Missed Monteggia fracture dislocations treated by open reduction of the radial head.

AIMS: Delayed diagnosis is a well-known complication of a Monteggia fracture-dislocation. If left untreated, the dislocated radial head later becomes symptomatic. The purposes of this study were firstly, to evaluate the clinical and radiological results of open reduction of the radial head and secondly, to identify the factors that may affect the outcome of this procedure. MATERIALS AND METHODS: This retrospective study evaluated 30 children with a chronic Monteggia lesion. There were 18 boys and 12 girls with a mean age of 7.4 years (4 to 13) at the time of open reduction. The mean interval

A Stepwise Approach to Management of Open Radiocarpal Fracture-Dislocations: A Case Report.

Radiocarpal dislocations are uncommon and occur after significant trauma. We describe a unique case of open radiocarpal fracture-dislocation presenting with progressive neurovascular compromise. Staged management was necessary. As a first stage, emergent provisional bedside reduction in the emergency room with manual pressure through the open wounds was performed. The second stage then involved formal open reduction and internal fixation as soon as operating room staff and resources became available.

Fracture dislocation of the pisiform bone in children: a case report and review of the literature.

Simultaneous fracture and dislocation of the pisiform is a rare condition, and only two cases have been reported in children. We retrospectively collected and reviewed clinical and radiographic data of a child with a type II Salter-Harris fracture of the distal radius, associated with fracture dislocation of the pisiform. In addition, we performed a systematic review of the literature available to date.

Delayed Migration of Greater Tuberosity Fractures Associated With Anterior Shoulder Dislocation.

OBJECTIVES: Treatment of greater tuberosity (GT) fractures occurring during anterior shoulder dislocation generally consists of initial closed reduction of the shoulder. Undisplaced fractures are treated conservatively, whereas displaced fractures generally undergo surgical management. Our hypothesis is that many well-reduced GT fractures will migrate. The primary goal of this study is to evaluate the outcomes of GT fractures associated with shoulder dislocation to optimize their management. DESIGN: Retrospective review with prospective outcomes. SETTING: Tertiary Level 1 trauma center. PAT

A severely displaced metaphyseal fracture of the proximal humerus with dislocation of the shoulder in a child.

Fracture of the proximal humerus metaphysis with coexistent dislocation of the shoulder in children is a rare injury. The injury often occurs as a consequence of high velocity trauma. Most fractures of the proximal humerus commonly associated with the epiphysis in children can be treated with closed reduction. We presented a case of 5-year-old girl who sustained this type of fracture- dislocation of the shoulder. Open reduction and internal fixation with multiple smooth K-wires was performed. At two years follow-up, the patient was pain free and regained full range of motion.

Functional outcomes after fixation of "terrible triad" elbow fracture dislocations.

Historically, the published literature on "terrible triad" injuries has shown a high rate of unacceptable results. The use of systematic treatment protocols may improve functional outcome. The authors performed a retrospective study of all patients aged 18 years or older who underwent surgical treatment for "terrible triad" elbow fracture dislocation at their institution over a period 7 years. Surgical treatment involved fixation or replacement of the radial head, repair of the anterior capsule or coronoid fracture in most cases, and repair of the lateral collateral ligament. Outcomes included…

Ulnar variance as a predictor of persistent instability following Galeazzi fracture-dislocations.

BACKGROUND: We investigated the radiographic parameters that may predict distal radial ulnar joint (DRUJ) instability in surgically treated radial shaft fractures. In our clinical experience, there are no previously reported radiographic parameters that are universally predictive of DRUJ instability following radial shaft fracture. MATERIALS AND METHODS: Fifty consecutive patients, ages 20-79 years, with unilateral radial shaft fractures and possible associated DRUJ injury were retrospectively identified over a 5-year period. Distance from radial carpal joint (RCJ) to fracture proportional to

Pediatric terrible triad elbow fracture dislocations: report of 2 cases.

An elbow dislocation associated with radial head and coronoid process fractures, the terrible triad injury, has an unpredictable outcome in adults and is rare in children. We present 2 such injuries in children, 1 combined with an olecranon fracture, and both with good early clinical outcomes. However, in 1 of the 2 cases, avascular necrosis of the proximal radius was evident on radiographs.

Recurrent shoulder dislocations secondary to coracoid process fracture: a case report.

Coracoid process fracture is easily missed in recurrent anterior shoulder dislocation. We report one such case in a 48-year-old man. Radiology revealed the Bankart lesion and the Hill-Sachs lesion only; the coracoid process fracture was discovered intra-operatively. The anatomy of the shoulder was restored by fixing the fragment to its scapular remnant with a 4-mm cannulated cancellous screw. The tip and the proximal fragment were reamed before inserting the screw. This fragment was routed with attached short head of biceps and coracobrachialis through the lower one third of the subscapularis,

Complex fracture-dislocations of the proximal ulna and radius in adults: a comprehensive classification.

