tracheal stenosis is treated by segment …

[Aorto-enteric fistulas].

INTRODUCTION: The aorto-enteric fistula (AEF) is a direct communication between aorta and intestinal lumen. There are primary and secondary forms. Primary AEFs are usually due to erosion of an aortic aneurysm (AAA) into the intestine, while secondary forms are caused by reconstructive procedures on the abdominal aorta. The incidence of primary AEF ranges from 0.1 to 0.8%, and secondary from 0.4% to 2.4% [2-4]. The mortality rate after surgical treatment of secondary AEFs is from 14% to 70% [5]. Therefore, they are of great medical importance. The aim of this paper is the presentation of 9 new …

[Treatment of anal fistulas].

The authors describe the traditional operative technique for correction of anal fistulae and analyse the outcome of surgical treatment. During a 5-years period between 1994 and 1998, 286 patients underwent surgery for anal fistula in the department--more than one--third of this population presented with recurrent disease. During the operation, the extrasphincteric segment of the anal fistula is excised and the margin of the sinus is marsupialized. Introducing a rubber band through the sinus tract eliminates lesions that penetrate the sphincter. As the tied band shears through the encircled sph…

[Postoperative intestinal fistulae].

58 (6.25%) intestinal fistulae raised after 928 operations performed for varied surgical small bowel diseases were studied. The primary operation was performed for bowel obstruction, acute peritonitis, small bowel tumours, postoperative eviscerations and for entro-mesenteric infarction. We also noticed that the surgery was performed for the small bowel diseases in 47 (81%) acses and for the diseases belonging to other abdominal viscera in the rest, having an emergency character in 81% of cases. The small bowel fistulae were single (51 cases) or multiple having the following topographic distrib…

Arteriovenous fistulae for hemodialysis.

Primary arteriovenous fistulae remain the gold standard for hemodialysis access. The radiocephalic or Brescia-Cimino fistula is the autologous fistula of choice, with a primary success rate of nearly 93% and a 1-year patency rate of up to 82%. Alternative arteriovenous fistulae include the snuff box fistula and a brachiobasilic fistula with a 1-year patency rate of up to 90%. Complications include thrombosis, infection, arterial steal syndrome, venous hypertension, aneurysms, and congestive heart failure. Despite these complications, arteriovenous fistulae provide good long-term hemodialysis a…

[Surgery of anal fistulas].

The authors report a study on 120 patients with anal fistula (111 males and 9 females). The average age was 44.3 years (median 44, SD +/- 14.807). 64.1% of patients had an intersphincteric fistula, 23.3% hanal transphincteric fistula, 1.6% a suprasphincteric fistula, 7.5% a horseshoe fistula and the 3.3% an extrasphincteric fistula. We treated 14 patients (11.66%) with direct surgical treatment. The other 106 had various types of treatment depending on the localisation and the involvement of the anorectal sphincter. We had 11 cases (9.1%) of complications, such as recurrence in 5 patients (4.1…

[Cholecysto-biliary fistulas].

Out of a total of 910 cholecystectomies performed on patients suffering from biliary calculosis, from January 1980 to December 1994 the Authors observed two cases of cholecysto-biliary fistulas (0.2%), a complication which can rarely affect biliary stones. In the light of the international literature and of their own, however, limited experience, the Authors take into account the pathophysiologic and clinical aspects of this pathology. They agree with the literature data on the lack of specificity of the clinical signs and uselessness of preoperative diagnostic tools, the only exception someti…

[Pulmonary arteriovenous fistulas].

Pulmonary arteriovenous fistulas are rare congenital malformations associated in 40% of cases with Osler-Weber-Rendu disease. PAF acquired in connection with the chest trauma, surgery and some inflammatory or neoplastic lung diseases were also described. In the Institute of Tuberculosis and Chest Diseases we have observed 11 cases of congenital PAF in 4000 resected lungs during the last years. In this report we described one more case with PAF. A 40 years old man was admitted to the Institute of Tuberculosis and Chest Diseases with hemoptysis. History and physical examination was unremarkable.…

[Renal arteriovenous fistulae].

Arteriovenous fistulas (AVFs) involving the kidney are rare. They are generally classified as congenital, idiopathic or acquired. The authors present 5 cases of renal arterio-venous communications observed in recent years. These cases don't include the congenital type and represent the forms of AVFs that are more frequently observed (1 idiopathic, 3 iatrogenic, 1 associated with malignancy). This study is aimed at examining the clinical picture and the procedures of detection and monitoring of renal AVFs. The various adopted therapeutic procedures (nephrectomy, surgical ablation, radiological …

[Urinary bladder fistulas].

During the period 1978 to 1991, 19 patients were treated for fistula of the urinary bladder, 12 of the fistulas were enterovesical, four vesicovaginal, one vesicouterine and two vesicocutaneous. Most of them occurred after diverticulitis or operation, or when the patient suffered from a malignant disease. Four patients received conservative treatment. One patient decided against an operation, two were inoperable, and one fistula closed with use of an indwelling catheter. 15 were operated on. 12 extirpations of fistula were carried out. Additional resection of intestine was performed in seven p…

Bacteriology of anal fistulae.

Anal fistulae are said to arise from cryptoglandular infection of the anal glands, which lie within the intersphincteric space. The type and virulence of the micro-organism responsible may determine whether an anal fistula develops. The microbiology of chronic anal fistulae has not been reported previously. Twenty-five consecutive anal fistulae were studied prospectively (eight intersphincteric fistulae, 12 trans-sphincteric fistulae, two suprasphincteric fistulae, one extrasphincteric fistula, one superficial fistula, one anovaginal fistula). There were 18 men and seven women, with a median a…

Cerebrospinal fistulas in children.

Cerebrospinal fluid leaks can be difficult management problems. Little information has been published about handling these cases in the pediatric population. A review of cases over the past 10 years at Boston Children's Hospital was undertaken to identify any prognostic factors and therapeutic recommendations. Twenty-seven patients were identified. While most patients responded to conservative therapy, those with leaks that persisted longer than 8 days required some type of operative closure. Severity of injury had little correlation with the occurrence of a leak. The findings in this study in

[Treatment of vesicovaginal fistulas].

The authors evaluated in a retrospective analysis their experience with treatment of vesicovaginal fistulae in 36 patients. Most frequently--almost in 90%--the fistulae developed after a gynaecological operation, in particular surgery on account of benign, but also malignant disease. The authors used for surgical correction of the fistulae a transvaginal approach in 28 (77.8%) patients, a transvesical approach in 5 (13.9%) patients and a transvesical and transperitoneal approach in two (5.5%) patients. One fistula receded spontaneously during drainage of the urinary bladder. Primary correction…