BACKGROUND: Complex fracture-dislocations of the proximal ulna and radius include multiple anatomic lesions, the management of which is known to be demanding. Although several classifications have been proposed, none appear to be exhaustive, and most of them have neither therapeutic nor prognostic value. The purpose of this study was to design a comprehensive classification that may provide a guide for the operative management of these injuries. MATERIALS AND METHODS: The classification is aimed at identifying definite anatomic lesions, called the "main lesions," the presence of which can aff

[Dislocated basicervical fracture of the femoral neck in a 7-year-old girl].

This report discusses the treatment of a seven-year-old girl with a displaced fracture of the proximal femur (AO 32-M / 3.1 II, Delbet type III) after falling 4 m from a tree. She was treated by immediate (within 6 h after trauma) open reduction and capsular decompression. The fracture was stabilised with 2 cannulated titanium screws that did not go beyond the epiphyseal cartilage. Clinical and radiological examination after 2 years showed no complications, especially no sign of avascular necrosis (AVN) of the femoral head. The functional outcome with Ratliff's classification was good. In acco

[Osteosynthesis with Kirschner wires in the treatment of dislocated supracondylar humeral fractures in children].

INTRODUCTION: Elbow fractures account for 7-9% of all fractures in childhood, while the supracondylar type is the most common (up to 70%). 84% of all supracondylar elbow fractures occur under the age of 10 years (most often between the age of 5 and 8). The aim of this study was to present the technique and results of treatment of supracondylar fractures with grade III fragment dislocation in pediatric population. Treatment included orthopedic repositioning and percutaneous pin fixation. MATERIAL AND METHODS: Over the period 1991-2000, 255 patients were hospitalized due to supracondylar fractu

[The direct, dorsal approach to the treatment of unstable tibial posteromedial fracture-dislocations].

The surgical treatment of highly unstable tibial posteromedial fracture-dislocations (medial split fracture,Moore type I) is demanding and may cause significant problems. The anterior and the posteromedial approach require detachment of the medial capsuloligamental structures or arthrotomy of the knee joint. Neither of these operative techniques allow the optimal posterior positioning of the buttress plate in order to prevent caudal dislocation of the fragment during knee flexion. The published posterior approaches to the proximal tibia allow appropriate positioning of the anti-glide plate. Ho

Risk factors of neurological lesions in low cervical spine fractures and dislocations.

Eighty-nine patients with lower cervical spine fractures or dislocations were evaluated for risk factors of neurological lesion. The age, sex, level and pattern of fracture and sagittal diameter of the spinal canal were analysed. There were no significant differences on the age, gender, level and Torg's ratio between intact patients and those with nerve root injury, incomplete or complete spinal cord injuries. Bilateral facet dislocations and burst fractures are a significant risk factor of spinal cord injury.

Treatment of locked posterior fracture-dislocations of the shoulder by total shoulder arthroplasty.

Total shoulder arthroplasty is indicated in the treatment of locked posterior fracture-dislocations of the shoulder where there is extensive damage to the articular surfaces of the humerus and glenoid. This report reviews the surgical technique, postoperative rehabilitation program, and functional results of seven shoulders in five patients who had locked posterior dislocations of the shoulder. Activities involving overhead positioning of the arm and heavy lifting continued to be challenging for this group of patients. The American Shoulder and Elbow Surgeons' Shoulder Score improved from 20.1

Interphalangeal dislocation of the fourth toe with avulsion-fracture in a child: report of a case.

Phalangeal dislocations of toes are extremely rare in childhood and usually can be treated by closed reduction. We present a proximal interphalangeal dislocation of the fourth toe with an irreducible avulsion fracture of the middle phalanx requiring open reduction. To our knowledge concomitant avulsion fractures in this condition have not been reported thus far in pediatric patients. The pathological mechanism of this injury is discussed, and the significance of the plantar plate for joint stability is emphasized.

Combined fracture of the hook of the hamate and palmar dislocation of the fifth carpometacarpal joint.

A case of fracture of the hook of the hamate associated with a palmar dislocation of the fifth carpometacarpal joint is described. Surgical exploration revealed that the different elements of the flexor carpi ulnaris musculotendinous unit, which includes the pisiform, the pisohamate and pisometacarpal ligaments, were displaced proximally together with the avulsed fragment of the hook of the hamate and the fifth metacarpal. These findings suggest that such an injury results from a sudden, violent contraction of the flexor carpi ulnaris against the fixed wrist. It was successfully treated by ope

Unusual posterior hip dislocation with ipsilateral fractures of the femoral neck and head.

An unusual combination of posterior hip dislocation with ipsilateral femoral neck and head fractures is reported. The femoral head migrated too far inferiorly in the posterior part of the thigh, making it different from the previously reported solitary case report of obturator hip dislocation associated with femoral neck and head fractures.
